Item Hepatosellüler karsinomda ilaç salınımlı mikrosferlerle transarteriyel kemoembolizasyon (DEB-TAKE) tedavisinin etkinliğinin retrospektif değerlendirilmesi(Bursa Uludağ Üniversitesi, 2022) Tonkaz, Mehmet; Nas, Ömer Fatih; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Radyoloji Anabilim Dalı.HEPATOSELLÜLER KARSİNOMDA İLAÇ SALINIMLI MİKROSFERLERLE TRANSARTERİYELÇalışmamızın amacı, Hepatosellüler Karsinom (HSK) tanısı alan uygun hasta gruplarında ilaç yüklenebilir mikrosferlerle transarteriyel kemoembolizasyon (DEB-TAKE) tedavisinin etkinliğini değerlendirmek ve sonuçlarını literatürle karşılaştırmaktır. Bu çalışmada Ocak 2015-Aralık 2020 tarihleri arasında Bursa Uludağ Üniversitesi Radyoloji Anabilim Dalı Girişimsel Radyoloji Ünitesi’nde doksorubisin yüklü 30-60 μm mikrosferlerle (Hepasphere; Merit Medical, South Jordan, UT, USA) transarteriyel kemoembolizasyon tedavisi uygulanmış 88 hasta retrospektif olarak incelendi. Tüm hastaların demografik ve klinik özellikleri, laboratuvar parametreleri kaydedildi. Radyolojik görüntüleme yöntemleriyle (Bilgisayarlı Tomografi veya Manyetik Rezonans Görüntüleme) işlem öncesi ve sonrasında tümör boyutu, dağılımı, sayısı, kronik karaciğer hastalığı varlığı, portal ven trombozu ve asit varlığı değerlendirildi. DEB-TAKE sonrasında tedaviye yanıt değerlendirmesi, mRECIST (Modified Response Evaluation Criteria in Solid Tumors) kriterlerine göre yapıldı. Progresyona kadar geçen süre (TTP), progresyonsuz sağkalım (PFS), genel sağ kalım ve advers etkiler kaydedildi. Hastaların medyan (ortanca) takip süresi 17 ay (2-64 ay) idi. Toplam 134 seans DEB-TAKE prosedürü uygulanmış 88 hastada (74 erkek, 14 kadın) derece (grade) 4 veya 5 advers etki izlenmedi. 8 hastada (%9,1) tam yanıt (TY), 42 hastada (%47,8) parsiyel regresyon (PR), 10 hastada (%11,3) stabil hastalık (SH) ve 28 hastada (%31,8) progresif hastalık (PH) izlendi. Ayrıca 50 hastada (%56,9) objektif yanıt (TY+PR) izlendi. DEB-TAKE tedavisinden önceki ve 1 ay sonraki serum AFP değerleri arasında istatistiksel olarak anlamlı farklılık saptandı (p<0,001). DEB-TAKE tedavisi sonrasında ortotopik karaciğer transplantasyonu yapılmış 17 hasta genel sağkalım analizlerinden çıkartıldı. iv Medyan (ortanca) genel sağkalım 17 ay (%95 güven aralığı, 10,3-23,7) olarak bulundu. 6 aylık, 1, 2, 3 ve 4 yıllık genel sağkalım oranları sırasıyla %86, %70,2, %40,2, %19,7 ve %14,7 idi. Tek değişkenli ve çok değişkenli Cox regresyon analizinde Child Pugh sınıflaması (B vs A) (HR: 3,5; %95 GA, 1,232-10,108 p=0,019), TAKE seans sayısı (1 vs >1) (HR: 2,95; %95 GA, 1,443-6,028 p=0,003) ve işlem öncesi serum alfa fetoprotein (AFP) değerinin (400+ vs <400) (HR:2,98; %95 GA, 1,152-7,717 p=0,024) sağkalımı etkileyen bağımsız risk faktörleri olduğu saptandı. Hastaların medyan progresyonsuz sağkalımı (PFS) 8 ay (%95 GA, 5,8-10,2) olarak bulundu. PFS, BCLC (Barcelona Clinic Liver Cancer) A’da 14 ay (%95 GA, 9,9 -18,1 ) ve BCLC B’de 8 ay (%95 GA, 6,4-9,5) olup iki grup arasındaki fark istatistiksel olarak anlamlı değil ancak kritik değere yakındır (p=0,055). BCLC A ve B HSK hastalarında, doksurubisin yüklü Hepasphere 30-60 μm mikrosferlerle yapılan transarteriyel kemoembolizasyon (DEB-TAKE) güvenli ve etkili bir tedavi yöntemidir. Methods This study included 540 patients. Contrast-enhanced magnetic resonance venography combined with conventional sequences was considered the gold standard test (GST), while I3DV was considered as diagnostic test. We accessed the diagnostic success of I3DV for intracranial venous thrombosis detection, thrombosed vessel identification, and total/partial thrombus distinction. The possible relationship between false-positive thrombus diagnosed by I3DV and venous sinus hypoplasia or aplasia diagnosed by GST was investigated. Results The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of I3DV in the detection of intracranial venous thrombosis were 95.7%, 92.1%, 64.1%, 99.3%, and 92.6%, respectively. A significant association was observed between I3DV and GST in terms of thrombosis detection and total/partial thrombus distinction (p < 0.001). A significant relationship was observed between false-positive thrombosis diagnosis in I3DV and hypoplasia in the left transverse sinus (p < 0.001). Conclusion Intracranial venous thrombosis may be diagnosed faster and more accurately than traditional phase contrast magnetic resonance angiography in I3DV. This technique can be used in situations where contrast medium application is contraindicated. Methods: 45 patients clinically identified with BD and evaluated with CTA were included. IOA of five different scoring systems used for CTA BD diagnosis, the effect of intracranial interventions on scoring systems, and vascular opacification were evaluated. Results: IOA was almost perfect (Kappa = 0.843-0.911, p < 0.05) and substantial (Kappa = 0.771-0.776, p < 0.05) in all scoring systems. Significant relationships were observed between craniectomy presence and middle cerebral artery M4 segment and internal cerebral vein (ICV) opacification. No opacification was observed in straight sinus (SS) by observers in any of the craniectomized patients. Conclusion: IOA of CTA scoring systems is adequate. But a significant degree of false-negative results is observed due to ICV filling in craniectomy cases. Opacification presence in SS can give an idea of BD in these cases.
