Ödemin eşlik ettiği, hedef tahtası benzeri hemorajik plaklarla karakterizedir. Lezyonlar yüz ve ekstremite distallerine yerleşir. Hastalık sekelsiz, kendiliğinden iyileşebilmektedir. Olgu Sunumu: Burada akut infantil hemorajik ödemli 8 aylık bir kız çocuğu sunulmuştur. Tartışma: Hem dermatoloji hem pediatri doktorları lökositoklastik vaskülitin bu nadir görülen tablosuna aşina olmalıdır. BHK'ların tedavisinde cerrahi, halen altın standart olarak yerini korumaktadır. Buna rağmen, cerrahiye uygun olmayan ve kozmetik kaygıları olan bireylerde imikimod krem ve intralezyonel interferon (IFN) alfa2b gibi, daha az invaziv, cerrahi olmayan seçenekler de bulunmaktadır. Yalnız başına imikimod ya da imikimod ile kombine olarak interferon alfa-2b ile başarılı bir şekilde tedavi ettiğimiz değişik alt tiplerde 11 BHK vakasını sunduk. 2005-2010 yılları arasında polikliniğimizde histopatolojik olarak BHK oldukları kanıtlanmış, tek başına ya da IFN alfa-2b ile kombine olarak imikimod ile tedavi edilmiş çeşitli alt tiplerdeki hastalar bu rapora dahil edildi. Sunduğumuz 11 hastadan sadece 4 'ü (3 infiltratif, 1 solid tipler) topikal imikimoda ek olarak haftada 3 gün intralezyonel interferon alfa2b 3 milyon IU tedavisi de almıştı. Geri kalan 7 hasta sadece imikimod %5 krem kullanmıştı. Tüm hastalarda bu tedavi seçenekleri ile tam iyileşme sağlandığı görüldü. Bu olgulardaki gözlemlerimiz, İmikimodun sadece yüzeyel değil; infiltratif, solid, nodüler tip BHK'larda da etkili olduğunu düşündürmektedir. İmikimod, sadece yüzeyel değil, aynı zamanda, infiltratif, solid ve nodüler tip BHK'larda da etkili olduğunu düşündürmektedir. Gereç ve Yöntem: Uludağ Üniversitesi Tıp Fakültesi Dermatoloji Anabilim Dalı, Çocuk Nöroloji Bilim Dalı ve Çocuk Alerji Bilim Dalı’na 2000-2008 yılları arasında başvuran SJS ve TEN tanısı almış olan hastalar retrospektif olarak incelenmiştir. Bulgular: Toplam 14 hastanın (7’si kız ve 7’si erkek) kayıtları incelendi. Hastaların 9’u SJS, 2’si SJS-TEN overlap sendrom, 3’ü TEN idi. Dokuz hastanın epilepsi tanısı ile, diğer hastaların anksiyete, diş apsesi, kriptik tonsillit, otitis media ve bronşit tanıları ile ilaç kullanım öyküleri mevcuttu. Hastaların 11’inde (%78,5) çoklu ilaç kullanımı bulunmaktaydı. SJS/TEN etyolojisinde 7 hastada (%50) Lamotrijin (7 hastanın 4'ünde ek olarak Valproik asit kullanımı vardı) diğer hastalarda Okskarbamazepin, Azitromisin, Fenitoin, ‹mipramin, Sefuroksim aksetil, Siproşoksasin, Seftriakson kullanımının sorumlu olduğu düşünüldü. Oniki hastaya sistemik kortikosteroid, 2 hastaya intravenöz immünglobulin tedavisi uygulanmıştı ve hastaların tümünde tam iyileşme görüldüğü saptandı. Mannose-binding lectin (MBL) gene polymorphisms decrease MBL serum levels, thereby increasing the synthesis of proinflammatory cytokines such as TNF-. Objectives This trial was designed to evaluate the role of the MBL2 codon 54 polymorphism in the pathogenesis of psoriasis. Methods Fifty patients diagnosed with psoriasis vulgaris and 53 healthy subjects were included in the trial. The polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method was applied to determine the MBL2 codon 54 polymorphism. Genotypes were determined according to the bands formed in agarose electrophoresis gels. For the statistical analysis, the level of significance was set at P<0.05. Results A total of 33 (66.0%) of the 50 psoriasis patients were detected to have A/A genotype and 17 (34.0%) had B/B genotype. Of the control subjects, 44 (83.0%) had A/A genotype and nine (17.0%) had B/B genotype. There was a statistically significant difference between the groups (P=0.047). The analysis of allele frequencies revealed A allele prevalences to be 79 (79.0%) and 95 (89.6%), and B allele prevalences to be 21 (21.0%) and 11 (10.4%), in the patient and control groups, respectively. A statistically significant difference between allele frequencies was detected (P=0.031). Conclusions This study suggests that the MBL2 codon 54 polymorphism may have an association with psoriasis in the Turkish population.Item Eritema multiforme, Stevens Johnson sendromu ve Stevens Johnson sendromu/toksik epidermal nekroliz overlap sendrom'da toll like reseptör polimorfizmi(Uludağ Üniversitesi, 2008) Turan, Hakan; Başkan, Emel Bülbül; Uludağ Üniversitesi/Tıp Fakültesi/Dermatoloji Anabilim Dalı."Toll like reseptör'' (TLR) 9, farklı viral ve bakteriyel patojenlere karşı doğal immün sistemin devreye girmesini sağlayarak, kazanılmış immün yanıtın etkili bir şekilde oluşmasında görev almaktadır. TLR 9 gen polimorfizmi halinde, reseptör fonksiyonları bozulacağından, konak mikrobiyal patojenlere duyarlı hale gelir. Bu çalışmada, TLR 9 gen polimorfizmininin viral ve bakteriyel enfeksiyonları takiben deri ve mukozalarda yaygın, şiddetli bir inflamasyona neden olabileceği düşüncesinden yola çıkarak, "eritema multiforme'' (EM), "Stevens Johnson sendromu'' (SJS) ve "Stevens Johnson sendromu/toksik epidermal nekroliz overlap sendrom''a (SJS/TEN) yatkınlık oluşturup oluşturmadığı araştırılmıştır.Çalışmamıza 42 hasta ve kontrol grubu olarak da 50 sağlıklı birey alındı. TLR 9 geni 1237 Timin/Sitozin (T/C) polimorfizmi için polimeraz zincir reaksiyonu-restriksiyon fragment uzunluk polimorfizmi (polymerase chain reaction-restriction fragment length polymorphism=PCR-RFLP) yöntemi uygulandı. Agaroz jel elektroforezinde oluşan bantlara göre genotip tayini yapıldı. İstatistiksel değerlendirme Uludağ Üniversitesi Tıp Fakültesi Biyoistatistik Anabilim Dalı'nda SPSS 13.0 paket programı kullanılarak yapıldı.Hasta grubunda TT genotipi oranı %73,8, TC genotipi oranı %26,2 iken CC genotipi saptanmadı; kontrol grubunda TT genotipi oranı %74, TC genotipi oranı %24 ve CC genotipi oranı %2 bulundu. TT, TC ve CC genotipleri açısından hasta grubu ile kontrol grubu karşılaştırıldığında istatistiksel olarak anlamlı fark saptanmadı. Hasta grubu ve sağlıklı bireylerde T ve C alel sıklığı sırasıyla %84,5 ve %15,5; kontrol grubunda % 86 ve %14 olarak hesaplandı.Bu çalışma EM, SJS ve SJS/TEN overlap sendrom'un TLR ile ilişkisini inceleyen ilk çalışma olma özelliği taşımaktadır. Subjects and Methods: In this case-control study, 105 patients with plaque-type psoriasis and 102 healthy controls were recruited from the dermatology outpatient clinics of two university hospitals. Genomic DNA was extracted from whole blood using a DZ DNA isolation kit. Multiplex PCR was used to determine GSTM1 and GSTT1 polymorphisms in the isolated DNAs. Results: Of the 150 patients with psoriasis, 83 (79%) were identified with the GSTT1 genotype and 22 (21%) with the null genotype. Of the 102 patients in the control group, 69 (67.6%) subjects were identified with the GSTT1 genotype and 33 (32.4%) with the null genotype. There was no significant difference between the patient and control groups (p = 0.063). Regarding the GSTM1 polymorphism, 54 (51.4%) patients were identified with this genotype and 51 (48.6%) with the null genotype; in the control group, 50 (49%) were identified with this genotype and 52 (51%) with the null genotype. Again there was no statistically significant difference between the groups (p = 0.957). Conclusion: In this Turkish cohort of patients with psoriasis, neither GSTT1 nor GSTM1 polymorphisms were associated with disease susceptibility. Topikal antibiyotiklerin benzoil peroksit ile kombinasyonu etkinliği artırır ve Propionibacterium acnes’e karşı gelişen antibiyotik direncini azaltır. Sodyum sulfasetamid sülfonamid grubu bir antibiyotiktir. Uzun yıllardır bilindiği halde akne tedavisinde fazlaca yer almamaktadır. Ancak son zamanlarda kullanımı tekrar gündeme gelmiştir. Çalışmamızda hafif ve orta derecedeki akne vulgariste sodyum sulfasetamidin etkinliğini klindamisin ve eritromisin ile karşılaştırmalı olarak değerlendirmeyi amaçladık. Gereç ve Yöntem: Karşılaştırmalı çalışmamızda akne vulgarisli 60 hasta, birinci grup sodyum sulfasetamid %10 losyon, ikinci grup klindamisin %1 losyon ve üçüncü grup eritromisin %2 jel 12 hafta süre ile günde iki kez uygulamak üzere, randomize olarak 3 gruba ayrıldı. Her grupta 20 olgu yer aldı. Tedavi tüm hastalarda benzoil peroksit %5 losyon ile kombine edildi. Hastalar başlangıçta, 4, 8 ve 12. haftalarda noninflamatuvar (açık ve kapalı komedo) ve inflamatuvar (papül ve püstül) lezyon sayıları açısından değerlendirildi ve yan etki ler kaydedildi. Bulgular: Gruplar arasında yaş, cinsiyet ve akne süresi yönünden anlamlı fark yoktu (p>0,05). Çalışma sonunda her 3 tedavi yöntemi ile de istatistiksel olarak anlamlı azalma saptandı (p<0,05). Noninflamatuvar lezyon sayısı ve total lezyon sayısındaki azalma açısından 4, 8 ve 12. haftalarda her 3 grup arasında anlamlı fark saptanmazken inflamatuvar lezyon sayısındaki azalma açısından 8. haftada klindamisin diğer tedavilere göre daha üstün bulundu (p<0,05). Yan etki görülme oranları açısından da her 3 grup arasında anlamlı fark saptanmadı (p>0,05). Sonuç: Bu çalışmada topikal sodyum sulfasetamidin, hafif ve orta şiddette akne vulgarisin tedavisinde, benzoil peroksitle kombine kullanıldığında klindamisin ve eritromisin gibi etkili ve güvenilir olduğu sonucuna varılmıştır. The genetic polymorphisms in the MBL2 gene can result in a reduction of serum levels, leading to a predisposition to recurrent infection. The aim of this study is to investigate the influence of a polymorphism in codon 54 of the MBL2 gene on the susceptibility to Erythema Multiforme, Stevens-Johnson Syndrome and Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis Overlap Syndrome (EM, SJS and SJS/TEN overlap syndrome). Material and Methods: Our study included 64 patients who were clinically and/or histopathologically diagnosed with EM, SJS, and SJS/TEN overlap syndrome and 66 healthy control subjects who were genotyped for the MBL2 gene codon 54 polymorphism using the PCR-RFLP method. For all statistical analyses, the level of significance was set at p<0.05. Results: The prevalence of the B allele was 18% in the EM, SJS and SJS/TEN patient groups and 13% in the control group. No significant differences in allele frequencies of any polymorphism were observed between the patient and control groups, although the B allele was more frequent in the patient groups (p=0.328). Conclusion: Our results provide no evidence of a relationship between MBL2 gene codon 54 polymorphism and the susceptibility to EM, SJS and SJS/TEN overlap syndrome. With the help of history, physical examination and histopathological features, the patient was diagnosed with iododerma secondary to topical povidone-iodine use. Iododerma develops frequently after oral or intravenous but rarely after topical use of iodine. MBL2 gene polymorphisms affect MBL serum levels. Therefore, this increases the risk of infection and may result in predisposition to autoimmune diseases. The aim of this study was to investigate whether there is an association between the MBL2 gene codon 54 (allele B: rs1800450, c. 161G>A; p. 54Gly>Asp) polymorphism and vitiligo in Turkish patients. One hundred and one patients who were diagnosed with vitiligo and 101 control subjects were included in the study. The DNA was analyzed using the Kbioscience Competitive Allele Specific PCR (KASP) technique. MBL2 gene codon 54 polymorphism frequencies were compared between the two groups. In statistical analysis, the level of significance was set at p<0.05. No significant differences in frequencies of the A allele were observed between the patient and control groups. It was observed at similar frequencies in both groups (p=0.890). Multiple miliary osteoma cutis of the face is a rare complication of chronic inflammatory acne vulgaris and has invasive and non-invasive treatment alternatives different from acne vulgaris. Invasive techniques should be simple, easy, and inexpensive, with minimal risk of scarring and pigmentation. We used a needle microincision-extirpation technique in a patient with multiple miliary osteoma cutis unresponsive to non-invasive treatment modalities. Skin overlying the papules was incised with a needle and then the calcificated papules were extirpated by using a small curettage device. Lesions were left to secondary healing. The lifetime incidence of nail involvement increases up to 80-90% for psoriatic patients. Nail psoriasis is considered a significant social problem. Many topical agents have been used for psoriatic nails with various side effects and some benefits; management is currently inconclusive. Methotrexate (MTX) is a folic acid analog, which irreversibly binds to dehydrofolate reductase and blocks deoxyribonucleic acid synthesis. It is considered a potential treatment option for rapidly growing cells and has an anti-inflammatory effect through inhibition of the polyamine pathway in autoimmune diseases. Intralesional MTX has been used successfully for various indications. We present a case successfully treated with low-dose intralesional MTX with no observed side effects in a 26-year-old female psoriatic patient suffering from nail dystrophy. In contrast, conventional topical and systemic therapies have various side effects, which limit their use. The purpose of the present study was to assess the efficacy of NB-UVB phototherapy in the management of pityriasis lichenoides (PL). Methods: The therapeutic response in 31 PL patients (23 pityriasis lichenoides et varioliformis acuta; PLEVA, eight pityriasis lichenoides chronica; PLC) treated with NB-UVB phototherapy between 2000 and 2007 was assessed. Results:NB-UVB treatment led to a complete response (CR) in 15 out of 23 PLEVA patients (65.2%) with a mean cumulative dose of 23 J/cm(2) after a mean number of 43.4 exposures and a partial response (PR) in eight patients (34.8%) with a cumulative dose of 15.6 J/cm(2) after a mean number of 32.3 exposures. NB-UVB treatment led to CR in seven out of eight PLC patients (87.5%) with a mean cumulative dose of 18.4 J/cm(2) after a mean number of 45.8 exposures and PR in one patient (12.5%) with a cumulative dose of 9.1 J/cm(2) after a mean number of 19 exposures. Relapses occurred in four PL patients within a mean time period of 6 months. Objective: We evaluated the efficacy of oral cyclosporine in refractory vulvar LS. Methods: Five patients with refractory vulvar LS were treated with oral cyclosporine (3-4 mg/kg/d) for 3 months. They were followed up on a monthly basis. Results: At the end of the treatment, the mean total symptom score regressed significantly and clinical findings such as erythema and erosion showed marked improvement. Mild adverse effects were seen in 3 patients. Limitations: The patients did not give consent to rebiopsy at the end of the treatment. The etiology of the disease is totally unknown, however, many epidemiological and clinical studies have suggested that infectious agents may cause the disease. Seasonal changes in the incidence may be an epidemiologic evidence for potential infectious etiology. In this study, we aimed to analyze the demographic data of PR patients and to explore the role of climatic factors in the etiology of the disease. Material and Method: We retrospectively reviewed the patient files of PR cases that had been followed up in our clinic between 2000 and 2005. Demographic data of the patients as well as the date of applications were recorded. Temperature, raining, pressure and humidity data for the City of Bursa for years 2000-2005 were obtained from the General Directorate of Meteorology, Republic of Turkey. Any potential relationship between onset time of PR and meteorological parameters was investigated statistically by using cluster analysis. Results: We reviewed the medical records of 413 patients, of whom 271 were female and 142 were male. Herald plaque was seen in 88 patients (21.3%). Pityriasis rosea was observed predominantly in persons between 20 and 29 years of age (139 patients; 33.6%). Distribution of number of cases between 2000-2005 was 51, 57, 80, 75, 63, 87. The highest number of patients was seen in winter (n:122; 29.5%) followed by spring (n:101; 24.4%), autumn (n:101; 24.4%) and summer (n:89; 21.7%). No statistically significant difference was found between annual and seasonal changes in the incidence of PR (p>0.05).
