A total of 50 patients (seven males and 43 females) with an age range of 34 to 60 who had the diagnosis of lateral epicondylitis were included in the study. The patients were distributed into two groups. In group I, 25 patients (21 females and four males) were given a lateral epicondyle bandage. In group II, 25 patients (22 females and three males) were given a wrist resting splint holding the wrist in slight dorsiflexion. Evaluations of the patients were done before treatment and at the second and sixth weeks of treatment. Evaluation parameters were pain during rest and movement, sensitivity, algometer score, hand grip strength, and evaluation of the response to treatment. The response to treatment was evaluated according to the following categories: excellent, good, medium, and bad. In group I, only pain during rest and movement significantly decreased at 2 weeks while significant improvement was obtained for all parameters at 6 weeks. In group II, all parameters except for algometric sensitivity showed significant improvement at 2 weeks. Significant improvement was obtained for all parameters at 6 weeks in this group. Comparison of the two groups showed significantly better improvement in resting pain in group II at 2 weeks while there was no difference for other parameters including response to treatment at either evaluation stage. Braces might be a good strategy to help wait out the natural course of tennis elbow complaints. Although epicondyle bandage was not found to be superior to wrist splint in our study, we may suggest that it could be favored over splint since it is more practical and cosmetically acceptable.Item Efficacy of a self-management program for osteoporotic subjects(Lippincott Williams & Wilkins, 2007-08) Alp, Alev; Kanat, Elvan; Yurtkuran, Mustafa; Uludağ Üniversitesi/Tıp Fakültesi/Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı.; ABG-2019-2020; 56256023200; 23980622500; 55408539300Objective: This study is based on whether the self-management program choices For Better Bone Health is effective to promote behavioral strategies for improving bone health, life quality, pain perception, physical function, and balance in osteoporotic subjects. Design: In this single-blind, randomized controlled study, a total of 50 sedentary women with postmenopausal and idiopathic osteoporosis were selected from the outpatients of AtatUrk Balneotherapy and Rehabilitation Center according to their physical activity level and T scores of dual-energy x-ray absorptiometry as the inclusion criteria. Fifty sedentary women with BM D T scores of -2.5 or lower were randomized into two groups (self-management group: group 1; and control group: group 2) and enrolled in a 6-mo study. Participants attended self-management class once a week for 5 wks. Evaluations were done at baseline, at the end of the fifth week, and at the sixth month. Pain-intensity evaluation by Visual Analogue Scale (VAS), life-quality assessments by SF-36, balance testing by Sensitized Romberg Test (SRT), and functional assessment by Timed Sit to Stand test (TSS) and a simple questionnaire were the outcome measures. Results: When the groups were compared by change scores and percentages of change, improvements observed in pain intensity by VAS (P < 0.001), SF-36 Physical Function (P < 0.001), SF-36 Physical Role Limitations (P < 0.001), SF-36 Social Function (P < 0.001), SF-36 Mental Health (P < 0.001), SF-36 Vitality (P < 0.01), SF-36 Pain (P < 0.001), SF-36 General Health Perceptions (P < 0.05), SF-36 Emotional Role Limitations (P < 0.01), SIRT eyes open (P < 0.001), SRT eyes closed (P < 0.001), and TSS (P < 0.001) were determined to be superior in group 1 at the end of the sixth month. Seventy-four percent of patients in group 1 engaged in regular physical activities, and 92% of them declared that they understood the purpose and benefits of medications and dietary calcium intake. Fifty-seven percent of them formed personal plans for preventing traumas, whereas 8% of the subjects in group 2 experienced new falls but no fractures. Conclusion: It is determined that the self-management class led to improvements in functional, balance, and life-quality outcomes and to reductions in pain perception.Item El osteoartritli hastalarda topikal %5 ibuprofenin etkinliğinin araştırılması(Uludağ Üniversitesi, 2009) Kanat, Elvan; Yurtkuran, Merih; Uludağ Üniversitesi/Tıp Fakültesi/Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı.Bu çalışmada, el osteoartritli hastalarda %5 topikal ibuprofenin klinik etkinliğinin araştırılması amaçlandı. Yaşları 41 ile 80 arasında değişen toplam 50 kadın hasta çalışmaya alındı. Hastalar iki gruba randomize edildi (Grup 1; ev bazlı el egzersizi grubu, Grup 2;%5 topikal ibuprofen ve ev bazlı el egzersizi grubu:). Hastalar ev egzersizlerini 16 hafta süreyle gerçekleştirdi. %5 topikal ibuprofen ağrılı el üzerine günde 3 kez 10 cmlik strip şeklinde, 4 hafta uygulandı. Tedavilerin etkinliği 4 ve 16 hafta sonra değerlendirildi ve sonuçlar tedavi öncesi ile karşılaştırıldı. Ağrı ve katılığın değerlendirilmesi için Vizüel Analog Skala (VAS), fonksiyonların değerlendirilmesi için AUSCAN ve Duruöz indeksi ve yaşam kalitesinin değerlendirilmesi için SF–36 kullanıldı. Her iki tedavi yöntemi de el osteoartritli hastalarda etkili bulundu. Bununla birlikte, fark skorları ve yüzde değişimleri açısından gruplar arasında yapılan karşılaştırmada, VAS ile ağrı (p<0.001), SF–36 fiziksel fonksiyon skoru (p=0.045), SF–36 ağrı skoru (p=0.001), SF–36 vitalite skoru (p=0.009), katılık skoru (p<0.001) ile AUSCAN ve Duruöz indeks skorlarındaki (p<0.001) iyileşmenin 16. haftanın sonunda grup 2’de daha fazla olduğu saptandı. %5 topikal ibuprofen el osteoartritli hastalar için umut verici bir tedavi olabilirPublication Magnetotherapy in hand osteoarthritis: A pilot trial(Churchill Livingstone, 2013-12-01) Kanat, Elvan; Alp, Alev; Yurtkuran, Merih; ALP, ALEV; Yurtkuran, Merih; Uludağ Üniversitesi/Tıp Fakültesi/Fizik Tedavi ve Rehabilitasyon Bölümü; ABG-2019-2020; GLB-5791-2022Objective: To evaluate the effectiveness of magnetotherapy in the treatment of hand osteoarthritis (HO).Methods: In this randomized controlled single-blind follow-up study, patients with HO were randomly assigned into 2 groups (G1 and G2). The subjects in G1 (n = 25) received 25 Hz, 450 pulse/s, 5-80G, magnetotherapy of totally 10 days and 20 min/day combined with active range of motion/strengthening exercises for the hand. G2 (n =25) received sham-magnetotherapy for 20 min/day for the same duration combined with the same hand exercises. Outcome measures were pain and joint stiffness evaluation, handgrip and pinchgrip strength (HPS), Duruoz and Auscan Hand Osteoarthritis Indexes (DAOI) and Short Form-36 Health Questionnaire (SF-36) administered at baseline, immediately after treatment and at the follow up.Results: When the groups were compared with each other, improvement observed in SF-36 Pain (p < 0.001), SF-36 Social Function (p = 0.030), SF-36 Vitality (p = 0.002), SF-36 General Health (p = 0.001), Pain at rest (p < 0.001), Pain at motion (p < 0.001), Joint stiffness (p < 0.001), DAOI (p < 0.001) were in favor of G1.Conclusions: Changes in pain, function and quality of life scores showed significant advantage in favor of the applied electromagnetic intervention in patients with HO.Item Primer osteoporoz hastalarında demografik veriler, biyokimyasal ölçümler, DXA değerleri ve kırık arasındaki i̇lişki(Uludağ Üniversitesi, 2008-05-28) Kanat, Elvan; Çelen, Hüseyin; Yıldırım, Murat; Dıraz, Nuray; Alp, Alev; Yurtkuran, Melih; Uludağ Üniversitesi/Atatürk Rehabilitasyon Uygulama Araştırma Merkezi.Çalışmanın amacı osteoporoz hastalarında demografik özellikler, vücut kitle indeksi (VKI), kalsitriol, parathormon (PTH), alkalen fosfataz (ALF) düzeyleri ile dual-enerji-X ray absorpsiyometri (DXA) değerleri ve kırık oluşumu arasındaki ilişkiyi araştırmaktır. Osteoporoz tanı kriterlerine uyan 310 hasta değerlendirildi. Hastaların serum kalsiyum, kalsitriol, PTH, ALF düzeyleri ve VKİ'leri tespit edildi. DXA lomber total (LT), femur boyun (FB) ve femur total (FT) değerleri ölçüldü. Torakal, lomber yan, pelvis grafileri çekildi. Kadınlarda FT değerleri ve VKI, LT ile FB ve FT; FB ile FT arasında pozitif korelasyon; PTH ile kalsitriol arasında negatif korelasyon saptandı. Erkeklerde LT ile PTH arasında pozitif korelasyon saptandı. 113 (%36.5) hastada kırık saptandı. Bu hastalarda PTH düzeyi kırığı olmayanlara göre daha yüksekti. Kırığı olan kadın hastaların VKİ değerlerinin daha yüksek olduğu görüldü. VKI ile kırık ve erkeklerde LT DXA ile PTH düzeyleri arasında pozitif korelasyon saptanmış olması dışında verilerimiz literatürle uyumludur. Literatürle çelişkili sonuçlar hasta sayımızın azlığıyla ilişkili olabilir.
