The determination of alterations in expression profiles of ADAMTS family genes in Alzheimer's disease (AD) patients may contribute to the explanation of tissue pathology and also new ideas for remedial approaches for this incurable but preventable disease. Therefore, the goal of this study was to describe and identify the distribution, characteristics, and any changes in the expression, in other words, immunoreactivity, for aggrecanases (ADAMTS4, 5, 9, and 15) proteins in AD brain. Methods: Nine cases that were autopsied in the Council of Forensic Medicine, Bursa Morgue Department in 2013, were selected. All of the cases were sent for autopsy to the institution within 8 hours after death. At autopsy, tissue samples were obtained for histopathological examination of organs for determining the cause of death. Out of these, two cases were diagnosed with AD by neurologists when they were alive. Immunohistochemical staining was performed on the brain slides by using relevant primary and secondary antibodies against aggrecanase proteins. All images were acquired using a X200 objective of a microscope (Olympus BX53) and evaluated by the staining intensity using a semi-quantitative scoring system. Results: ADAMTS4 and 5 were slightly under-expressed in the brains from autopsied AD cases compared to those of control brains and suggested that extracellular matrix (ECM) degradation was not endorsed in AD brain. On the other hand, ADAMTS9 and 15 aggrecanases were not found to be expressed in correspondent brain sections of AD and control cases. Conclusion: The current study demonstrated that some aggrecanases were found to be under-expressed in AD brains. Additional studies in which all ADAMTS enzymes will be studied in terms of mRNA and protein levels are needed to understand the relative contributions of ADAMTS genes and proteins in AD brains.Item Age-related histopathological changes in the cardiac conducting system in the Turkish population: An evaluation of 202 autopsy cases(Via Medica, 2012-08) Çomunoğlu, Cem; Çomunoğlu, Nil; Eren, Bülent; Tanrıöver, Özlem; Gündoǧmuş, Ümit Naci; Eren, Filiz; Türkmen, Nursel; Uludağ Üniversitesi/Tıp Fakültesi/Adli Tıp Anabilim Dalı.; 8715251700Background: Histopathological features of the cardiac conducting system (CCS) in the Turkish population have not been investigated previously. Material and methods: We examined CCS of 202 autopsy heart specimens dissected between the years 2004 and 2005 in Bursa Forensic Medicine Institution. Of the 202 cases from all age groups, 154 were males and 48 were females. Results: In our cases, an increase in fibrous and adipose tissue concordant with age, indicating an age-related nature, were detected. Fibrous and fatty tissue infiltration appeared at the age of 35. Fatty infiltration started between the ages 20 and 34 years at the sinoatrial node (SAN). There was no relationship between obesity and fatty tissue infiltration in SAN and atrioventricular node (AVN). In 4 cases calcification and in 19 cases inflammation was observed. Amyloid accumulation was not present. In 7 cases myocardial infarction not involving CCS was seen. In 1 case fibroelastoma was detected. Conclusions: In the Turkish population age-related fibrosis and fatty infiltration in CCS appeared at the age of 35 years and increased with age. Fatty infiltration in the SAN started at a younger age than that reported in the literature. In cases where the cause of death could not be determined, we could not detect lethal pathological features. However, we think that examination of the CCS will improve the quality of autopsy diagnosis.Item The role of ADAMTS1 and versican in human myocardial infarction: A postmortem study(Oxford University, 2016-08) Pehlivan, Sultan; Akyol, Sümeyya; Eren, Filiz; Güleç, Mehmet Akif; Eren, Bülent; Demircan, Kadir; Akyol, Ömer; Gürses, Murat Serdar; Ural, Mustafa Numan; İnanır, Nursel Türkmen; Fedakar, Recep; Uludağ Üniversitesi/Tıp Fakültesi/Adli Tıp Anabilim Dalı.; 0000-0002-9982-0476; 0000-0002-4047-6455; AAH-6287-2021; AAC-8913-2020; 55979536300; 57163358100; 57188706721; 8725968900Objective: To determine the role of a disintegrin and metalloproteinase with thrombospondin type 1 motif (ADAMTS1) and fragmented versican in the myocardial infarction (MI) process in humans and to evaluate the diagnostic efficacy of ADAMTS1 for postmortem diagnosis of MI. Methods: Thirty autopsied individuals were allocated into 2 groups, namely, a study group of individuals who died of myocardial infarction (n = 20), and a control group who died of trauma (n = 10). We performed standard immunohistochemical staining on myocardial tissue specimens, studying anti-ADAMTS1, anti-versican, and anti-versican C terminal peptide sequence (DPEAAE) fragments. Results: Strong, diffuse staining was observed throughout myocardial tissue for ADAMTS1 in the 2 groups. However, in the study group, we observed no expression for ADAMTS1 around fibrotic areas but detected slight staining in coagulative and necrotic zones. Conclusion: Similar localizations of ADAMTS and fragmented versican in human heart tissue indicate that versican presumably is cleaved by ADAMTS1. Hence, ADAMTS1 can be regarded as a new marker for postmortem differential diagnosis of MI.Item Uçucu gazlarla zehirlenmeye bağlı ölümler: retrospektif otopsi çalışması(Bursa Uludağ Üniversitesi, 2019-04-08) Gürses, Murat Serdar; Eren, Bülent; Eren, Filiz; Aydoğan, Taner; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Adli Tıp Anabilim Dalı.Uçucu maddeler hızlı, keyif verici, hafif öfori yapan etkileri nedeniyle gençler arasında sıklıkla kullanılır. Ülkemizde ve dünyada yaygın bir halk sağlığı problemi olan uçucu madde kötüye kullanımı önemli derecede mortalite ve morbiditeden sorumludur. Bu çalışmamızın amacı uçucu madde zehirlenmesine bağlı otopsi yapılmış vakalardaki ölüm nedenlerini, ölüm mekanizmasını, ölüm orjini, toksikolojik analiz raporları ve vakalarının sosyodemografik özelliklerini tartışmaktır. Çalışmamızda Adli Tıp Kurumu Bursa Grup Başkanlığı Morg İhtisas Dairesince 01/01/2016 ve 01/08/2018 tarihleri arasında otopsisi yapılmış olan toplam 4625 vaka retrospektif olarak değerlendirilmiştir. Çalışmaya, uçucu gaz zehirlenmesi sonucu öldüğü tespit edilen 11 vaka dahil edilmiştir. Vakaların tamamı erkek olup yaşları 15-62 arasında değişmektedir. Vakaların yarısından fazlasının 18 yaş ve altı (n=5, %55.6) olduğu tespit edildi. Vakaların 5 tanesinde ölüm orjini intihar olduğu geriye kalan 6 vaka ise ölüm orjini kaza olduğu tespit edildi. 11 vakanın 8’inde histopatolojik inceleme yapıldı. Yapılan toksikolojik incelemelerde; bütan, etan, propan, toluen, heksan, isobütan gibi uçucu gazlar tespit edildi. Organların mikroskobik ve makroskopik incelemesinde; non-spesifik bulgular ve akciğerlerin ağırlıklarında anlamlı artış tespit edildi. Bu ölümlerde otopsi bulguları ve histopatolojik bulgular non-spesifik bulgulardır. Ancak toksikolojik analiz raporlar, olay yeri, tanık ifadeleri ve tıbbi öykü yardımcı olabilir. Özellikle bu tür vakalarda otopsinin çok hızlı yapılması ve usulüne uygun alınan numunelerin hava ile temas kurulmadan laboratuvara ulaştırılması gerekmektedir.Item Ülseratif kolit tanılı olgularımızın retrospektif olarak değerlendirilmesi(Uludağ Üniversitesi, 2012-12-27) Ünal, Fatih; Şahin, Gülseren; Cebe, Ayşegül; Ayhan, Semin; Eren, Filiz; Kasırga, ErhunGiriş: İnflamatuvar barsak hastalıkları (İBH), gastrointestinal kanalın remisyon ve alevlenmeler ile seyreden kronik inflamatuvar hastalıklarıdır. Çalışmamızda İBH tanısı ile izlediğimiz hastaların klinik ve laboratuvar özelliklerinin geriye dönük olarak değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çocuk gastroenteroloji bölümlerimizce 7 yıldır İBH tanısı ile izlemleri yapılan 18 hastanın başvuru yakınmaları, demografik özellikleri ve beraberinde bulunan hastalıklar kayıtlardan incelendi. Fizik bakıda saptanan olağan dışı bulgular endoskopik bulgular, histopatolojik bulgular ve uygulanan tedaviler gözden geçirildi. Bulgular: Çalışmamıza alınan 18 hastanın tanı yaşı ortalamaları 13,6±2,9 yıl (10K, 8E) ve semptomların başlangıcından tanıya kadar geçen süre ortalama 6,9±4,5 aydı. Onyedi hasta ülseratif kolit (ÜK) ve bir hasta da intermediate kolit (İK) tanısı almıştı. İki hastada (%11) hastalık açısından pozitif aile öyküsü vardı. Tanı anında en sık başvuru yakınmaları karın ağrısı (%100), kanlı ishal (%94,5) ve tenezm (%44,4) olarak bulundu. Laboratuvar bulgularından CRP pozitifliği (%89), sedimantasyon yüksekliği (%83,3) ve demir eksikliği anemisi (DEA) (%77,7) en sık rastlanan bulgulardı. Kolonoskopik olarak en sık pankolit (%66,6) tutulumuna rastlandı. İBH’na eşlik eden diğer hastalıklar ise ailevi Akdeniz ateşi (FMF) (%11), çölyak hastalığı (ÇH) (%5,5) ve Helikobakter pylori gastriti (Hp) (%11) olarak bulundu. Pankolit tutulumu olan medikal tedaviye yanıtsız bir hastamıza (%5,5) ise kolektomi uygulandı. Sonuç: Ülkemizde çocukluk çağında İBH tanısı alan hasta sayısı her geçen gün artmaktadır. Yine de nonspesifik semptomlar ile seyreden hastaların tanınması güç olabilmekte ve tanı gecikmelerine neden olabilmektedir. İBH’nın otoimmün hastalıklar ile birlikteliği nedeniyle uygun tedaviye rağmen remisyon sağlanamayan hastalarda gastrointestinal inflamasyonla giden FMF gibi hastalıklar da araştırılmalıdır.Item Unusual autopsy finding: Simon's bleeding(Slovene Medical, 2013-09) Eren, Bülent; Akgöz, Semra; Eren, Filiz; Gündoǧmuş, Ümit Naci; Türkmen, Nursel; Çetin, Selçuk; Uludağ Üniversitesi/Tıp Fakültesi/Adli Tıp Bölümü.; 8715251700; 55891084500Objective: To determine whether or not hanging had taken place before or after the death of the victim is quite important in the identification of criminal cases pretended to be suicidal attempts. Material and Methods: Evidence for Simon's bleeding was searched in cases included in the study group after retracting soft tissue and organs away from the field of interest for full exposure of the region of lumbar spinal veretebrae without macroscopic grading of the severity of bleeding, and determining the lumbar level of bleeding. The cases autopsied in Bursa Morgue Department between 2009 and 2011 were retrospectively investigated and evaluated. Results: A total of 848 cases (664 males, and 184 females, male/female ratio, 3.6 :1) were included in the study. A total of 270 (31.8 %) cases had Simon's bleeding. A significant difference in the frequency of Simon's bleeding exists among various causes of death. A 28.5 % (n = 77) of the cases with Simon's bleeding were in the group with hanged death cases (p < 0.001). Conclusion: Splinter bleedings identified originally by Simon on the ventral, and later dorsal aspects of the spinal disci in cases of hanging were also observed later in deaths not related to hanging, however it was concluded that these types of bleeding could not be detected in cases of postmortem hanging. When we evaluated all groups, the results we obtained do not support the assertion that the incidence of Simon's bleeding decreases with aging. Instead, when compared with all other groups, the incidence of Simon's bleeding was also higher in cases of hanging with the highest mean age.
