Item ADA2 enzim düzeyleri ve aktivitesinin, COVİD-19 enfeksiyon şiddeti üzerindeki etkisi(Bursa Uludağ Üniversitesi, 2023-07-13) Bozkurt, Tuğçe; Budak, Ferah; Gültekin, Sara Şebnem Kılıç; Bursa Uludağ Üniversitesi/Sağlık Bilimleri Enstitüsü/Tıp Fakültesi/İmmunoloji Anabilim Dalı.; 0000-0001-6563-8716COVID-19; Aralık 2019'da Çin/Wuhan'da ortaya çıkan tüm dünyayı etkisi altına alan pandemik hastalıktır. SARS-CoV-2, bulaşıcılığı yüksek, ağır klinik tablolara sebep olabilen enfeksiyon etkenidir. Adenozin deaminaz (ADA) özellikle lenfoid dokularda bulunan, immün sistem fonksiyonlarını etkileyebilen, ADA1 ve ADA2 (CECR1) izoenzimlerine sahip bir enzimdir. ADA1 T hücre proliferasyonunu, ADA2 ise monositlerin farklılaşmasını etkileyebilmektedir. Çalışmamızda; 41 ağır vaka, 34 hafif/orta vaka, 39 a/presemptomatik ve 6 kritik seviye yetişkin COVID-19 hastası ve 40 sağlıklı kontrol grubu dahil edilmiştir. Enzim aktivitesi Giusti ve Galanti yöntemi ile çalışılmıştır. 6 sağlıklı kontrolden elde edilen PKMH ve monositler ile eş-kültür deneyleri yapılmıştır. Eş-kültür sonrası elde edilen süpernatanlar ve COVID-19 hasta serumları, ELISA yöntemi ile incelenerek M1 ve M2 ilişkili sitokin/kemokin profilleri ortaya konmuştur. ADA enzim aktiviteleri incelendiğinde en yüksek değeri kritik COVID-19 vakaları oluşturmaktadır. Sitokin/kemokin çalışmalarında; IL-6, TNF-α, IL-12, IFN-ɣ, GM-CSF, ROS, iNOS, arjinaz, IL-4, IL-10 ve M-CSF'de kritik ve ağır vakalarda artış gözlemledik. Hücre kültürü deneylerimizde ise; kritik, ağır vakalar ve in vitro COVID-19 grubunda, ADA2'in M2 ilişkili sitokin ve kemokinlerle pozitif korelasyon gösterdiği bulunmuştur. Bunun yanında, AHÖ incelemelerinde CD206+ CD86+ M1-M2 miks makrofajlar COVID-19 grubunda anlamlı derecede yüksek bulunmuştur. Sonuç olarak; makrofaj polarizasyonunun ADA2 ile doğrudan bağlantılı olduğu, ADA1'in ise daha düşük katkı sağladığına dair bulgular elde edilmiştir. Tüm analizlere göre, ağır COVID-19 vakalarında makrofajların M1'den M2'e yönelik bir polarizasyon gösterdiği, fakat daha çok M1-M2 karışık fenotipte heterojenik bir yapı göstermekte olduğuna dair kanıtlar elde edilmiştir. Bu durum, COVID-19'da, ADA2'in makrofaj polarizasyonuna etkisini ve hastalık süreciyle muhtemel ilişkisini gösterebilir.Item Akut lösemilerde minimal residüel hastalığın akım sitometrisi yöntemiyle araştırılması ve relaps ile korelasyonu(Uludağ Üniversitesi, 2004-11-19) Ozan, Ülkü; Özkalemkaş, Fahir; Ali, Rıdvan; Budak, Ferah; Göral, Güher; Özkocaman, Vildan; Özçelik, Tülay; Tunalı, Ahmet; Uludağ Üniversitesi/Tıp Fakültesi/İç Hastalıkları Anabilim Dalı/Hematoloji Bilim Dalı.; Uludağ Üniversitesi/Tıp Fakültesi/Klinik Mikrobiyoloji ve Enfeksiyon Hastalıkları Anabilim Dalı.Morfolojik olarak tam remisyonda kabul edilen akut lösemi olgularında kemik iliğindeki rezidüel lösemik hücrelerin relapsa yol açtığı bilinmektedir. Bu çalışmanın amacı, minimal residüel hastalık (MRD) tesbitinde akım sitometrisinin kullanımını ve MRD ile hastalıksız yaşam arasındaki korelasyonu değerlendirmektir. Biz önce 89 akut lösemi olgusunda tanıdaki immünfenotip sonuçlarını değerlendirdik ve %73’ünde nadir görülen koekspresyonlar saptadık. Daha sonra, halen hayatta olan 16 olgunun başlangıçta ve komplet remisyondaki (KR) koekspresyon düzeyleri karşılaştırıldı ve 28-40 haftalık takip süresince KR’da kalan 8 olguda anlamlı azalma gözlendi. Buna karşın, olguların 3’ünde, remisyondaki koekspresyon düzeylerinin tanıda belirlenene yakın olduğu dikkati çekti ve bunlardan 2’si 19 ve 30 hafta sonrasında nüksettiler. Bu veriler, rezidüel hastalığın relapsın habercisi olduğunu ve bunu belirlemede akım sitometrisi yönteminin kullanılabileceğini desteklemektedir.Item Akut lösemili hastalarda kantitatif in situ hibridizasyon yapılarak akım sitometri ile telomer uzunluğu ölçümü ve prognozla ilişkisi(Uludağ Üniversitesi, 2004-11-19) Demirel, Gülderen Yanıkkaya; Özkocaman, Vildan; Özkalemkaş, Fahir; Budak, Ferah; Ali, Rıdvan; Oral, H.Barbaros; Özçelik, Tülay; Ozan, Ülkü; Tunalı, Ahmet; Uludağ Üniversitesi/Tıp Fakültesi/İç Hastalıkları Anabilim Dalı/Hematoloji Bilim Dalı.; Uludağ Üniversitesi/Tıp Fakültesi/Klinik Mikrobiyoloji ve Enfeksiyon Hastalıkları Anabilim Dalı.Akut lösemili hastalarda telomer uzunluğunu inceleyerek, tanıda ve remisyon peryodunda rölatif telomer uzunluklarını karşılaştırarak prognostik bir değeri olup olmadığını araştırdık. Bu çalışma 13’ü kadın 8’i erkek, ortanca yaşı 37 ve yaş aralığı 17-66 ve 15’i AML 6’sı ALL olan 21 akut lösemili hastayı içerdi. Rölatif telomer uzunluğu düzeyi flow-floresan in situ hibridizasyon (FISH) metodu ile kemik iliği kan mononükleer hücrelerinde saptandı. Tanı anında 3 hastada kısa telomer 18 hastada uzun telomer boyu gözlendi. Remisyonda ise 11 hastada kısa 10 hastada uzun telomer boyu gözlendi. Telomer uzunluğu ile laboratuvar parametreleri arasında istatistiksel anlamlı ilişki bulunamadı. Yaşam süresi ve remisyon süresi bakıldığında uzamış ve kısalmış rölatif telomer uzunluklu gruplar arasında istatistiksel anlamlı farklılık bulunmadı. Sonuç olarak bulgularımız rölatif telomer uzunluğu parametresinin analizi hakkında bizim popülasyonumuzun heterojen olduğunu düşündürdü. This study aims to measure the serum levels of interleukin (IL)-23, IL-17, IL-1 beta, tumor necrosis factor (TNF)-alpha, IL-10, transforming growth factor (TGF)-beta, C-reactive protein (CRP), procalcitonin (PCT), and serum amyloid A (SAA) in cases of candidemia and to compare them with those observed in cases of bacteremia. For this purpose, the serum cytokine levels from 50 patients with candidemia were compared with those of 14 patients with polymicrobial sepsis, 30 patients with bacteremia, and 27 healthy control subjects. The cytokine levels were studied using sandwich ELISAs according to the manufacturer protocol. The serum levels of TGF-beta, IL-23, and IL-17 were found to be significantly higher in the candidemia group in comparison with the samples from those with bacteremia and healthy controls. The PCT and SAA levels were higher in samples from the group with bacteremia those from individuals with candidemia and the healthy control group. Assuming an IL-17 level threshold of > 38.79 pg/ml, the sensitivity and specificity were 38% and 96.6%, respectively but considering an IL-23 threshold of > 59.97 pg/ml, the sensitivity and specificity valueswere found to be 72% and 60%, respectively. The sensitivity and the specificity of the TGF-beta levels were found to be 85.71% and 53.33%, respectively, when the TGF-beta threshold is > 560 pg/ml. PCT and SAA demonstrated a superior performance for the differentiation of candidemia and bacteremia. Despite early diagnosis and treatment of patients, chronic infections are seen in 10-30% of patients. In this study, we aimed to investigate the immunological factors that play roles in the transition of brucellosis from acute infection into chronic infection. Here, more than 2000 miRNAs were screened in peripheral blood mononuclear cells (PBMCs) of patients with acute or chronic brucellosis and healthy controls by using miRNA array, and the results of the miRNA array were validated through qRT-PCR. Findings were evaluated using GeneSpring GX (Agilent) 13.0 software and KEGG pathway analysis. Four miRNAs were expressed in the chronic group but were not expressed in acute and control groups. Among these miRNAs, the expression level of miR-1238-3p was increased while miR-494, miR-6069, and miR-1393p were decreased (p< 0.05, fold change > 2). These miRNAs have the potential to be markers for chronic cases. The differentially expressed miRNAs and their predicted target genes involved in endocytosis, regulation of actin cytoskeleton, MAPK signaling pathway, and cytokine-cytokine receptor interaction and its chemokine signaling pathway indicate their potential roles in chronic brucellosis and its progression. Hastalık temel olarak akut ve kronik olarak iki fazda görülmektedir. Bruselloz tanısında çok sayıda laboratuvar yöntemi geliştirilmiş olmasına rağmen halen özgüllük ve duyarlılığı yüksek, ucuz ve güvenli kriterlerini karşılayan özel bir test yoktur. Ayrıca tedaviye yanıtı değerlendirecek kronikleşmeyi öngörecek bir testte henüz bulunamamıştır. Human ABI gene family member 3 (ABI3), Human C-type lectin domain family 12, member B (CLEC12B), Human DNA damage inducible transcript 4 like (DDIT4L), Human zinc finger protein 1 (ZFP1), Human protein inhibitor of activated STAT4 (PIAS4), Human protein phosphatase 2A activator regulatory subunit 4 (PPP2R4), Human WD Repeat-Containing Protein 33 (WDR33) Human Indoleamine 2,3- Dioxygenase (IDO), son dönemlerde keşfedilmiş ve immün sistemin çeşitli aşamalarında görev alan proteinlerdir. Bu çalışma Brucella patogenezinin aydınlatılmasına katkı sağlayacak, bruselloz tedavisine yanıtı gösterecek ve kronikleşmeyi öngörebilecek yeni markerler keşfedebilmek amacıyla yapılmıştır. Çalışma grupları akut, kronik, iyileşen ve kontrol hastalar olmak üzere dört grup olarak oluşturuldu. Tüm gruplardan alınan kan örneklerinden serum ayrılarak Enzyme Linked Immunosorbent Assay (ELISA) yöntemi ile çalışmamızda yer alan protein seviyeleri tespit edildi. ABI-3, CLEC12B ve PPP2R4'ün artan seviyelerinin kronikleşmeyi öngörmede, azalan seviyelerinin ise tedavi yanıtının izlenmesinde, DDIT4L, IDO, PIAS4 VE ZFP-1'in azalan seviyesinin kronikleşmeyi öngörmede, artan seviyesinin tedaviye yanıtı değerlendirmede kullanılabileceği görülmüştür. Eksozomlar, birçok hücre tipi tarafından salınan multiveziküler gövdelerden türetilen küçük lipit veziküllerdir. Eksozomal hücre kargosu, kökenlendiği hücrenin tipini yansıtır. Bu çalışmada akut, kronik bruselloz ve sağlıklı kontrollerden alınan serum örneklerinden elde edilen eksozomların Akan Hücre Ölçer (AHÖ) yardımıyla immünfenotiplemesini yapmak, farklı gruplarda eksozomların hangi hücre gruplarından köken aldığını belirlemek, brusellozun kronikleşmesinde, eksozomların etkisini ortaya koymak ve bruselloz immünpatogenezinde eksozomların potansiyel rollerine dair literatüre katkılar sunmak amaçlanmıştır. Çalışmamıza, bruselloz tanısı konmuş 10 akut ve 10 kronik hasta ile 10 sağlıklı kontrol dahil edilmiştir. Serum örneklerinden eksozomlar ticari izolasyon kiti kullanılarak elde edilmiştir. Önceden tasarlanan panelde, monoklonal antikorlarla boyandıktan sonra, AHÖ ’de eksozomların immünfenotiplemesi yapılmıştır. Eksozom varlığının doğrulanması, eksozoma spesifik monoklonal antikorlar olan CD9, CD63 ve CD81 boyamaları ve elektron mikroskobu (SEM) değerlendirmeleriyle yapılmıştır. Değerlendirme sonucuna göre granülosit kökenli eksozom değerleri akut grupta, kronik ve sağlıklı gruba göre anlamlı derecede yüksek çıkmıştır, bunun yanında kronik ve sağlıklı grup arasında anlamlı bir fark gözlenmemiştir. G-MDSC (granülositik-myeloid kökenli süpressör hücreler) kökenli eksozom değerleri, akut grupta kronik ve sağlıklı gruba göre anlamlı derecede yüksek bulunmuştur, aynı zamanda kronik grup ve sağlıklı kontrol grubu arasında anlamlı derecede farklılık gözlenmiştir. Tez çalışması sonucunda, akut dönemde hastalığın patogenezinde granülosit kökenli eksozomların rol oynadığı ve ortamda bulunan myeloid kökenli supressör hücrelerden kaynaklanan eksozomlarında granülosit kökenli eksozomlar olduğu bulunmuştur. Data on physical examination, anthropometric measurements and blood biochemistry analysis were recorded for each subject along with analysis for SOCS1-1478 CA/del polymorphism by polymerase chain reaction-restriction fragment length polymorphism. The relation of SOCS1-1478 CA/del polymorphism to PCOS status and insulin resistance was analysed via logistic regression analysis. Mean (SD) levels for BMI (28.5(6.5) vs.22.5 (4.9) kg/m(2), p<.001), HOMA-IR (3.1(1.8) vs.1.5 (1.0), p<.001), LDL-cholesterol (115.9(32.7) vs.100.7 (27.3)mg/dL, p=.03) and triglyceride (113.8(64.9) vs.83.3(36.3)mg/dL, p=.017) were significantly higher in patients. Groups were similar in terms of SOCS1-1478 CA/del polymorphism. No significant relation of this polymorphism was noted to PCOS and HOMA-IR. Our findings revealed no difference between groups in terms of the rate of SOCS1-1478 CA/del polymorphism, and no significant relation of this polymorphism to insulin resistance and PCOS status. IMPACT STATEMENT Polycystic ovary syndrome (PCOS), the most common cause of anovulation and the most commonly encountered form of female endocrine disease. SOCS proteins have been suggested to play a fundamental role in the negative feedback regulation of the JAK-STAT pathway, which is the major signalling pathway involved in a wide range of physiologic and pathologic processes, including inflammatory diseases, malignancies and immune disorders. Pathways involving the induction of suppression of SOCS proteins were also shown likely to be involved in mediating cytokine-induced insulin resistance. The present study was designed to determine the frequency of SOCS1-1478 CA/del gene polymorphism in patients with PCOS in relation to healthy controls and insulin resistance. Our findings revealed significantly higher rates of insulin resistance, obesity and dyslipidaemia in Turkish patients with PCOS compared with age-matched healthy controls, while no difference between study groups in terms of the rate of SOCS1-1478 CA/del polymorphism along with no significant relation of SOCS1-1478 CA/del polymorphism to insulin resistance and PCOS status. The patients were divided into 2 groups: Group I included 16 children with naive chronic replicative hepatitis B infection, and Group II included 6 children who are inactive hepatitis B virus (HBV) carriers. Group I received interferon-alpha subcutaneously (10 mU/m(2)/dose), 3 times a week during 4 months. Initial serum alanine aminotransferase (ALT) levels, hepatitis B serologic markers, serum interleukin-12 and interferon-gamma levels were measured. In Group I, laboratory tests were re-evaluated in the second and fourth months. Liver biopsy was performed in all patients and samples were used for tissue interleukin-12 level evaluation and histopathological examination. Hepatic activity index (HAI) and serum interferon-gamma were significantly higher in Group I (P < 0.05). Initial tissue interleukin-12 levels in Group I were low but a significant increase was observed at the fourth month (P < 0.05). While responder patients in Group I had marked elevation of tissue interleukin-12 levels, nonresponders did not reveal considerable changes at the fourth month evaluation. A negative correlation was found between serum HBV-DNA copies and interferon-gamma levels prior to therapy (P < 0.01, r: -0.66). The analysis of cytokine levels with serum transaminases demonstrated a positive correlation between the tissue interleukin-12 levels at the fourth month and serum ALT levels at the beginning and second month of the therapy (r: 0.77, P < 0.05 and r: 0.92, P < 0.05, respectively). This is the first study emphasizing the relationship between tissue cytokine levels and therapy success. Recently, studies are focused on the effect of antihypertensive drugs on insulin sensitivity and cytokines. We undertook this study to evaluate the effect of amlodipine, a long-acting dihydropyridine calcium channel blocker treatment on TNF-α, homeostasis model assessment (HOMA) IR and leptin levels in obese hypertensive type 2 diabetic patients. Methods: Amlodipine 5-10 mg for 12 wk was given to type 2 diabetic patients in the amiodipine group. Pre- and post-treatment values of laboratory parameters in the amlodipine group were compared with those of normotensive nondiabetic obese controls. At baseline blood pressures (BP) and metabolic parameters were measured in all patients and repeated after 12 wk in the amlodipine group. Results: Basal waist-to-hip ratio, systolic and diastolic BPs, fasting glucose, TNF-α and HOMA-IR values of the amlodipine group were higher than the control group. No difference was detected in body mass index, fasting insulin, hemoglobin Alc and leptin values between groups. The systolic and diastolic BPs, fasting glucose, HOMA-IR and TNF-α values decreased significantly after the treatment. But, there was no correlation between percentage change in TNF-α and HOMA-IR. Interpretation & conclusion: Besides reducing BP, amlodipine seemed to improve IR and decrease TNF-α levels. Screening for thyroid autoimmunity in patients with chronic idiopathic urticaria (CIU) is generally recommended. However, there are not yet sufficient data as to whether levothyroxine treatment is beneficial for the clinical symptoms of CIU in patients with thyroid autoimmunity. Aim. We investigated the effect of levothyroxine treatment on clinical symptoms and serum tumour necrosis factor (TNF)-alpha, interleukin (IL)-10 and interferon (IFN)-gamma levels in euthyroid patients with CIU and thyroid autoimmunity. Methods. In total, 15 patients with CIU and positive thyroid autoantibodies were randomized to receive either levothyroxine plus 5 mg/day desloratadine (suppression group, n = 8) or 5 mg/day desloratadine alone (control group, n = 7) for 12 weeks. Clinical symptoms of CIU, thyroid hormone levels, thyroid antibodies and serum cytokine levels were assessed at baseline and after the treatment. Results. There were significant improvements in pruritus score and severity of weals in both groups compared with baseline values, but when the two groups were compared, there was no significant difference in the patients' clinical symptoms. Thyroid antibody titres were not different according to intragroup and intergroup analysis. In the suppression group, serum IFN-gamma and TNF-alpha levels were increased after treatment with levothyroxine compared with baseline values and there was a borderline statistical significance (P = 0.05 for both). Conclusions. These results suggest that levothyroxine treatment is not a reasonable option in euthyroid patients with CIU and thyroid autoimmunity. Fifty type 2 diabetic patients were randomized to receive rosiglitazone 4 mg/day (n = 19) or metformin 850 mg/day (n = 16) with MNT or MNT alone (n = 15), for 52 weeks. Arterial stiffness was assessed by using large and small artery elasticity index (SAEI and LAEI, respectively). SAEI, LAEI, serum MCP-1 and MMP-9 levels were measured at baseline and following 52 weeks of treatment. SAEI was improved only in the rosiglitazone group, and the difference was still statistically significant when the three groups were compared (p = 0.024). There were no differences in LAEI in inter- and intragroup comparisons at the end of the study. Serum MMP-9 levels were decreased in the metformin (-13.5 +/- 34.8%, p = 0.02) and rosiglitazone (-27.2 +/- 51.0%, p = 0.023) groups compared with baseline values, whereas no significant change was seen in serum MCP-1 levels. The patients were randomized into two groups: group 1 (n = 16) received 250 mg S. boulardii dissolved in 5 ml of water orally twice daily for 7 days and group 2 (n = 11) received placebo. Clinical and laboratory assessments were performed on admission and on day 7 of follow-up. Both groups experienced reduced daily stool frequency, the decrease being significantly greater in group 1 on days 3 and 4 compared with group 2. Group I demonstrated significant increases in serum immunoglobulin A and decreases in C-reactive protein levels on day 7. The percentage of CD8 lymphocytes on day 7 was significantly higher in group I than group 2. The aim of this study was to investigate the immunological alterations that occur during the storage of erythrocyte suspensions which may lead to transfusion-related immunomodulation following allogeneic blood transfusion. Materials and methods. One part of the erythrocyte suspensions obtained from donors was leucoreduced while the other part was not. The leucoreduced (LR) and non-leucoreduced (NL) erythrocyte suspensions were then further divided into three equal amounts which were stored for 0, 21 or 42 days prior to measurements, by enzyme-linked immunosorbent assays, of cytokine levels in their supernatants. T-helper (Th) lymphocyte subgroups and gene expression were analysed in the NL erythrocyte suspensions by flow cytometry and real-time polymerase chain reaction, respectively. Results were compared to those of storage day 0. Results. By day 21, the number of Th2 cells had increased significantly and the numbers of Th1, Th22 and Treg cells had decreased significantly in the NL erythrocyte suspensions. On day 42 the numbers of Th2 and Treg cells in the NL suspensions were significantly increased while the number of Th1 cells was significantly decreased. The levels of transcription factors (TBX21, GATA3, and SPI.1) were significantly decreased on days 21 and 42, and AHR, FOXP3 and RORC2 levels were significantly increased on day 42 in NL erythrocyte suspensions. The decrease in interleukin-22 and increase in transforming growth factor-beta levels found in NL erythrocyte suspensions on day 21 were statistically significant. Elevated levels of interleukin-17A were found in both LR and NL erythrocyte suspensions on day 42. Discussion. Los aditivos naturales se han convertido en uno de los mayores potenciadores alternativos de inmunidad actualmente. Spirulina platensis (SP) y Saccharomyces cerevisiae (SC) se han usado para mejorar el sistema inmunológico y la calidad de vida. El objetivo de este estudio fue regular el efecto inmune de la combinación de S. cerevisiae y S. platensis (A. platensis). Se analizaron 40 conejos blancos machos de Nueva Zelanda, de 5 a 6 semanas de edad, en 4 grupos: Control (dieta basal); SC (añadido 3 g / kg de dieta); SP (añadido 5% de la dieta); SC y SP (se añadieron 3 g/kg SC y 5% SP de la dieta), respectivamente. El experimento completo duró 90 días. Muestras de sangre fueron obtenidas mediante punción venosa de oído en el día 90. Los valores de linfocitos T CD4+ y CD8+ se determinaron mediante citometría de flujo y las citoquinas (IFN-γ e IL-4) se determinaron por ELISA. De acuerdo con los resultados, no hubo diferencias significativas en la expresión de citoquinas, pero el suero CD4+/CD8+ aumentó en los animales alimentados con SP y SC + SP dietas suplementadas (3 g/kg y 5% de la dieta, respectivamente). Ferroptoz aynı zamanda, ferröz (Fe+2) demire bağımlı hücre ölüm tipi olarak tanımlanmıştır. Apoptoz, piroptoz, otofaji gibi diğer hücre ölüm yollarından farklı özellikler gösterir. Ferroptoz sırasında gözlemlenen en önemli morfolojik özellikler; mitokondride gözlenen küçülme ve membran yoğunluğudur. Biyokimyasal özellikler ise, hücre içi serbest demir miktarındaki artış ve lipid peroksidasyonudur. Ferroptoz, nörodejeneratif hastalıklar ve kanser gibi hastalıkların ortaya çıkmasında ve gelişiminde önemli rol oynaması nedeniyle çok sayıda araştırmanın odak noktası haline gelmiştir. Bu hastalıkların yanı sıra; GPX4, GSH aktivitesinde azalma ve ortamda reaktif oksijen türlerinin (ROT) birikimi gibi olaylar ile birçok enfeksiyon hastalığında da ferroptoz süreci görülebilmektedir.
