Bu tümörler içerisinde yer alan 146 intrakranial meningioma olgusuna ait dökümanlar tekrar gözden geçirilmiştir. We aimed to investigate types and frequencies of mismatch repair (MMR) gene mutations in Turkish patients with HNPCC and to identify specific biomarkers for early diagnosis of their non-symptomatic kindred's. The molecular characteristics of 28 Turkish colorectal cancer patients at high-risk for HNPCC were investigated by analysis of microsatellite instability (MSI), immunohistochemistry and methylation-specific PCR in order to select tumors for mutation analysis. Ten cases (35.7%) were classified as MSI (+). Lack of expression of the main MMR proteins was observed in MSI (+) tumors. Hypermethylation of the MLH1 promoter region was observed in one tumor. Nine Lynch syndrome cases showed novel germ-line alterations of the MMR gene: two frame-shifts (MLH1 c.1843dupC and MLH1 c.1743delG) and three missense mutations (MLH1 c.293G > C, MLH1 c.954_955delinsTA and MSH2 c.2210G > A). Unclassified variants were evaluated as likely to be pathogenic by using the in-silico analyses. In addition, the MSH2 c.2210G > A alteration could be considered as a founder mutation for the Turkish population due to its identification in five different Lynch syndrome families and absence in control group. The present study adds new information about MMR gene mutation types and their role in Lynch syndrome. The medical records of 37 patients who were diagnosed and operated on between January 1996 and December 2002 were retrospectively reviewed. The patients' age, tumor size, type of surgery, histologic type, mitotic counts, presence of necrosis, Ki-67 proliferative index, National Institutes of Health 2001 consensus classification, immunohistochemical staining, and recurrence were examined to analyze factors affecting survival. Overall actuarial survival for all patients was 46%. When analyzed by type of resection, the complete resection group (R0 resection) had a mean overall survival of 48.2 +/- 6.18 months compared with the patients with incomplete resection (R1-R2) who survived a mean of 10.8 +/- 3.2 months (P = 0.00). Univariate analysis showed development of recurrence (P = 0.00), tumor size of 8 cm or greater (P = 0.05), Ki-67 proliferative index greater than 0.82 (P = 0.0448), desmin staining (P = 0.0076), age younger than 49 years (P = 0.0009), and incomplete resection (P = 0.00) to be significantly correlated with a poor survival. In multivariate analysis, desmin staining (P = 0.031), tumor size (P = 0.033), age (P = 0.01), recurrence (P = 0.038), and R0 resection (P = 0.02) were significant independent prognostic factors. The lesions can be misdiagnosed as hemorrhoids during clinical examination. We reviewed the morphological and clinical features of 14 anorectal melanomas, and discuss the treatment modalities of this entity. Fourteen patients who were diagnosed with anorectal malignant melanoma between 1997 and 2004 were evaluated with regard to age, sex, size, morphology, lymph node or distant metastasis, treatment modality and survival. Eight patients were female and six were male, and their mean age was 58 years. The size of melanoma ranged from 3 cm to 8 cm. Pathological evaluation revealed epithelioid and spindle cell type tumor in seven and two patients, respectively, whereas, in the remaining seven patients, the tumor was composed of both types. Pigmentation was apparent in all tumors. There was lymph node metastasis in 11 patients and distant metastasis in all patients. Eleven patients underwent abdominoperineal resection and three were treated by local excision. Mean survival was 8.7 months. Prognosis of anorectal melanoma remains poor. The efficacy of bio-absorbable oxidized regenerated cellulose in preventing post-operative pericardial adhesions was evaluated in the present study. Forty New Zealand white rabbits were divided into four groups of 10. In all rabbits an area of pericardium (2 x 2 cm) was excised. The wound was left open in groups I and 2 but replaced with bio-absorbable oxidized regenerated cellulose in groups 3 and 4. Rabbits in groups 1 and 3 were killed 3 weeks after surgery and those in groups 2 and 4 were killed at 6 weeks. Groups 1 and 2 showed more severe pericardial adhesions, more fibrous reaction and increased visibility of coronary vessels than groups 3 and 4, although there was no difference in inflammation. Light microscopy showed a mesothelium-like cell layer in groups 3 and 4. ApN acts via two receptors, namely adiponectin receptor-1 (Adipo-R1) and adiponectin receptor-2 (Adipo-R2). Studies have shown the presence of Adipo-R1 and Adipo-R2 expression immunohistochemically in human colorectal cancers (CRCs). However, only a few studies exist which investigated effects of adiponectin receptor expression on CRC characteristics. Objectives: In the present study, we aimed to explore Adipo-R1/-R2 expression in human colorectal cancers and any association with clinicopathological characteristics and survival. Materials and Methods: The study enrolled 58 colorectal cancer patients with tumor resection and a control group of 30 subjects with normal colon mucosa. Results: Positivity for Adipo-R1/-R2 expression was significantly more common in the control group in comparison to the patient group (both p<0.001). There was no significant association between Adipo-R1/-R2 expression and clinicopathological characteristics including age, sex tumor location, pTNM stage, Duke's stage, metastasis, histological differentiation, perineural invasion, venous invasion sex, lymphatic invasion, cancer-related mortality, tumor size and recurrence. AdipoR1/-R2 positivity was also not significantly linked to progression-free or overall survival [p values (0.871, 0.758) and (0.274, 0.232), respectively]. Conclusions: Although significantly reduced Adipo-R1/-R2 expression was found in colorectal cancer patients, it had no influence on survival.Item Bazal hücreli karsinamaların histopatolojik özellikleri (1989 yılında tam konan 75 olgunun retrospektif incelenmesi)(Uludağ Üniversitesi, 1991) Gücin, Zuhal; Yerci, Ömer; Erol, Oktan; Tolunay, Şahsine; Özuysal, Sema; Uludağ Üniversitesi/Tıp Fakültesi/Patoloji Anabilim Dalı.Bazal hücreli karsinoma derinin en yaygın tümörüdür. Klinik seyri iyidir. Fakat rekürrens potansiyelleri yüksektir. Klinik ve histolojik olarak değişik formları vardır. Agresif tümörler belirli histopatolojik özellikler gösterir. Bu çalışmada 75 bazal hücreli karsinoma olgusu gözden geçirildi. Yaş, cinsiyet lokalizasyon ve histolojik türleri gibi özellikleri saptandı. Agresif formlar ve bunların histopatolojik değişiklikleri karşılaştırıldı.Item Beş aylık kız çocuğunda bilateral yerleşimli adrenal korteks tümörü(Uludağ Üniversitesi, 1990) Yerci, Ömer; Tolunay, Şahsine; Gücin, Zuhal; Avşar, Talat; Özuysal, Sema; İnce, İbrahim; Uludağ Üniversitesi/Tıp Fakültesi/Patoloji Anabilim Dalı.Virilizan adrenal korteks tümörleri çocuklarda nadirdir. Heme'radar bu tümörlerin benign ve malign şekillerin ayrımı çok zorsa da çoğunluğu malign potansiyele sahiptir. Bu tümörler bütün karsinoma olgularının % 0,2'sini ve çocukluktaki adrenal tümörlerin % 6'sını oluştururlar. Olgumuz beş aylık kız çocuğu olup kliniğe başvurduğunda virilismus saptanmıştı. Gönderilen tümöral oluşumların histopatolojik incelemesinde, her iki adrenal tümörün benzer yapıda ve kötü diferansiye tümörler olduklan saptandı. Tümöral dokularda hücre çekirdekleri ileri derecede hiperkromazi, pleomorfizm göstermekte olup bazıları dev hücre şeklinde ve bizare görünümde idi. Sitoplazmaları bol ve granüler yapıda olup çok az mitoz gözlendi. Tümöral dokuda kapsüler invazyon mevcuttur. Kaynakların ışığında bu tümörün apple mikroskobik incelemeleri sonunda olgunun malign potansiyele sahip, fonksiyone bir tümör olduğu kanısına varıldı.Item Böbrekte leiomyosarkoma(Uludağ Üniversitesi, 1989) Yerci, Ömer; Erol, Oktan; Gücin, Zuhal; Tolunay, ŞahsineBöbrek sarkomları nadir görülen ve tüm malign böbrek tümörlerinin % 1-3 ünü oluşturan tümörlerdir. Leiomyosarkomlar ise böbrek sarkomları arasında en fazla görülenleridir. Tümörler genellikle 40 yaş üzerindeki kadınlarda bulunur. En sık rastlanan şikayetler, üst kadran ağrısı, üst kadranda ele gelen kitle, hematüri, kilo kaybı ve gastrointestinal yakınmalardır. Böbrek leiomyosarkomaları malignite dereceleri yüksek tümörlerdendir. Hastalarda tedaviye rağmen prognoz iyi değildir. There was no history of gastrointestinal bleeding, and endoscopic examinations of the gastrointestinal system were normal. Fine needle aspiration cytology from the thyroid nodule was suspicious. After blood transfusion, total thyroidectomy was performed. Postoperative histopathological examination of the specimen revealed hemangiosarcoma of the thyroid. After establishment of the diagnosis chemotherapy was started. But hemoglobin values decreased again and hepatosplenomegaly developed at the second month of surgery. Bone marrow aspiration cytology which was performed demonstrated the same tumoral cells infiltrating bone marrow. The patient died at 12th week after surgery. Thyroid hemangiosarcoma can metastasize to the bone marrow and anemia may be an indicator of the advanced disease. Methods: Duration of disease, HCV-RNA, liver steatosis, and the hepatitis activity index (HAI) were correlated with serum ALT in 36 patients with HCV. ALT values were also investigated in 16 control subjects without any liver diseases. Results: In bivariate analyses, ALT levels correlated with duration of HCV infection (P < 0.01), HCV-RNA (P < 0.05), and the HAI (P < 0.01). Among the components of the HAI, ALT concentrations were significantly associated with periportal bridging/necrosis (P < 0.01) and fibrosis (P < 0.05). In multivariate analysis, periportal bridging/necrosis (β = 0.508; P < 0.01), duration of HCV infection (β = 0.413; P < 0.01), and HCV-RNA (β = 0.253; P < 0.05) were independently associated with ALT activity. The normal ALT activity for men and women was < 23 IU/L and < 22 IU/L, respectively. Conclusion: In patients with HCV, alterations in the liver tissue as reflected by ALT elevation are mainly associated with periportal bridging/necrosis, viral load and duration of disease. Nevertheless, recurrence can unexpectedly occur in some eCC cases after surgical resection. The aims of the present study were to evaluate the relation of dysregulated MACC1, c-MET, and NM23-H1 expression with the histopathological features of tumors in recurrence formation in eCC cases. A total of 100 sporadic eCC patients without poor prognosis factors were evaluated in this study. The relationship between the altered expression of MACC1, c-MET, and NM23-H1 and pathological microenvironmental features, including the presence of tumor budding and desmoplasia, were assessed. The primary outcomes, including 5-year overall survival (OS) and disease-free survival (DFS), were also measured. Compared with nonrecurrent patients, the expression level of MACC1 was 8.27-fold higher, and NM23-H1 was 11.36-fold lower in patients with recurrence during the 5-year follow-up (p = 0.0345 and p=0.0301, respectively). In addition, the coexistence of high MACC1 and low NM23-H1 expression and tumor budding was associated with short OS (p < 0.001). Methods: Between January 2004 and October 2005, 92 patients with non-small cell lung cancer (NSCLC) who underwent cervical video mediastinoscopy were included in this prospective study. Tissue biopsy samples and FNAs were obtained from the same lymph node and were sent for a pathologic examination. Tissue samples and FNAs were stained with Hematoxilene-Eosine. All specimens were evaluated by two histopathologists. Results: Using tissue biopsy samples, no metastasis was found in 54 patients, while 38 patients were metastatic. Forty-eight patients had no metastasis, while 44 patients were metastatic using FNA samples. The difference was statistically non-significant (p>0.05). The false negativity of the FNA and tissue biopsy were 5.4% and 12%, respectively. Subsequently, 43 patients with negative FNA and tissue biopsy underwent lung resection with thoracotomy and sistematic lymph node dissection. Following lymph node dissection, lymph node metastases were found in two cases (4.6%). As a result, false negativity of mediastinoscopy was reduced to 4.6% when two techniques were combined. There was no complication related to the technique. Material and Methods: Forty female Wistar-Albino rats divided into four groups based on the drugs administered during the experimental laparotomy and caecal abrasion : the control group - no drug administered, the intra-peritoneal tissue plasminogen activator (TPA) group, the subcutaneous fondaparinux sodium (FS) group and the intra-peritoneal activated drotrecogin alfa (ADA) group. After two weeks, intra-peritoneal adhesions were macroscopically and micro-scopically scored. Results : The macroscopic scores of the three groups were similar but all lower than the control group (p = 0.002). Inflammation (p = 0.023) and fibrosis (p = 0.019) scores were lower in just the ADA group when compared to the control group. Conclusions : All three agents were effective in preventing adhesions when compared to the control group. Cytokeratin (CK), an intermediate sized filament detected primarily in epithelial Cells. Most commonly used CKs in immunohistochemical studies are CK7, CK20 and these molecules are used as a markers to help identify the site of origin of metastatic carcinomas. The aim of this study to evaluate the expression pattern of CK7 and CK20 molecules in primary adenocarcinoma of the stomach and their relation between clinicopathologic prognostic factors. Material and method: Gastric adenocarcinoma specimens were obtained from 103 patients who underwent total I subtotal gastrectomy with extended lymphadenectomy between December 2011-December 2013. Results: The 103 gastric adenocarcinomas included 80 (77.60%) cases with CK7 expression and 56 (54.30%) cases with CK20 expression, and were classified into 4 (3.80%) cases with CK7-/CK20 - pattern, 43 (41.70%) cases with CK7+/CK20 - pattern, 19 (18.40%) cases with CK7-/CK20+ pattern and 37 (36.10%) cases with CK7+/CK20+ pattern. CK expression patterns was not associated with additional variables which were age, gender, tumor location, size, depht of invasion, lymphatic and perineural invasion, lymph. node and distal organ involvement, existence of H. pylori and intestinal metaplasia, his type according to Lauren classification system and p53 expression. Conclusion: The CK expression patterns cannot helpful in determining prognostic factors for gastric adenocarcinoma.
