Item 1998-2005 yılları arasında Türkiye'de 5 farklı bölgedeki üniversitelerin Adli Tıp Anabilim dallarında raporladırılan yaş tayini olgularının i̇rdelenmesi(Ortadoğu Yayınları, 2009-04) Baransel, Aysun; Büken, Bora; Tokdemir, Mehmet; Dülger, Hikmet Ergin; Erel, Özlem; Fedakar, Recep; Uludağ Üniversitesi/Tıp Fakültesi/Adli Tıp Anabilim Dalı.; AAH-6287-2021; 8725968900Adli tıpta yaş tayini, gerek canlıda gerekse ölüde kimlik tespitinde yaygın olarak kullanılmakta ve cezai ve hukuki sorumluluğun belirlenmesinde çok önemli bir yer tutmaktadır. Yaş tayininde, günlük uygulamada sıklıkla kullanılan yöntem, fiziksel muayene ile birlikte, kemik gelişiminin radyolojik olarak görüntülenmesi ve hazırlanmış atlaslara uyarlanarak kemik yaşının tespiti esasına dayanmaktadır. Çalışmamızda Türkiye’de yaş tayininde yaygın olarak kullanılan ve 5 ayrı coğrafi bölgede bulunan Gaziantep, Fırat, Adnan Menderes, Uludağ ve Düzce Üniversitelerinin Adli Tıp Anabilim Dalı Başkanlıkları tarafından da kıstas alınan “Gök Atlası”na göre yaş verilerinin değerlendirilerek irdelenmesi amaçlanmıştır. Gereç ve Yöntemler: Bu çalışmada, 1998-2005 tarihleri arasında çeşitli nedenlerle mahkeme tarafından yaş tayini istenen 275’i erkek, 312’si kadın toplam 587 olgu değerlendirmeye alındı. Radyolojik inceleme ile çeşitli yaş gruplandırılmaları yapılarak kronolojik yaş ve kemik yaşı arasındaki uyum veya farklılık olup olmadığı araştırıldı. Bulgular: 0-12 yaş grubuna giren 39 olgunun 31 (%79.5)’inin kemik yaşı kronolojik yaşından büyük, 6 (%15.4)’sının kemik yaşı kronolojik yaşı ile uyumlu ve 2 (%5.1)’sinin kemik yaşı kronolojik yaşından küçük bulundu. 13-22 yaş grubunda kronolojik yaşa göre ileri düzeyde kemik gelişimi olguların %79.8’inde, kronolojik yaşa göre geri kalmış kemik gelişimi %9.6’sında ve kemik ve kronolojik yaş uyumu %10.6’sında tespit edildi. Sonuç: Bu çalışmada, puberte dönemindeki fiziksel gelişmenin kemik gelişimi üzerindeki önemli etkisi ve kesin yaş tayini yapılamayan puberte sonrası ileri yaşlarda yeni ve gelişmiş tekniklerin rutin uygulamada kullanılması gerektiği vurgulandı. Method: In this retrospective observational study, the records of all tuberculosis cases (1630 patients) treated in the last 30 years (1985-2014) at the Bursa Nilufer Tuberculosis Dispensary were examined and statistically analyzed. Results: Males comprised 65.2% of the patients. The ages of the patients ranged from 1 to 87 years, and the mean age was 37.4 (95% CI: 36.6-38.2). Among the cases, 86.7% were new infections and 74.1% were pulmonary tuberculosis. In the last decade, the education level, the percentage of patients who had received a BCG vaccination, the proportion of women and active employees among them increased (p<0.05), while it decreased among men (p<0.05). Clinical symptoms accompanying TB such as weakness, anorexia, weight loss, and cough, decreased to a statistically significant degree (p<0.05). In the last decade, the mortality rate was 3.6% and increased compared with previous decades (p<0.05). Mortality was higher among patients who were elderly, male, did not have a BCG scar or had a chronic disease (p<0.05). Conclusion: This study adds information about the change of TB epidemics in Turkey in the last 30 years. Translocations of this gene are well defined, and their impact on the patient's prognosis is well known, but deletions of the same region are rare, and little is known about their prognostic significance and the significance of their accompanying translocations. Here we present a case of childhood ALL with a deletion of the 5' region of the MILL gene detected by fluorescence in situ hybridization (FISH) analysis, This result also confirmed the sensitivity and efficiency of FISH analysis.Item A global multicenter study on reference values: 2. Methods: As described in the first part of this paper, RVs of 50 major serum analytes from 13,396 healthy individuals living in 12 countries were obtained. Analyzed in this study were 23 clinical chemistry analytes and 8 analytes measured by immunoturbidimetry. Multiple regression analysis was performed for each gender, country by country, analyte by analyte, by setting four major SVs (age, BMI, and levels of drinking and smoking) as a fixed set of explanatory variables. For analytes with skewed distributions, log-transformation was applied. The association of each source of variation with RVs was expressed as the partial correlation coefficient (rp). Results: Obvious gender and age-related changes in the RVs were observed in many analytes, almost consistently between countries. Compilation of age-related variations of RVs after adjusting for between-country differences revealed peculiar patterns specific to each analyte. Judged from the rp, BMI related changes were observed for many nutritional and inflammatory markers in almost all countries. However, the slope of linear regression of BMI vs. RV differed greatly among countries for some analytes. Alcohol and smoking-related changes were observed less conspicuously in a limited number of analytes. Conclusion: The features of sex, age, alcohol, and smoking-related changes in RVs of the analytes were largely comparable worldwide. Both Achilles tendons of 23 female Wistar-Albino rats weighing between 350 and 450 grams were cut and repaired. On the right side, Interceed absorbable adhesion barriers were wrapped around the repaired tendon (group I). On the left, the same procedures were applied except for the Interceed wrapping and these were grouped as control (group II). Animals were sacrificed at postoperative day 28 and macroscopic and histological examination was performed. All the animals survived and no tendon rupture was observed. No wound dehiscence, wound infection or exposure of repaired tendons occurred. Macroscopically, there were three (13.1%) tendons without adhesion formation and 20 (86.9%) tendons with inferior adhesion formation in group I; on the other hand, there were 16 tendons (69.5%) with medium grade adhesion formation and seven tendons (30.5%) with severe peritendinous adhesion formation in group II (control group) (p < 0.05). Histologically, adhesion formation was absent in 11 tendons (47.8%) and slight in 12 tendons (52.2%) in group I; while in group II, it was slight in two (8.6%), moderate in 15 (65.2%) and severe in six tendons (26.2%) (p < 0.05). Sixteen (69.5%) of 23 tendons in group I and 11 (47.8%) of 23 tendons in group II showed no inflammatory reaction (p < 0.05). Nineteen (82.6%) tendons in group I and only one tendon in group II showed excellent to good tendon healing (p=0.00). According to our results, we feel that Interceed may have an intraoperative role to play in the reduction of adhesions after surgical tendon repair. This study suggests that absorbable oxidised regenerated cellulose merits further evaluation as a potential treatment to inhibit the formation of peritendinous adhesions. The long-term efficacy of cytotoxic T-lymphocyte-associated antigen 4-immunoglobulin (abatacept) as targeted therapy for its immune dysregulatory features remains to be established. OBJECTIVE: To determine the clinical and immunologic features of LRBA deficiency and long-term efficacy of abatacept treatment in controlling the different disease manifestations. METHODS: Twenty-two LRBA-deficient patients were recruited from different immunology centers and followed prospectively. Eighteen patients on abatacept were evaluated every 3 months for long-term clinical and immunologic responses. LRBA expression, lymphocyte subpopulations, and circulating T follicular helper cells were determined by flow cytometry. RESULTS: The mean age of the patients was 13.4 +/- 7.9 years, and the follow-up period was 3.4 +/- 2.3 years. Recurrent infections (n = 19 [86.4%]), immune dysregulation (n = 18 [81.8%]), and lymphoproliferation (n = 16 [72.7%]) were common clinical features. The long-term benefits of abatacept in 16 patients were demonstrated by complete control of lymphoproliferation and chronic diarrhea followed by immune dysregulation, most notably autoimmune cytopenias. Weekly or every other week administration of abatacept gave better disease control compared with every 4 weeks. There were no serious side effects related to the abatacept therapy. Circulating T follicular helper cell frequencies were found to be a reliable biomarker of disease activity, which decreased on abatacept therapy in most subjects. However, high circulating T follicular helper cell frequencies persisted in 2 patients who had a more severe disease phenotype that was relatively resistant to abatacept therapy. Diffuse, mild cellular infiltration of neutrophils, lymphocytes and plasma cells was observed in the lamina propria of the intestine. In addition, bacterial colonies were seen in the intestinal content, wall of intestine and liver. Streptococcus equi subsp. zooepidemicus was isolated in pure culture from all organs. For this reason, we aim to examine the abstraction process of the limit knowledge of two volunteer participant students using the RBC+C abstraction model. With this aim, the students' abstraction processes were examined over recognition, building-with, construction, and consolidation. In the process, three problems that enable students to use their pre-knowledge were designed by the researchers to reveal the students' mathematical thinking levels, abstraction processes, and finally to improve a new structure. These problems were used in the application, and students' dialogues were video-recorded. The semi-constructed interview and observation data-collection methods were used in this process, and one of the researchers participated in the study as a participant observer. The data was analyzed and interpreted by transcribing the video recordings into written text and grouping them into the cognitive actions of the RBC+C model. In conclusion, these students were indicated to have managed recognizing and building-with their pre-knowledge on sequence, function, and infinity, and thus constructing knowledge on limits.Item Acaricidal, repellent and oviposition deterrent activities of Datura stramonium L. against adult Tetranychus urticae (Koch)(Springer Heidelberg, 2010-05) Çobanoğlu, Sultan; Kumral, Nabi Alper; Yalçın, Cem; Uludağ Üniversitesi/Ziraat Fakültesi/Bitki Koruma Bölümü.; 0000-0001-9442-483X; A-1388-2019; 15846048400; 36062093300The ethanol extracts obtained from both leaf and seed in the Thorn apple (Datura stramonium L.) (Solanaceae) were investigated for acaricidal, repellent and oviposition deterrent properties against adult two-spotted spider mites (Tetranychus urticae Koch) (Acari: Tetranychidae) under laboratory conditions. Leaf and seed extracts, which were applied in 167,250 and 145,750 mg/l concentrations, respectively (using a Petri leaf disc-spray tower method), caused 98 and 25% mortality among spider mite adults after 48 h. The simple logistic regression analysis showed that the independent variable, an increase in the dose of leaf extract was associated with a significant increase in the death rate of T. urticae females, but an increase in the dosage of seed extracts did not have a significant effect. Using probit analysis and estimating the parameters with a confidence limit of 95%, we determined the LC50 values of leaf extract to be 70,593 mg/l. According to Pearson's chi(2) test, mites showed the strongest run off to bean leaf surfaces sprayed with both leaf and seed extracts (in sub-lethal doses: < 7,500 mg/l and < 25,000 mg/l, respectively) and moved towards surfaces that had not been sprayed with extracts. Furthermore, repeated measures ANOVA indicated a significant difference between the number of eggs laid on unsprayed bean leaves compared to bean leaves that were sprayed with leaf and seed extracts at sub-lethal doses, 2,500 and 25,000 mg/l concentrations, respectively. However, statistical power decreases dramatically in the presence of genetic heterogeneity. Methods The authors applied an empirical approach to test for linkage accounting for genetic heterogeneity by calculating the sum of positive per-family multipoint LOD scores (S) across all positions, and obtaining corresponding empirical p values (EmpP) through permutations. Results The statistical power of the approach was found to be consistently higher than the classical heterogeneity LOD by simulations. Among 21 first-cousin matings with a single affected child, for five families linked to a locus of interest and 16 families to other loci, S/EmpP achieved a power of 40% versus 28% for heterogeneity LOD at an alpha level of 0.001. The mean size of peak linkage regions was markedly higher for true loci than false positive regions. The S/EmpP approach was applied to a sample of 17 consanguineous families with Mendelian susceptibility to mycobacterial disease, leading to the identification of two mutations in IL12RB1 and TYK2 from the largest of six linkage regions at p<10(-3). Conclusions The S/EmpP approach is a flexible and powerful approach that can be applied to linkage analysis of families with suspected Mendelian disorders.Item Accumulation profiles of PrPSc in hemal nodes of naturally and experimentally scrapie-infected sheep(Bmc, 2013-04-19) Dassanayake, Rohana P.; Truscott, Thomas C.; Zhuang, Dongyue; Schneider, David A.; O'Rourke, Katherine I.; Özyiǧit, Musa Özgür; Uludağ Üniversitesi/Veteriner Fakültesi/Patoloji Anabilim Dalı.; AAR-6478-2021; AAH-2873-2021; 6507338060Background: In classical scrapie, the disease-associated abnormal isoform (PrPSc) of normal prion protein accumulates principally in the nervous system and lymphoid tissues of small ruminants. Lymph nodes traffic leukocytes via lymphatic and blood vasculatures but hemal nodes lack lymphatic vessels and thus traffic leukocytes only via the blood. Although PrPSc accumulation profiles are well-characterized in ovine lymphoid tissues, there is limited information on such profiles in hemal nodes. Therefore, the objective of this study was to compare the follicular accumulation of PrPSc within hemal nodes and lymph nodes by prion epitope mapping and western blot studies. Results: Our studies found that PrPSc accumulation in 82% of animals' abdominal hemal nodes when PrPSc is detected in both mesenteric and retropharyngeal lymph nodes collected from preclinical and clinical, naturally and experimentally (blood transfusion) scrapie-infected sheep representing all three major scrapie-susceptible Prnp genotypes. Abdominal hemal nodes and retropharyngeal lymph nodes were then used to analyze immune cell phenotypes and PrPSc epitope mapping by immunohistochemistry and PrPSc banding patterns by western blot. Similar patterns of PrPSc accumulation were detected within the secondary follicles of hemal nodes and retropharyngeal lymph nodes, where cellular labeling was mostly associated with macrophages and follicular dendritic cells. The pattern of PrPSc accumulation within hemal nodes and retropharyngeal lymph nodes also did not differ with respect to epitope mapping with seven mAbs (N-terminus, n = 4; globular domain, n = 2; C-terminus, n = 1) in all three Prnp genotypes. Western blot analysis of hemal node and retropharyngeal lymph node homogenates revealed identical three banding patterns of proteinase K resistant PrPSc. Conclusion: Despite the anatomical difference in leukocyte trafficking between lymph nodes and hemal nodes, the follicles of hemal nodes appear to process PrPSc similarly to lymph nodes.Item Acetabular fractures treated surgically: Which of the parameters affect prognosis(Türk Travma ve Acil Cerrahi Derneği, 2019-07-02) Bilekdemir, Utku; Civan, Osman; Özdemir, Hakan; Cavit, Ali; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Ortopedi ve Travmatoloji Anabilim Dalı.; AAK-9223-2020; 55507113700BACKGROUND: This study aims to evaluate the surgical approaches, complications, clinical and radiological findings in acetabular fractures treated with surgical methods and to determine the parameters affecting prognosis.METHODS: Out of 144 patients undergone surgical treatment with the diagnosis of displaced acetabular fractures between 1994 and 2014, a total of 103 patients with 75 male and 28 female with a mean age of 36.3 years (range 19-67 years) whom clinical and radiologic follow-ups (mean: 34 months, range 2-8 years) were performed at least for two years were included in this study.RESULTS: Clinically excellent to good outcomes were obtained in 64% of the patients and moderate to poor outcomes were recorded in 36% of the patients, while radiologically excellent to good outcomes were achieved in 57.3% of the patients and moderate to poor outcomes were recorded in 42.7% of the patients. Presence of one of the complications, creating mechanical block (chi-square p<0.001), complex fractures (chi-square p=0.023), increased duration between trauma and operation (p=0.039), operational time taking longer than six hours (chi-square p<0.001), more than 3 mm intra-articular step (Fisher's p=0.033), avascular necrosis (p<0.001), arthritis (p=0.006) and heterotopic ossification (p=0.007) worsened the clinical outcomes (chi-square p<0.001). The age of the patient was not effective on the clinical outcome (p=0.461).CONCLUSION: It was found that three major parameters affecting the prognosis of acetabular fractures are as follows: type of fracture, operational time and reduction quality. The duration between trauma and operation indirectly affects the outcomes. Although the amount of research in the field of pathogenic fungi has also increased, there is still a need for studies on fungal virulence. In this investigation, we focused on testing bloodstream-identified Candida albicans in experimental in vitro virulence assays. The proteinase enzyme activities of 30 C. albicans strains isolated from the bloodstream and sterile body fluids were investigated (15 colonizations, 15 infections). The spectrophotometric and plate methods were used to determine proteinase activity. The correlations between the 2 methods were compared. In terms of emergency service, the elderly constitute a special group. We aimed to study the demographic, clinical, and imaging findings in patients who are 65 years and older presented to Emergency Department. Materials and Method: By obtaining the data of the patients with stroke diagnosis, who are 65 and older and who applied to Emergency Department between 01.01.2011 and 29.02.2012, demographic, clinical, and imaging findings were investigated and then compared among 65-74, 75-84 ve >85 age groups in this study. Results: According to the study results, among the age groups of 65-74, 75-84 and >= 85, significant difference was not found out between gender, complaints for application, duration of complaints, and comorbidities. Similarly, among physical examination findings such as consciousness, eye movements, visual fields, facial nerve examinations, lower and upper extremity motor functions, ataxia, negligence, aphasia, dysarthria, and NIHSS scores, statistically significant difference was not established. Besides, no significant difference was found among diagnoses, Cranial Computed Tomography and magnetic resonance imaging findings. Following the statistical analyses, the parameters revealing significant differences among age groups were conscious status, sense examinations, infarct in Cranial Computed Tomography, and bleeding sides. The lesion had grown fast for 2 months and then had remained stable in size. Excisional biopsy was performed. Histopathological examination of an excisional biopsy specimen pointed to proliferative vessels lined by increased endothelial cells without nuclear atypism. The nodular mass evaluated as a capillary hemangioma.
