This condition may be mistaken for orbital trauma. In this report, a 10 months-old patient with metastatic orbital neuroblastoma who was previously diagnosed as orbital trauma has been presented. The presenting sypmtoms were left proptosis and the appearance of racoon eyes. The diagnosis of neuroblastoma was made by typical radiological study, blood and urine findings and bone marrow aspiration biopsy. The ophthalmological findings regressed after multiple chemotherapy with multidrug and disappeared 5 months after the treatment. However, 10 months after the initiation fo the treatment, the patient was lost due to bone and lung metastasis and fulminating sepsis secondary to febrile neutropeni.Publication Anatomical and the refractive outcomes in the eyes undergoing laser photocoagulation for retinopathy of prematurity(Galenos Yayincilik, 2009-09-01) YILDIZ, MERAL; ÖZMEN, AHMET TUNCER; YALÇINBAYIR, ÖZGÜR; Yalçınbayır, Özgür; Ayvaz, Ayse Tor; Kaderli, Berkant; Avcı, Remzi; Gelişken, Öner; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.; 0000-0002-7311-5277; 0000-0001-9581-4856; AAH-6625-2021; IYJ-9408-2023; JAC-9459-2023Aim: This study aimed to evaluate the anatomical and the refractive outcomes in cases that underwent laser photocoagulation (LP) for retinopathy of prematurity (ROP).Materials and Method: Medical records of 38 cases that underwent LP for ROP in Uludag University Department of Ophthalmology, between 2004 and 2008, were reviewed. LP was performed in 73 eyes. Stages of ROP before and after LP were recorded. Additionally gestational age and birth weight, complications, sciascopic refractions were assessed and visual acuities were evaluated with "Teller Acuity Cards".Results: Gestational age ranged from 26 to 34 weeks, (mean 29+/-2 weeks), birth weight ranged from 850 g to 1950 g (mean 1262+/-304 g). Among all cases in the study 89%, responded with a remarkable regression of ROP with LP treatment. However, 8 eyes progressed to stage 4 and 5 despite treatment. Three eyes developed vitreous hemorrhage and 2 eyes developed preretinal hemorrhage. In the 6th month of the corrected age, spherical equivalent ranged between -8D to + 2.5D, however it ranged between -11D to + 2D in the 12th month. Hyperopia was present in 53% of all eyes and myopia was present in 25.5%. Mean visual acuity was 3.5 cpd (1.3-6.4) in the 6th month of the corrected age whereas it was 6.2 cpd (1.3-9.6) in the 12th month.Conclusion: Adequate and appropriate treatment with LP, has led to fair visual outcomes and an evident regression of ROP in all cases of the study group. This study aims to compare the efficacy and safety of infliximab (IFX) and adalimumab (ADA) in pediatric noninfectious uveitis that were refractory to conventional immunosuppresives.Methods: This retrospective single-center study included 33 patients who were treated with anti-tumor necrosis factor (TNF) agents (16 with IFX and 17 with ADA). Patients had diverse etiologies, including juvenile idiopathic arthritis, idiopathic uveitis, and Behcet's disease. Demographic characteristics, systemic diagnosis, findings of the ophthalmological examination, control of ocular inflammation, response to treatment, and the rate of clinical remission were studied.Results: Fourteen (87.5%) patients receiving IFX and 10 (58.8%) patients receiving ADA achieved response to treatment during the follow-up (P = 0.118). The agents were discontinued with complete clinical remission in 6 (37.5%) patients receiving IFX and in 2 (11.8%) patients receiving ADA (P = 0.118). Increased number of Newborn Intensive Care Units leads to survive of increased number of immature babies, therefore, there is an increased number of premature infants to be screened and eyes to be treated. The aim of this review is to evaluate the current improvements and treatment modalities in retinopathy of prematurity. Age, gender, the cause of the trauma, presence of intraocular foreign body and the extent of the ocular damage were recorded.Results: The study was comprised of 79 patients (23 female, 56 male) with a mean age of 7.7+/-4.4 (range: 2-18 years). Thirty-nine patients were injured with a blunt tip object, 27 with a sharp object and one has rupture with trauma. Thirty-five injuries occurred outside and 45 at home. The season in which most of the injuries took place was summer (32%). Seventy-five percent of the injuries were noted in 11 years and under. The mean age subject to blunt tip object injury, organic body injury and outdoor injury was significantly higher compared to that of with sharp object, inorganic body injury and indoor injury.Conclusions: The open globe-injury rates in Northwest Turkey is found to be higher in smaller age groups. Localized OLE involving the eyelid is seen rarely. In this report, a 28 year-old male who had a localized OLE involving the eyelid has been presented. The examination findings of the patient were a welldefined, slight elevated erythema through the right lower eyelid, scaling on the lesion and madarosis. The diagnosis was made by observing the typical histopatological changes such as hyperkeratosis, lymphoid infiltration at the dermoepidermal junction and telangiectasis in the substantia propria. The eyelid lesion totally disappeared with the treatment of 3 months-systemic hydorxychloroquine (200 mg/day). However, it recurred in the same region 14 months later. Discoid lupus did not recur after the second-systemic hydorxychloroquine treatment. Herpes zoster ophthalmicus-(HZO) related cranial nerve palsy is a transient and self-limiting condition. Systemic antiviral treatment is administered in order to prevent sight-threatening complications. In suspected cases, zosteriform rash should be questioned. Currently, its etiology is not fully understood. In this study we aimed to investigate the correlation between a patient group with ROP and a control group in terms of the tumor necrosis factor- alpha (TNF-alpha) (G308A) gene and glutathione-S-transferase P1 (GSTP1) (Ile105Val) gene polymorphism.Materials and methods: Sixty-two patients diagnosed with ROP and 58 control subjects were included in this study. For TNF-alpha (G308A) gene and GSTP1 (Ile105Val) gene polymorphisms, the polymerase chain reaction- restriction fragment length polymorphism method was used. In statistical analysis the significance level was determined as P < 0.05.Results: When the patient and control groups were compared in terms of TNF-alpha (G308A) gene and GSTP1 (Ile105Val) gene polymorphisms, no statistically significant difference was found (P > 0.05).Conclusion: In our study, no correlation was identified between TNF-alpha (G308A) gene and GSTP1 (Ile105Val) gene polymorphisms and susceptibility for development of ROP. This study aims to describe a novel technique for 23-gauge (G) sclerotomy closure in TSV surgery and discuss its effectiveness. Materials and Methods:Two hundred and thirty cases of 180 patients who underwent 23G TSV with various diagnoses were included to the study. Cases with connective tissue diseases, thin sclera, and significant conjunctival and scleral scarring were excluded. Nonperforating intrascleral stabbing (NIS) was performed to leaking 23G sclerotomies (n = 650) and 27G chandeliers (n = 84). Demographics, rate of sclerotomy closure with NIS procedure, the need for suturing, and complications of the procedure were recorded. Results:The overall success of NIS was found to be 91.0% in sclerotomies. Although 9.1% of sclerotomies required sutures, 592 of the 650 sclerotomies could be closed with NIS procedure (P < 0.001). Nonperforating intrascleral stabbing procedure helped close 98.8% of leaking 27G chandeliers. An average of 1.28 0.52 NIS attempts were needed for successful sclerotomy closure. Closure of sclerotomies with the NIS technique were somewhat related to the location of the sclerotomy, history of TSV and NIS, duration of TSV, and type of endotamponade. Hypotonia and choroidal detachment were seen in one case. No additional interventions were needed to normalize intraocular pressure in any case. Subconjunctival hemorrhage happened to be the leading complication of the procedure. Conclusion:NIS procedure seems as a practical, reproducible, cost-effective, and uncomplicated approach, which significantly reduces the need to suture 23G sclerotomies. A total of 40 infants were enrolled in the study. The retinal examination time was determined in accordance with the American Academy of Pediatrics recommendations for retinopathy of prematurity screening and follow-up. Retinopathy of prematurity was classified according to the international classification of retinopathy of prematurity. Serum Insulin like growth factor 1 levels were measured serially in blood samples on the 1st, 3rd, 7th, 21st, and 28th day.Results: Among the 40 infants, 11 (27.5%) constituted the retinopathy of prematurity group and 29 comprised the non-retinopathy of prematurity group. In the retinopathy of prematurity group, the mean gestational age and birth weight was significantly lower. The demographic features of the study cohort were similar. The duration of mechanical ventilation was significantly greater in the retinopathy of prematurity group compared with the non-retinopathy of prematurity group (p=0.036). In terms of neonatal morbidities such as respiratory distress syndrome, intraventricular hemorrhage, bronchopulmonary dysplasia, patent ductus arteriosus, and necrotizing enterocolitis, no differences were detected between the groups. The mean serum insulin-like growth factor-1 levels in retinopathy of prematurity group were significantly lower than those in the non-retinopathy of prematurity group at each time point (1st, 3rd, 7th, 21st, and 28th day of postnatal life) (p=0.001).Conclusions: This study demonstrated the low serum insulin-like growth factor-1 levels was associated with retinopathy of prematurity development.
