We report herein the case of a 41-year-old woman who developed abdominal actinomycosis involving the left colon and the surrounding retroperitoneum. The patient, who had an intrauterine contraceptive device (IUCD) in situ, presented with left bank pain and the signs and symptoms of an intraabdominal mass, which was initially diagnosed as a neoplasm originating from the left colon or the retroperitoneal space. Thus, a left colectomy was performed, but the histopathological diagnosis revealed abdominal actinomycosis. Methodology: At the Surgical Department of the Medical Faculty of Uludag University between 19862000 (15 years), 680 cases with colorectal. cancer diagnosis were operated on. One hundred and thirty-six (20%) of these cases who were under the age of 45 were retrospectively evaluated and compared with other patients. Results: The family history of ten of those patients was positive. The most common referring symptoms were rectal bleeding (61.7%), and weight loss (29.4%). Tumor was localized at right colon in 19%, at left colon in 6.6%, at the sigmoid and the rectum in 74%. Nine of those cases (6.6%) were operated emergently, the rest (93.3%) were operated electively. One hundred and eighteen of the cases (86.7%) were resectable and 61 cases (44.8%) were showing distant metastases during the operations. Conclusions: As a conclusion, in Turkey, young age group colorectal cancers are 4-5 times more common than in Western communities. Because of the high probability of colorectal cancers, considering the possibility of colorectal cancer and taking a careful family history will help to catch the case at an earlier stage and will improve the chance of cure.Item Effect of CO2 pneumoperitoneum on bacteremia in experimental peritonitis(Karger, 1996) Özgüç, Halil; Yılmazlar, Tuncay; Zorluoǧlu, Abdullah; Gedikoǧlu, Suna; Kaya, Ekrem; Uludağ Üniversitesi/Tıp Fakültesi/Cerrahi Anabilim Dalı.; Uludağ Üniversitesi/Tıp Fakültesi/Klinik Mikrobiyoloji Anabilim Dalı.Bacteria transfer to the blood from the peritoneum is thought to be augmented when the diaphragmatic stomata are activated by an increased intra-abdominal pressure. Therefore, it may be expected that the increase in intra-abdominal pressure during laparoscopic surgery can augment the absorption of bacteria from the peritoneum to the blood. Mutations in the adenomatous polyposis coli gene are primarily responsible for the development of this disease. This study was designed to investigation of adenomatous polyposis coli (APC) gene mutations in members of familial adenomatous polyposis family to identify individuals at risk of the disease. METHODS: We examined one patient with familial adenomatous polyposis and 21 family members including one affected person from familial adenomatous polyposis and 20 nonsymptomatic persons. We studied E, D, F, and G segments of exon 15 of the adenomatous polyposis coli gene by heteroduplex analysis. RESULTS: We used silver staining method for staining. We found a mutation for five persons at segment F of exon 15 of the adenomatous polyposis coli gene. Two of them were affected by colorectal cancer, one of whom was the proband, and the other three were non-symptomatic family members. The pathogenetic mutation was a T deletion at codon 1172, causing a frameshift in the adenomatous polyposis coli gene, as a result of the sequencing analysis of these cases. CONCLUSIONS: Investigation of adenomatous polyposis coli gene mutations is very important for the identification of genetic susceptibility to colorectal cancer and for the definition of tumor developing at an early stage. The aim of this study was to identify the altered gene expression patterns depending on the MSI status of early onset CRC and determine specific biomarkers that could provide novel therapeutic molecular targets in the Turkish population. Materials and methods: MSI markers (BAT25, BAT26, D2S123, D5S346, and D17S250) were investigated in tumors from 36 early onset sporadic CRC patients in whom gene expression profiles were analyzed previously. The relationship between the gene expression profiles depending on MSI status was evaluated. Results: A total of 15 tumors (16.66%) were identified as having MSI and 21 tumors (58.33%) were identified as having microsatellite stability (MSS). CK20 and MAP3K8 upregulation, observed in MSS tumors, was significantly associated with lymph node metastasis, recurrence, and/or distant metastasis and a short median survival (P < 0.05). REG1A upregulation is also correlated with recurrence and/or distant metastasis and a short median survival in patients with MSI tumors (P < 0.05). Conclusions: High expression levels of CK20 and MAP3K8 in MSS tumors and REG1A in MSI tumors correlated with a poor prognosis in CRC patients. Further studies and validations are required; these genes may provide novel therapeutic molecular targets for the development of anticancer drugs related to MSI status for early onset CRC treatment.Item Non-traumatic colorectal perforations(Int College of Surgeons, 1999) Yılmazlar, Tuncay; Toker, Süleyman; Zorluoǧlu, Abdullah; Uludağ Üniversitesi/Tıp Fakültesi/Genel Cerrahi Anabilim Dalı.; 6701800362; 6602323861; 6602076843Over a 6 year period, between January 1992 and December 1997, 30 patients with nontraumatic colorectal perforations undergoing laparotomy were reviewed. The aim of this study was to evaluate predictions on the prognosis using the Mannheim Peritonitis Index (MPI) and to evaluate the risk of this complication. The mean age of the patients was 56.4 years (range 16-88 years). The male:female ratio was 19:11. All patients showed signs of peritonitis and underwent emergency operations. In 50% (15) of the patients, tumor was the cause. According to the MPI scoring, there were 18 patients with an MPI score of 26 or less and 12 patients with an MPI score of 27 or more. For patients with a score less than 27 the mortality rate was zero (0/18) and for score greater than 26, 66.6% (8/12). Overall mortality was 26.6% (8/30). Of 15 patients with perforated colorectal cancers, four patients died (26.6%). The mortality rate for benign perforations was 26.6% (4/15) also. In conclusion, colorectal cancers are the most common cause of the non-traumatic colorectal perforations. Methodology: Thirty-eight patients with locally advanced rectal cancer were enrolled in the study. Ranges of total radiation doses were between 50.4 Gy and 61.2 Gy with a median of 59.4 Gy with fraction size of 1.8 Gy five times weekly. 5-fluorouracil was administered thrice weekly with the dose of 250300mg/m(2)/day concomitantly with radiation therapy. Results: Median follow-up time was 30 months. Administration of chemotherapy concomitant with radiotherapy (p=0.089), AJCC stage III (p=0.079), Duke's stage C (p=0.079), presence of lymph node involvement (p=0.079) and presence of local recurrence (p=0.066) appeared to be effecting distant metastasis although differences did not reach statistically significance. Mean overall survival was 46 months in patients without any distant metastasis (SD: 3.28; 95% CI: 39.46 and 52.31) while it was 35 months in patients with distant metastasis (SD: 5.71; 95% CI: 23.52 and 45.90, p = 0.0 16). Herniographic examination revealed 62 occult inguinal hernias in 51 cases, nine cases were normal. Fifty of these 51 cases were operated on. Surgery was postponed until the end of the league in one case. There was only one false positive examination. The herniographic and operative diagnoses corresponded well in the other 49 cases. There were three minor complications which have related to the needle sigmoid colon puncture, all of them were managed conservatively. There was no technical failure. These results indicate that herniography is a safe and valuable method to identify non palpable herniations causing groin pain of unknown origin in football players.
