The study was performed as a randomized, prospective, controlled, and single-blind trial. Fifty-five participants (30 men, 25 women) who were under a regular follow-up protocol in our Rheumatology Clinic with the diagnosis of AS according to the modified New York criteria were included in the study. The participants were randomly assigned into two groups: in group I, Pilates exercise program of 1 h was given by a certified trainer to 30 participants 3 times a week for 12 weeks, and in group II, designed as the control group, 25 participants continued previous standard treatment programs. In groups, pre-(week 0) and post treatment (week 12 and week 24) evaluation was performed by one of the authors who was blind to the group allocation. Primary outcome measure was functional capacity. Evaluation was done using the Bath Ankylosing Spondylitis Functional Index (BASFI). Exploratory outcome measures were Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Bath Ankylosing Spondylitis Metrology Index (BASMI), Chest expansion, and ankylosing spondylitis quality of life (ASQOL) questionnaire. In group I, BASFI showed significant improvement at week 12 (P = 0.031) and week 24 (P = 0.007). In group II, this parameter was not found to have significantly changed at week 12 and week 24. Comparison of the groups showed significantly superior results for group I at week 24 (P = 0.023). We suggest Pilates exercises as an effective and safe method to improve physical capacity in AS patients. Our study is the first clinical study designed to investigate the role of Pilates method in AS treatment. Postmenopausal female osteoporotic patients were randomized into three groups: balance and coordination, strengthening, and aerobic exercise. The exercise programs were performed for 12 weeks, 1 hr each day for 3 days of the week. Patients were followed-up for 12 weeks after the initial intervention. After the exercise program, patients continued their daily life activities and were called back to the clinic for additional testing after 12 weeks. Static and dynamic balance measurements and pain and life quality assessments were performed at enrollment, and at the 12th and 24th weeks. Significant improvements in both the Timed Up and Go test and Berg Balance Scale values at the 12th week were only observed in the balance-coordination group. There were statistically significant improvements in night and daytime pain visual analog scale scores at the 12th and 24th weeks in the strengthening exercise group. No patient experienced falling during the 24th week follow-up. The strengthening exercises were observed to be more effective in pain reduction, and balance and coordination exercises were found to be more effective in improvement of static and dynamic balance.Item Postmenopozal osteoporozlu hastalarda denge-koordinasyon, kuvvetlendirme ve aerobik egzersizlerin düşmeye etkisi(Uludağ Üniversitesi, 2012) Dizdar, Meltem; İrdesel, Jale; Uludağ Üniversitesi/Tıp Fakültesi/Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı.Çalışmamız, postmenopozal osteoporoz'lu (OP) hastalarda denge-koordinasyon, kuvvetlendirme ve aerobik egzersizlerinin statik ve dinamik denge fonksiyonlarına, yaşam kalitesine ve düşme sıklığına etkisini değerlendirmek ve birbirlerine üstünlükleri olup olmadığını belirlemek amacıyla planlanmıştır.Çalışmaya Dünya Sağlık Örgütü OP Tanı Kriterlerine göre primer OP tanısı konan 75 hasta alınmıştır. Hastalar 3 gruba eşit olarak randomize edilerek 1.gruba denge-koordinasyon, 2.gruba kuvvetlendirme ve 3. gruba aerobik egzersizler verildi. Egzersiz programı 12 hafta boyunca haftada 3 gün, 1 saat süre ile eğitmen eşliğinde uygulandı. Değerlendirme egzersiz öncesi (0. hafta) ve sonrası (12. ve 24. haftada) uygulanan tedaviden habersiz bir araştırmacı tarafından yapıldı. Statik denge TeknoBody PK stabilometri denge platformu, dinamik denge Berg Denge Skalası (BDS) ve Zamanlı Ayağa Kalkma ve Yürüme Testi (ZAKY), yaşam kalitesi Quality Of Life Quetionnarie Of The Europen Foundation For Osteoporosis (QUALEFFO-41) ve ağrı Vizüel Anolog Skala (VAS) ile değerlendirildi.Tedavi sonrası her 3 egzersiz grubu ile total QUALEFFO-41, VAS ve ZAKY parametrelerinde anlamlı düzelmeler saptandı. ZAKY ile BDS'deki düzelmenin birlikteliği sadece denge-koordinasyon grubunda gözlendi. VAS'daki düzelmenin kuvvetlendirme grubunda 24. haftada da devam ettiği belirlendi. BDS ve göz kapalı çevre parametresinde denge-koordinasyon grup lehine, zihinsel fonksiyon ve göz açık Y (24.hafta) parametresinde kuvvetlendirme grup lehine ve sosyal fonksiyonda (24.hafta) aerobik grup lehine anlamlı düzelmeler tespit edildi. Her 3 egzersiz grubu ile takip süresince düşme gözlenmedi.Sonuç olarak OP'li hastalarda egzersizlerin erken dönemde ağrı, dinamik denge fonksiyonları, yaşam kalitesi ve düşme sıklığını azaltmada etkili olduğu söylenebilir. Ayrıca denge-koordinasyon egzersizlerinin diğer egzersizlere nazaran statik ve dinamik dengede ve düşmelerin önlenmesinde daha etkili olduğu tespit edildi.
