Real-life data on the use of pirfenidone 200 mg tablets are limited. This study aimed to investigate the efficacy and safety of pirfenidone 200 mg tablets for the treatment of IPF in a real-life setting. Materials and methods: A retrospective, multicenter study conducted in four university hospitals in Turkey between January 2017 and January 2019. Clinical records of patients diagnosed with mild to moderate IPF and receiving pirfenidone (200 mg tablets, total 2400 mg/day) were reviewed retrospectively and consecutively. Pulmonary function measurements including forced vital capacity (FVC%) and diffusing capacity of the lungs for carbon monoxide (DLCO%) were analyzed at baseline and after 6-month of pirfenidone treatment. Descriptive statistics were expressed as mean, standard error or median (minimum-maximum), number and percentage, where appropriate. Results: The study included 82 patients, of whom 87.8% were males (mean age, 66 years). After 6-month of treatment, 7 patients discontinued the treatment. Of the remaining 75 patients, 71 (94.6%) remained stable, 4 (5.4%) had progressive disease as evident by a decline in the FVC% of at least 10% while on treatment, and 45 (61.3%) had improved cough. At least one adverse event (AE) associated with the treatment was observed in 28 (37.3%) patients. He was in a habit of cleaning secretions of upper airway with a wooden stick covered with cotton on the tip soaked in olive oil, via tracheostomy. After applying topical aerolized lidocaine spray through the tracheostomy stoma a flexible video-brochoscopy was performed and a tree twig over 11 cm in length was removed. The patient's symptoms were resolved by a bronchoscopy. With experience and availability of accessories, the removal of the foreign body using flexible bronchoscope under local anesthesia can be performed safely and successfully. Understanding the characteristics of pain symptoms is crucial for a better clinical approach. Methods Patients who were diagnosed as having COVID-19 using reverse transcription-polymerase chain reaction were included in the study. Patients were asked to complete a 51-item questionnaire via a phone interview, which included questions on demographics, acute COVID-19 symptoms, the presence of pain symptoms, and their characteristics in the acute phase of COVID-19. Results A total of 222 out of 266 patients with COVID-19 participated in the study, yielding a response rate of 83.5%. A total of 159 patients reported at least one kind of pain syndrome with a prevalence of 71.6%. Myalgia was reported in 110 (49.6%) patients, headache in 109 (49.1%), neuropathic pain symptoms in 55 (24.8%), and polyarthralgia in 30 (13.5%) patients. A total of 66 patients reported only one type of pain, 46 reported two types, 42 reported three types, and five patients reported all four types of pain. Logistic regression analysis showed that there were significant associations between these pain syndromes and a strong association was found between neuropathic pain and headache. Conclusion Pain is a frequently observed symptom of mild-to-moderate COVID-19. There are significant relationships between pain syndromes in COVID-19, which may be due to a sequence of common etiologic factors. Significance This study described the main pain syndromes associated acute phase of mild-to-moderate COVID-19 and its associated features. Although the potential use of serum D-dimer levels has been assessed as a screening test for venous thromboembolism, its role in other disorders has not been as well defined. Little is known about the relationship between D-dimer levels and the clinical outcomes of patients with community-acquired pneumonia (CAP). The aim of this study was to investigate the prognostic value of plasma D-dimer levels in patients with CAP.Material and methods: This prospective study was conducted between January 2006 to September 2006 including 64 cases diagnosed as CAP, 45 cases as pulmonary emboli and 20 cases for control group. The demographic characteristics, physical examination findings and laboratory test results of cases were recorded. The severety of cases in pneumonia group were assessed with Pneumonia Severity Index (PSI). Among three groups levels of D-dimer in serum were measured with latex buttressed turbidimetric method for quantitative determination and compared.Results: Serum D-dimer levels were higher in patients with pulmonary embolism (748.3 +/- 769.6 mu g/L) when compared other two groups and it was higher in CAP (357.8 +/- 294.7 mu g/L) group when compared to controls (149.7 +/- 99.6 mu g/L) (p<0.05). A questionnaire was administered to get information on their socio-demographic data, presence of witnessed apnea during sleep, the prevalence of snoring, excessive daytime sleepiness and frequency of other symptoms and having any occupational accident.Results: The prevalence of habitual snoring, witnessed apnea, excessive daytime sleepiness was 5.4%, 22.4%, 47.1% respectively. Snoring (p<0.001) and witnessed apnea (p<0.001) prevalence was higher in overweight nurses. Mean values of the body mass index (p<0.001), neck circumflex (p=0.03), co-morbid disease (p<0.001) and the prevalence of witnessed apnea (p<0.001) among the nurses with habitual snoring were statistically higher than the nurses without habitual snoring. 51 nurses (12.4%) had been involved in an occupational accident. The prevalence of snoring (p=0.008) and mean Epworth Sleepiness Score (ESS) were higher than the group without accidents. The ESS was higher than 10 points in 63 (15.4%) of the interviews. This study aims to find a predictive scoring model to estimate the COVID-19 diagnosis.Materials and Methods: Patients who applied to the emergency pandemic clinic between April 2020 and March 2021 were enrolled in this retrospective study. At admission, demographic characteristics, symptoms, comorbid diseases, chest computed tomography (CT), and laboratory findings were all recorded. Development and validation datasets were created. The scoring system was performed using the coefficients of the odds ratios obtained from the multivariable logistic regression analysis.Results: Among 1187 patients admitted to the hospital, the median age was 58 years old (22-96), and 52.7% were male. In a multivariable analysis, typical radiological findings (OR= 8.47, CI= 5.48-13.10, p< 0.001) and dyspnea (OR= 2.85, CI= 1.71-4.74, p< 0.001) were found to be the two important risk factors for COVID-19 diagnosis, followed by myalgia (OR= 1.80, CI= 1.082.99, p= 0.023), cough (OR= 1.65, CI= 1.16-2.26, p= 0.006) and fatigue symptoms (OR= 1.57, CI= 1.06-2.30, p= 0.023). In our scoring system, dyspnea was scored as 2 points, cough as 1 point, fatigue as 1 point, myalgia as 1 point, and typical radiological findings were scored as 5 points. This scoring system had a sensitivity of 71% and a specificity of 76.3% for a cut-off value of >2, with a total score of 10 (p< 0.001).Conclusion: The predictive scoring system could accurately predict the diagnosis of COVID-19 infection, which gave clinicians a theoretical basis for devising immediate treatment options. We conducted a narrative literature review and performed a PubMed and Scopus search for studies published up to November 18, 2020, using the following medical subject headings terms: ["comparison," "comparisons" AND "severe acute respiratory syndrome coronavirus 2," "ncov," "2019 ncov," "covid 19," "sars coy 2," "coronavirus," "coy" AND ("influenzas," "influenza," "influenzae," "human influenza" OR "adenoviridae," "adenovirus," "adenoviridae infections")]. This narrative review aims to compare pneumonia caused by severe acute respiratory syndrome coronavirus-2, influenza, and adenovirus in terms of clinical, laboratory, and radiological characteristics. Considering the prevalence, incidence, and outcomes of IPF, it is important to ensure the availability and validity of biomarkers that can be used in determining the diagnosis and prognosis. In this study, we aimed to assess the relationship between serum periostin levels, pulmonary function tests, 6-minute walking test (6MWT) and quality of life in IPF patients.Material and methods: Our study was carried out with 40 IPF patients who were followed at Uludag University, Faculty of Medicine, Department of Pulmonary Medicine. Spirometer and carbon monoxide diffusion test (DLCO) for respiratory functions, 6MWT and periostin level measurement were performed on the patients, and results were recorded.Results: A total of 31 of patients were males and nine were females. The diagnosis of IPF was made according to the guidelines of ATS / ERS / JRS / ALAT with 25 (62.5%) patients by radiologically and 15 (37.5%) patients with histopathological and radiological features. The mean periostin level was 32.9 +/- 37.57 ng/ml. No significant correlation was found between St. George's Respiratory Questionnaire (SGRQ) score, FVC% predicted, DLCO adj% predicted values, 6MWT distance and periostin level which were measured simultaneously. A negative correlation was found between SGRQ score and DLCO adj% predicted value, 6MWT distance.Conclusion: It has been shown that there is no correlation between periostin levels and the results of the pulmonary function tests, when measured and performed at the same time. No studies are evaluating the relationship between 6MWT and SGRQ scores and periostin levels.
