Case 1: A 68-year-old woman, presented initially with abdominal pain, nausea, vomiting, and jaundice. She was in poor general condition, and had acute abdominal signs and symptoms on physical examination. Emergency laparotomy was performed, acute pancreatitis and leptospirosis were diagnosed on the basis of surgical findings and serological tests. The patient died on postoperative d 6. Case 2: A 62-year-old man, presented with fever, jaundice, nausea, vomiting, and malaise. Acute pancreatitis associated with leptospirosis was diagnosed, according to abdominal CT scanning and serological tests. The performances of these two time-dependent ROC curve methods were compared, in which the Kaplan-Meier estimator and weighted Kaplan-Meier estimator were used. An application was presented for pancreatic cancer patients to evaluate the prognostic ability of the CA19-9 antigen. A simulation study was performed for different scenarios to see the performance of the proposed method. In all situations, it is observed that the AUC values that were obtained by the weighted time-dependent ROC (WTDR) curves more closely approximated the real AUC values than the classical time-dependent ROC (TDR) curve method and has got smaller mean square error rates.Item Akut pankreatitte hastalık şiddetinin ve nekrozun ekzokrin-endokrin yetmezlikle ilişkisinin araştırılması(Uludağ Üniversitesi, 2008) Garip, Gökhan; Kaya, Ekrem; Uludağ Üniversitesi/Tıp Fakültesi/Genel Cerrahi Anabilim Dalı.Akut Pankreatit sonrası geç dönemde hastalık şiddetinin, nekrozun, nekroz oranının ve lokalizasyonunun organın endokrin ve ekzokrin fonksiyonlara etkisinin araştırılmasıdır. Mart 2003 - Ekim 2007 tarihleri arasında Akut Pankreatit tanısı ile tedavi edilmiş ve üzerinden 6 ay süre geçmiş olan 109 hasta çalışmaya dahi edildi. Hastalar hastalık şiddetine, nekroz oranına ve lokalizasyonuna göre sınıflandırıldı. Ekzokrin fonksiyon değerlendirmesi için sübjektif klinik değerlendirme ve dışkıda ELISA ile Fekal Pankreatik Elastaz - I (FPE - I) çalışıldı. Endokrin fonksiyon değerlendirmesi için de Oral Glukoz Tolerans Testi (OGTT) yapıldı. Hastalık şiddeti, organda nekroz varlığı, nekrozun oranı ve lokalizasyonu ile ekzokrin ve endokrin disfonksiyon arasındaki ilişki araştırıldı. Hastaların 58' i erkek, 51' i kadın olup yaş ortalaması 56.5 ± 15.7 idi. % 35.8' i Ağır Akut Pankreatit (AAP) olan hastaların % 27.5' inde pankreasta nekroz vardı. Hastaların %13.7' sinde ekzokrin disfonksiyon tespit edildi (AAP' te % 17.9, Hafif Akut Pankreatitte % 11.4). Hastaların % 34.7' sinde endokrin disfonksiyon tespit edildi. FPE - I düzeyi AAP' te, Nekrotizan Akut Pankreatit' te (NAP) ve pankreas baş lokalizasyonunda nekrozu olan hastalarda diğerlerine göre düşüktü (sırası ile ; 330 ± 170, 292 ± 178 ve 189 ± 82 M9 / 9 dışkı) (p < 0.05). Nekrozektomi yapılan hastaların % 40' ında ekzokrin disfonksiyon görüldü. Endokrin disfonksiyon AAP ve NAP' li hastalarda diğerlerine göre daha belirgindi (sırası ile % 56.4 ve % 66.6) (p < 0.001). Nekrozektomi yapılan hastaların ise tamamında endokrin disfonksiyon saptandı. The present study aimed to determine the effect of pancreaticojejunostomy (PJ) anastomosis leakage, especially on the local recurrence rate and time of recurrence, in patients that underwent pancreaticoduodenectomy (PD) for pancreatic adenocarcinoma. MATERIALS AND METHODS: This retrospective study included 64 pancreatic adenocarcinoma patients that underwent PD between January 2007 and August 2015. PJ anastomosis leakage was evaluated based on International Study Group on Pancreatic Fistula criteria. The effects of PJ anastomosis leakage on local recurrence, disease-free survival, and overall survival were assessed. RESULTS: Among the patients, 44 were male and 20 were female, and median age was 61(39-84) years. In all, 11 patients developed PJ leakage. Local recurrence occurred in 5 (45,4%) of the patients that developed PJ leakage, versus in 4 (7,5%) of the patients without leakage (p=0.02). Local recurrence developed earlier in those with leakage than in those without (p=0,013). In contrast, there weren't any significant differences in disease-free survival, or overall survival. The possibility of torsion of the spleen is high due to the long and mobile nature of the vascular pedicle. Generally, cases are asymptomatic. Under conditions of delayed diagnosis, symptoms of splenomegaly, left portal hypertension, gastric fundal varices, and hypersplenism. may present as a result of development of vascular congestion associated with chronic torsion. There are only a few cases in the literature reporting the association of wandering spleen and fundal vat-ices. We report herein the case of a 55-year-old female who admitted to our clinic with complaints of fatigue and epigastric pain. Comorbidities, operating time, hospital stay, early and late complications rate, and weight loss evolution after 1 year of follow-up were recorded.Results: The mean age was 38.41 +/- 11.05 years with a mean body mass index (BMI) of 49.02 +/- 5.89 kg/m(2) (range 38-67). Laparoscopic sleeve gastrectomy (LSG) was performed in 93 patients (92.07%) and Roux-en-Y gastric bypass (RNYGB) in 8 patients (7.92%). Thirty-day morbidity rate was 7.92% (8/101). Within a mean 9.32 +/- 2.25 (range 1-19) months follow-up time, mean percent of the excess of weight loss of 1st, 6th, and 12th months were 22.7 +/- 6.1, 67.2 +/- 11.2, and 81.4 +/- 10.5, respectively. Diabetes (n = 38, 37.6%), hypertension (n = 13, 12.9%), and obstructive sleep apnea (n = 5, 4.9%) were resolved in 76%, 68.4%, and 100% of the patients, respectively (p < 0.001).Conclusions: LSG and RNYGB are safe and highly effective, particularly in patients with a BMI >50 kg/m(2). We report herein the case of a 41-year-old woman who developed abdominal actinomycosis involving the left colon and the surrounding retroperitoneum. The patient, who had an intrauterine contraceptive device (IUCD) in situ, presented with left bank pain and the signs and symptoms of an intraabdominal mass, which was initially diagnosed as a neoplasm originating from the left colon or the retroperitoneal space. Thus, a left colectomy was performed, but the histopathological diagnosis revealed abdominal actinomycosis. We evaluated the diagnosis and treatment of this disease in light of the knowledge acquired from the available literature on this rare entity.Item Colorectal cancer in young adults(Casa Editrice Ambrosiana, 1995-07-01) Yılmazlar, Tuncay; Zorluoğlu, Abdullah; Özgüç, Halil; Korun, Nusret; Duman, Hakan; Kaya, Ekrem; Kızıl, Ayhan; Uludağ Üniversitesi/Tıp Fakültesi.; 0000-0002-9562-4195; AAG-7319-2021The study was carried out to promote a greater awareness of the potential for colorectal cancer in young adults under 40 years of age. During the 8 years between 1986 and 1993, 237 patients with adenocarcinoma of the colon and rectum were operated at the Uludag University Hospital. Of these 237 cases, 46 patients under 40 years old were reviewed retrospectively. They accounted for 19.4% of the total number of patients with carcinoma of the colon and rectum operated during the same period. Rectal bleeding was the most common presenting symptom. The mean duration of time from the onset of symptoms to diagnosis was 5.8 months. The rectosigmoid area was the most frequently involved site (80%). Seventy-six percent of the patients had Dukes' stage C or D tumors. Forty-eight percent of the tumors were either poorly differentiated or mucinous. The cumulative survival rate at 5 years was 43.4%. Patients under 40 years old with carcinoma of the colon and rectum are usually symptomatic and have advanced disease at the time of presentation. PATIENTS AND METHODS: Etiologic causes and hepatic artery hemodynamic parameters of 121 patients with chronic liver parenchymal disease were compared with MELD scores. Doppler ultrasonography (US) was used to assess flow velocity, pulsatility index (PI) and resistance index (RI) in the hepatic artery (HA). Each patient's MELD scores were calculated at the time of Doppler ultrasound performed. RESULTS: There was statistically significant difference between MELD score and hepatic artery RI value (p < 0.001, r = 0.616). This difference was statistically more significant in the group which consisted of multiple etiologic causes (p < 0.001, r = 0.837). Çalışmamızda, Ocak 2018 – Haziran 2021 tarihlerinde 20-89 yaş arası 28 farklı kadaverik donörden alınan 28 arter, 26 ven iliyak damar grefti kullanılmıştır. Saklanma sürelerine göre greftler taze, 0-6 ay, 6-12 ay, 12- 24 ay ve 24 ay üzeri olacak şekilde gruplandırıldı. Çözdürülen greftler steril ortamda kesit alınarak üreme varlığı açısından değerlendirildi. Histopatolojik değerlendirme için örnekler ışık mikroskobunda internal elastik lamina (İEL) bütünlüğü, tunika media kalınlığı ve morfolojik inceleme sonuçlarıyla karşılaştırıldı. Endotel morfolojisinin değerlendirilmesi için örnekler taramalı elektron mikroskobunda (SEM) incelenerek skorlama sistemiyle karşılaştırıldı. Mekanik test için sağ ve sol ana iliyak damarlar in vitro anastomoz edilerek anastomoz patlama basıncı karşılaştırıldı. Donörlerin gruplar arası demografik verileri benzerdi. Saklanma süresine göre greftlerin patlama basıncı arasında farklılık yoktu. Mikrobiyolojik analizde hiçbir greftte kontaminasyon gözlenmemiştir. Işık mikroskobide damarların tunika media kalınlığı gruplar arası farklılık yoktu. Arterlerin İEL fragmantasyonunda farklılık yok iken reduplikasyonda 24 ay üzeri örneklerde anlamlı olarak artış göstermekteydi. Tunika mediada ödem, hyalinizasyon, bağ dokusu artışı ve yer yer nekroz alanları izlenmiştir. SEM incelemede endotel yapısı 12 ay öncesi saklanan greftlerin çoğu grade 1 ve grade 2 izlenirken 12 ay üzeri saklanan greftlerde grade 3, grade 4 ve grade 5 görünüm mevcuttu. Materials and methods: Donors with any 1 of following were considered marginal donors: age >= 65 years, sodium level >= 165 mmol/L and cold ischemia time >= 12 h. Donors were classified according to the donor risk index (DRI) <1.7 and >= 1.7. The transplant recipients' model for end-stage liver disease (MELD) scores were considered low if <20 and high if >= 20. Early graft dysfunction (EGD) and mortality rate were evaluated. Results: During the study period 47 patients underwent cadaveric LT. The mean age of the donors and recipients was 45 years (range: 5-72 years) and 46 years (range: 4-66 years), respectively. In all, there were 15 marginal donors and 18 donors with a DRI > 1.7. In total, 4 LT patients that received livers from marginal donors and 5 that received livers from donors with a DRI >= 1.7 had EGD. Among the recipients of marginal livers, 5 died, versus 4 of the recipients of standard livers. There was no significant difference in EGD or mortality rate between the patients that received livers from marginal donors or those with a DRI = 1.7 and patients that received standard donor livers. Therefore, it may be expected that the increase in intra-abdominal pressure during laparoscopic surgery can augment the absorption of bacteria from the peritoneum to the blood. The aim of this study was to evaluate the effect of peach nectar containing high fructose corn sirup (HFCS) consumption in a pancreatic carcinogenesis rat model induced by 7,12-Dimethyl benzanthracene (DMBA). Fifty-day-old male Sprague Dawley rats were fed with peach nectar containing HFCS + chow, peach nectar containing sucrose + chow and only chow. After 8 mo, feeding period, each group was divided into two subgroups, in which the rats were implanted with DMBA and no DMBA (sham). Histologic specimens were evaluated according to the routine tissue processing protocol. The animals withad libitumaccess to pn-HFCS, pn-sucrose and chow (only) showed significant differences in chow consumption and glucose level. Necropsy and histopathologic findings showed tumor formation in the entire group treated with DMBA. Excluding one rat in chow group, which was classified as poorly differentiated type, the others were classified as moderately differentiated pancreatic ductal adenocarcinoma (PDAC). The aim of this study was to investigate the role of L-glutamine and short-chain fatty acids, both via enema and oral administration, on mucosal healing in experimental colitis. Methods: Colitis was induced with trinitrobenzenesulphonic acid in ethanol enema in rats. Saline enema (Colitis group, n: 12), L-glutamine enema (n: 12), short-chain fatty acids enema (n: 12), oral L-glutamine (n:11) and oral short-chain fatty acids (n. 11) were applied twice daily for 10 days after induction of colitis. The sham group (n: 12) received only saline enema. Rats were sacrificed at the tenth day. Crypt depth and DNA content were measured in colonic mucosa. Results: Crypt depth was significantly greater in both glutamine groups and short-chain fatty acids enema group than in sham and colitis groups (p<0.05). The mucosal DNA contents of the colitis and glutamine enema groups were significantly greater than both short-chain fatty acids groups (p<0.05). DNA content in the oral glutamine group was significantly greater than in the short-chain fatty acids enema group (p<0.05). Conclusions: L-glutamine enema can accelerate mucosal healing and regeneration in experimentally induced colitis in rats.
