Method: 57 consecutive patients and their caregivers were enrolled. Hamilton Depression Rating Scale (HDRS), Montgomery-Asberg Depression Scale (MADRS), Hamilton Anxiety Rating Scale (HARS), and Caregiver Burden Scale (CBS) were applied to the caregiver group. Unified Parkinson Disease Rating Scale (UPDRS), Schwab England Activities of Daily Living Scale (SES), Behavioral Pathology of Alzheimer's Disease Frequency Weighted Scale (BEHAVE-AD), Standardized Mini Mental State Examination (S-MMSE), and Geriatric Depression Scale (GDS) were applied to patients. Results: Female caregivers had significantly higher MADRS and CBS scores (p<.05), and female spouses had significantly higher MADRS scores (p<.05). Female adult children had significantly higher scores on the CBS (p<.01). Further analysis revealed that GDS and SES scores are the determinant of HDRS (R2=0.439, p<0.001). GDS scores are the determinant of CBS and MADRS scores of the caregivers (p<.05, p<.01) (R2=0.157, p<.05 and R2=0.316, p=.001, respectively). Patients' BEHAVE-AD scores are the determinant of the CBS scores of the caregivers (R2=0.145, p<.05). Conclusion: This study suggests that the behavioral problems and depression of patients with Parkinson's disease have a significant impact on the burden and depression of their caregivers. Methods: This study was carried out on 23 patients with Parkinson's disease and 21 age-and gender-matched healthy volunteers. Carotid Doppler ultrasonography was performed in order to determine CIMT in all cases. In addition, plasma homocysteine, vitamin B12 and folate levels were measured. Carotid Doppler ultrasonography and homocysteine levels were repeated one year later to evaluate the rate of atherosclerosis progression. Results: Although the population sample was limited, there was an increase in baseline CIMT measurements that occurred in parallel with an increase in baseline homocysteine levels in these patients. At the end of the follow-up period, a bilateral increase in CIMT was observed, which was not correlated to hyperhomocysteinemia. Conclusion: In conclusion, many clinical studies on hyperhomocysteinemia caused by L-dopa therapy in Parkinson patients have been published. However, the relationship between hyperhomocysteinemia and increased risk of vascular disease is still controversial. Methods: Thirty-three patients with idiopatic Parkinson's disease (PD) and 16 control subjects who are being followed by the Neurology Outpatient Clinic were included in this study. After evaluation of the cognitive functions by using MMSE (Mini-Mental State Examination) test, all cases with PD were divided into 2 groups: normal and mild dementia. Glucose, hemoglobin, TSH, cholesterol, homocysteine, vitamin B12 and folic acid levels were examined. Depression was excluded using geriatric depression test. Results: Memory, attention, verbal and visual fluency were evaluated with neuropsychological tests. Both Parkinson's groups demonstrated poor performance compared to the control group, but mental fluency, planning, verbal-visual memory, results of Trail Making Test A and counting backwards were worse in the mild dementia group. Significant difference was found between control and mild dementia groups by orientation, mental control, abstract thinking, logical memory and visuospatial tests. According to Luria's drawing test, mental fluency, and stroop, planning and RAVEN tests, poor performance was observed in the mild dementia group when compared with the other two groups. Conclusion: MMSE test may be used to diagnose dementia in PD patients because of the correlation of MMSE scores and the other memory tests. However, due to disorders detected in verbal-visual memory, mental fluency and planning tests in patients with MMSE scores above 23, the importance of neuropsychological tests is emphasized in determining subclinical cognitive dysfunction in PD patients.Item Primary gait ignition disorder: Report of three cases(Springer, 2009-08) Taşkapılıoğlu, Özlem; Karlı, Necdet; Erer, Sevda Özbek; Zarifoğlu, Mehmet; Bakar, Mustafa; Turan, Faruk; Uludağ Üniversitesi/Tıp Fakültesi/Nöroloji Anabilim Dalı.; AAK-6623-2020; 23037226400; 6506587942; 25635370800; 6603411305; 26643051200; 17636734100Gait ignition failure (GIF) classifications all had major limitations. Few years ago, a new and simpler classification was proposed by Liston. The aim of this paper is to discuss three GIF patients with respect to this new classification. All three patients presented with hesitation to start walking and turning and their neurological examination revealed start and turn hesitation without any other abnormality. We classified our patients according to Liston's classification as ignition apraxia, which enabled us to approach the patients in a practical way. This classification helps to understand the underlying pathologies and combines clinical characteristics and pathophysiology. We reported our experience with pergolide in the treatment of patients suffering from primary GIF and underline the fact that more research is needed on the treatment of this condition.
