Item Ağrılı göze sahip terminal glokom olgularında siklokrioterapi(Uludağ Üniversitesi, 2014-11-21) Baykara, Mehmet; Başyaka, Kevser; Yayla, Uğur; Budak, Berna Akova; İncebıyık, İlker; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.Siklokriyoterapi non-invaziv ve göreceli olarak uygulaması kolay bir yöntemdir. Bu çalışmada ağrılı göz nedeniyle siklokriyoterapi uygulanmış hastalarda uygulamanın etkinliği ve komplikasyon gelişme oranları araştırıldı. Uludağ Üniversitesi Tıp Fakültesi Göz Hastalıkları Kliniği’nde Ocak 2012 –Şubat 2014 tarihleri arasında siklokriyoterapi uygulanmış olan 38 hastanın 39 gözü retrospektif olarak incelendi. Hastaların yaş ortalaması 67,5 ±10.6 idi. Otuz bir göz neovasküler glokom tanısı almıştı. Yedi göz son dönem primer açık açılı glokom, 1 göz açı kapanması glokomu tanısı almıştı. Tedaviyi takiben ortalama göz içi basıncı düşüşü primer açık açılı glokomda 9,2 ±14.3 mm Hg iken, neovasküler glokom grubunda 14.1±14.7 mm Hg idi. Göz içi basınç kontrolü tüm gözlerin sadece 17’sinde (% 41) sağlanabilmişti, primer açık açılı glokomu grubunda 1 gözde (% 14), neovasküler glokom grubunda 13 gözde (% 41) gib kontrolü sağlanmıştı. Ağrılı gözlerin % 79 ’unda tedaviyi takiben rahatlama sağlanmıştı. Takiplerde hiçbir hastada ftizis bulbi ya da hipotoni gelişmedi. Siklokriyoterapinin çeşitli glokom türlerinde basınç kontrolü olmadan da iyi ağrı kontrolü sağladığı sonucuna varabiliriz.Item Association of birth parameters with corneal thickness in prematurely born and full-term school-aged children(Lippincott Williams & Wilkins, 2016-01-05) Olcaysu, Osman Okan; Kıvanç, Sertaç Argün; Budak, Berna Akova; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.; 0000-0003-0995-5260; 0000-0002-0932-6977; AAH-6518-2021; ABE-3033-2020; 47861204900; 57188831894Purpose:The objective of this study was to investigate and compare corneal thicknesses of prematurely born and full-term early school-aged children, who were grouped into birth weight categories according to gestational age.Methods:This cross-sectional study included 136 school-aged white children. They were grouped as premature (born before 37 weeks of gestation) and full term (born at or after 37 weeks of gestation). These 2 groups were further divided into birth weight groups according to gestational age as small, appropriate, and large for gestational age. Central corneal thickness (CCT) was measured by ultrasound pachymetry. The intraocular pressures of both groups were measured by a Goldmann applanation tonometer.Results:The study included right eyes of 136 children. There were 53 girls and 83 boys whose mean age was 6.5 0.9 years (range: 5-8 yrs). Mean pachymetry of the small for gestational age group (526.8 +/- 37.9 m) in prematurely born children was significantly lower than that of appropriate for gestational age (554.6 +/- 34.2 m) (P = 0.039, mean difference 27.8, 95% confidence interval, 1.1-54.5). When prematurely born and full-term children are compared, the mean pachymetric values of the term children (574.5 +/- 37.8 m) were found to be significantly higher than those of the prematurely born children (545.4 +/- 35.6 m) (P 0.001).Conclusions:Birth parameters affect CCT in early school-aged children. Prematurely born, small for gestational age children have lesser CCT. In this study, our aim was to assess the efficacy of tea tree oil in Demodex treatment on caucasian patients in an industrialized region of Turkey, and to develop a systematic scoring system for extremely accurate diagnosis in the absence of advanced facilities. Material/Methods: Charts of 412 patients with blepharitis were reviewed. A group of 39 out of 412 cases were identified as chronic and treatment-refractory, and therefore were enrolled in this study. Eyelashes from each of the lower and upper eyelids of both eyes were evaluated at x40 and x100 magnification using light microscopy. Treatment was started with 4% tea tree oil eyelid gel and 10% eyelash shampoo. Symptoms and findings were scored according to the most common complaints. Results: The mean age of the patients was 54.1 +/- 15.4 years. Seventeen (43.5%) patients were male and 22 (56.5%) patients were female. In 30 out of the 39 patients (76.9%) D. folliculorum was detected. Symptoms disappeared in 25 patients. The mean score of patients who were Demodex-negative was 2.7 +/- 1.0, and the mean score of patients who were Demodex-positive was 3.8 +/- 1.6 (p=0.047). Ninety-four percent of those with a score of 4 and over were found to be Demodex-positive (p=0.025). Conclusions: Treatment with tea tree oil can be successful. The aim of this study was to report the ocular surface toxicity and scoring in patients whose treatment shifted to topical linezolid/ceftazidime from topical vancomycin/ceftazidime due to their vancomycin intolerance. Methods: A retrospective, interventional case series of bacterial keratitis was treated with topical linezolid (one drop of 0.2% solution per eye), administered hourly until epithelization and then gradually decreased. The number and extent of punctate epithelial erosions were noted across the entire surface of the cornea. Ocular discomfort was assessed by means of (a) patient-reported pain upon instillation of the medication (vancomycin/linezolid), (b) reported burning sensation between doses and (c) reported foreign-body sensation. No ocular surface toxicity related to linezolid use was noted. Patients were followed for at least 2 months after treatment between April and December 2013. Results: Of the seven patients included in the study (age range: 2-88 years; five females, two males), complete epithelization and resolution was achieved in five patients. One patient was treated with linezolid after penetrating keratoplasty. The second culture of another patient with impending perforation despite linezolid/ceftazidime therapy yielded Fusarium spp., so he underwent tectonic keratoplasty. The mean ocular surface score was 9.4 +/- 1.6 during vancomycin treatment and 5.9 +/- 1.3 during linezolid treatment after discontinuation of vancomycin. The topical linezolid score was significantly lower (p = 0.027). Visual acuity categories were defined according to the World Health Organization. The injury types and zones of the open globes were classified according to Birmingham Eye Trauma Terminology System.Results: Among 479 patients with work-related eye injuries in 5 years, there were 102 eyes of 101 patients with open globe injuries (21%). The mean age of the patients was 34.5 +/- 8.9 years with a mean follow-up of 12.5 +/- 12.6 months. The injuries peaked in June in the hour between 12: 00 and 13: 00. Eighty-six percent presented to emergency services within 12 hours after the injury. Twenty-two percent of the patients had been wearing protective eyewear at the time of injury. The open globe injuries were penetrating in 51%, intraocular foreign body in 40%, rupture in 7% and perforation in 2% of the eyes. The most frequent finding was traumatic cataract. Final visual acuity of 33.3% of patients was below 3/60. Seventy-eight percent of patients that had visual acuity worse than 6/18 at presentation had visual acuity of 6/18 or better at final visit. Sixty-three percent of eyes which had injuries involving all 3 zones resulted in phthisis bulbi, enucleation or evisceration.Conclusion: Work-related open globe injuries may have severe consequences such as visual impairment and blindness among the young male working population in industrialized areas. Materials and Methods: The medical records of the patients under 18 years old who had open-globe injury were retrospectively reviewed. Age, gender, the cause of the trauma, presence of intraocular foreign body and the extent of the ocular damage were recorded. Results: The study was comprised of 79 patients (23 female, 56 male) with a mean age of 7.7±4.4 (range: 2-18 years). Thirty-nine patients were injured with a blunt tip object, 27 with a sharp object and one has rupture with trauma. Thirty-five injuries occurred outside and 45 at home. The season in which most of the injuries took place was summer (32%). Seventy-five percent of the injuries were noted in 11 years and under. The mean age subject to blunt tip object injury, organic body injury and outdoor injury was significantly higher compared to that of with sharp object, inorganic body injury and indoor injury. Conclusions: The open globe-injury rates in Northwest Turkey is found to be higher in smaller age groups. Damage occurs in many end organs in diabetic patients. Eyes are also among the end organs and DM causes morbidity related to diabetic retinopathy. It is estimated that 10 % of the diabetic patients have visual impairment and 2 % have blindness. Diabetic macular edema is the main cause for visual impairment in patients with diabetic retinopathy. Treatment of the DME gains importance with new treatment modalities in last decade. Material and Method: The medical records of patients with ocular injuries who referred to Department of Ophthalmology or emergency of Uludag University, School of Medicine between January 2010 and December 2013 with official occupational accident report were retrospectively reviewed. The patients with closed globe injuries following trauma were included. Age, sex, the injured eye, the cause of the trauma, whether the precautions were taken or not by the patient, the damage due to trauma were recorded. Results: According to the official records, 108 patients referred to our clinic with closed globe injury related to occupational accident. One hundred twenty eyes of 108 patients (2 females, 106 males) were evaluated. The mean age of the patients was 33 +/- 8.6 years. The most frequent cause of injury was foreign bodies on the ocular surface followed by blunt trauma. The mean age of the patients injured with foreign bodies was found to be significantly lower than the patients injured with blunt objects (p=0.039). Thirteen patients reported that they had used preventive equipment. Discussion: It is of utmost importance that the awareness of the workers should be raised and they should be educated about the use of preventive equipment to prevent the occupational eye injuries. The education of particularly the younger patients about the occupational injuries when they begin to work may decrease the rate of occupational accident related eye injuries.Item Göz polikliniğine başvuran ve ilaç reçetesi verilmiş hastaların tedavi uyumlarının incelenmesi(Uludağ Üniversitesi, 2013-11-14) Gelişken, Öner; Budak, Berna Akova; Toka, Fatih; Baykara, Mehmet; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.İlaç uyumu tedavinin en önemli basamaklarındandır. Biz bu çalışmada göz polikliniğine başvuran hastalarda ilaç uyumunu ve uyumu etkileyebilecek faktörleri değerlendirmeyi amaçladık. Nisan 2010-Temmuz 2010 tarihleri arasında çalışmaya dahil edilen 80 hastaya demografik ve sosyoekonomik özelliklerini ortaya koymak için anket uygulandı. Hastaların demografik ve sosyoekonmik özelliklerinin ilaç uyumuna etkisi belirlenmeye çalışıldı. Hastalarımızın 42'si kadın 38'i erkekti. Hastaların ilaç uyumu %84 olarak saptandı. İlaç uyumunu bozabilecek birçok faktör vardır. Hastalara gerekli bilgi verilirse, ilaçların yan etkileri anlatılıp önlemler alınırsa ilaç uyumu artabilir.Item Katarakt cerrahisi sonrası refraksiyon sonucuna etkili faktörlerin farklı tipte intraokuler lenslerle değerlendirilmesi(Uludağ Üniversitesi, 2014-04-09) Baykara, Mehmet; Sabur, Huri; Poroy, Ceren; Budak, Berna Akova; Yılmaz, Hakan; Tok, Mediha; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.Çalışmamızsda, cerrahi öncesi benzer özelliklere sahip, fakoemülsifikasyon cerrahisi sırasında tek veya üç parçalı lens konulan olgularda ulaşılan refraktif sonuca ve zaman içerisindeki değişimine etki eden faktörlerin değerlendirilmasi amaçlanmıştır. Retrospektif olarak yapılan çalışmaya kliniğimizde Kasım 2012- Şubat 2013 tarihleri arasında katarakt ameliyatı olan 60 hastanın 67 gözü dâhil edildi. Olgulara 2,2 mm temporal kesili fakoemülsifikasyon + göz içi lensi (GİL) ameliyatı yapıldı. Kırk iki göze 3 parçalı hidrofobik akrilik (Alcon Acrysof MA60AC) , 25 göze tek parçalı hidrofobik akrilik (Zaraccom Ultraflex UF60125) intraokuler lens konuldu. Postoperatif 1. hafta ve 1. ayda refraksiyon değerleri, ön kamara derinliği, lens pozisyonları değerlendirildi. Ameliyat sonrası sferik refraktif kayma 1. ayda 3 parçalı Acrysoft grubunda ortalama +0,034 D, tek parçalı Ultraflex grubunda ise +0,75D idi. Ameliyat sonrası erken dönemde elde edilen postoperatif ön kamara derinliği ve refraksiyon değerleri her iki lens takılan grupta geç dönemde de aynen devam etti. Ameliyat öncesi durumları benzer olan ve aynı yöntemle ameliyat edilen bu iki gruptan tek parçalı lens grubunda hedeflenenden daha hiperopik yönde sonuç ortaya çıkması ve bunun takiplerde aynen kalması, her ne kadar hasta sayısı az olsa da bu tip lens takılacak gözlerde konacak lens gücünü hesaplarken bu durumun göz önünde bulundurulması gerektiğini göstermiştir.Item Kliniğimizde yatırılarak tedavi edilen keratit olgularının analizi(Uludağ Üniversitesi, 2011-08-03) Budak, Berna Akova; Baykara, Mehmet; Türüdü, Sevil; Yusupov, Murat; Çevik, Görkem; Özmen, Ahmet T.; Özçetin, Hikmet; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.Çalışmanın amacı Uludağ Üniversitesi Tıp Fakültesi Göz Kliniği’nde mikrobiyal keratit tanısıyla yatırılarak tedavi başlanan hastalarda predispozan nedenlerin, kültür sonuçlarına göre etyolojik faktörlerin retrospektif olarak incelenmesidir. Çalışmada Aralık 2008- Nisan 2011 tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi Hastanesi Göz Kliniği’nde keratit tanısıyla yatarak tedavi gören 46 olgunun dosyası retrospektif olarak gözden geçirildi. Hastaların yatış tanıları, başlatıcı etkenler, kültür sonuçları, izlenen tedavi yöntemleri ve son durumları kaydedildi. Çalışmaya alınan hastalar arasında korneal ülser ya da apse gibi ciddi korneal enfeksiyon tanısı alan hasta sayısı 37 ( %80) idi. Herpes simpleks virüse bağlı keratiti olan hasta sayısı 3 (%7) , çeşitli bakterilere bağlı olanlar 7 (%15) , fungal enfeksiyona bağlı olanlar 6 (%13) kadardı . En sık izole edilen bakteriler Streptokokkus pneumonia ve Pseudomonas aeruginosa idi. Dört olguda (%10) varolan korneal enfeksiyonun üstüne fungal enfeksiyon eklenmesi görüldü. Üçüncü basamakta yatırılarak tedavi edilen ve daha önce başka merkezlerde tedavi girişimleri yapılan keratitli hastaların tedavi ve takibinde etken belirlenmesi önemli olup prognozu olumlu yönde etkilemektedir. Mayıs 2006 ile Şubat 2010 arasında Uludağ Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı’nda takip edillmiş olgular sunulmuştur. Toplam 20 olgunun (hepsi erkek ve 19-49 yaş aralığında) 16 tanesi sanayide metal işçisi idi. Olguların ilacı kullanmasına sebep 8 kişide kaynak alması, 7 sinde yabancı cisim sonrası, diğerlerinde ise böcek ilacı kaçması, yabancı cisim çarpması, gözlük çarpması, keratit ve korneal ülser sonrası idi. İlacın ortalama kullanımı 19 gün (2-60) idi. Olgularda epitelizasyon ortalama 4-60 günde gerçekleşti.
