Publication Demographics of the open-globe injuries in pediatric age group in northwest Turkey(Galenos Yayincilik, 2016-12-01) KIVANÇ, SERTAÇ ARGUN; Kıvanç, Sertaç Argün; Budak, Berna Akova; Özmen, Ahmet Tuncer; ÖZMEN, AHMET TUNCER; Yıldız, Meral; YILDIZ, MERAL; Çevik, Mediha Tok; Hamidi, Nagihan Amuk; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Oftalmoloji Anabilim Dalı.; 0000-0003-0995-5260; AAH-1885-2021; AAH-6518-2021; ABE-3033-2020Introduction: Our aim was to investigate the pattern of open globe injury and assess the effect of age on different parameters of open globe injury in pediatric patients.Materials and Methods: The medical records of the patients under 18 years old who had open-globe injury were retrospectively reviewed. Age, gender, the cause of the trauma, presence of intraocular foreign body and the extent of the ocular damage were recorded. Results: The study was comprised of 79 patients (23 female, 56 male) with a mean age of 7.7+/-4.4 (range: 2-18 years). Thirty-nine patients were injured with a blunt tip object, 27 with a sharp object and one has rupture with trauma. Thirty-five injuries occurred outside and 45 at home. The season in which most of the injuries took place was summer (32%). Seventy-five percent of the injuries were noted in 11 years and under. The mean age subject to blunt tip object injury, organic body injury and outdoor injury was significantly higher compared to that of with sharp object, inorganic body injury and indoor injury. Conclusions: The open globe-injury rates in Northwest Turkey is found to be higher in smaller age groups. Making the parents aware of the danger and consequences of the trauma and avoiding the easily accessible sharp objects may decrease the trauma rates.Publication The role of serial measurements of serum insulin-like growth factor 1 levels in the development of retinopathy of prematurity(Turkish Pediatrics Assoc, 2017-03-01) ÖZKAN, HİLAL; Dorum, Bayram Ali; Yılmaz, Cansu Canbolat; Köksal, Nilgün; YILDIZ, MERAL; Yıldız, Meral; Özmen, Ahmet Tuncer; ÖZMEN, AHMET TUNCER; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Çocuk Sağlığı ve Hastalıkları Anabilim Dalı.; 0000-0002-2823-8454; AAH-1885-2021; AAG-8393-2021; A-5375-2017Aim: To determine the role of serum insulin-like growth factor-1 levels in the development of retinopathy of prematurity, which is a major cause of childhood blindness worldwide.Material and Methods: We prospectively studied newborn infants born at a postmenstrual age of < 32 weeks and birth weights < 1 500 gr, between January 1st, 2015, and December 31st, 2015. A total of 40 infants were enrolled in the study. The retinal examination time was determined in accordance with the American Academy of Pediatrics recommendations for retinopathy of prematurity screening and follow-up. Retinopathy of prematurity was classified according to the international classification of retinopathy of prematurity. Serum Insulin like growth factor 1 levels were measured serially in blood samples on the 1st, 3rd, 7th, 21st, and 28th day. Results: Among the 40 infants, 11 (27.5%) constituted the retinopathy of prematurity group and 29 comprised the non-retinopathy of prematurity group. In the retinopathy of prematurity group, the mean gestational age and birth weight was significantly lower. The demographic features of the study cohort were similar. The duration of mechanical ventilation was significantly greater in the retinopathy of prematurity group compared with the non-retinopathy of prematurity group (p=0.036). In terms of neonatal morbidities such as respiratory distress syndrome, intraventricular hemorrhage, bronchopulmonary dysplasia, patent ductus arteriosus, and necrotizing enterocolitis, no differences were detected between the groups. The mean serum insulin-like growth factor-1 levels in retinopathy of prematurity group were significantly lower than those in the non-retinopathy of prematurity group at each time point (1st, 3rd, 7th, 21st, and 28th day of postnatal life) (p=0.001).Conclusions: This study demonstrated the low serum insulin-like growth factor-1 levels was associated with retinopathy of prematurity development.Publication The relationship between serum insulin-like growth factor-1 levels and retinopathy of prematurity(Galenos Yayıncılık, 2008-09-01) Çetinkaya, Merih; Köksal, Nilgün; Özkan, Hilal; Özmen, Ahmet T.; Çetinkaya, Merih; Köksal, Nilgün; ÖZKAN, HİLAL; ÖZMEN, AHMET TUNCER; Uludağ Üniversitesi/Tıp Fakültesi/Çocuk Sağlığı ve Hastalıkları Anabilim Dalı/Neonatoloji Bilim Dalı.; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.; JGS-7600-2023; AFP-9671-2022; JRA-9088-2023; JIQ-6672-2023Aim: The aim of this study was to investigate the association between serum insulin like growth factor I (IGF-I) and retinopathy of prematurity (ROP).Materials and Method: We prospectively studied the newborn infants born at a postmenstrual age of <34 weeks between November 2006 and May 2007. ROP was classified according to the international classification of ROP. Total 33 infants were enrolled in the study. Serum IGF-I levels were studied after birth and weekly until discharge from the hospital. Results: Among the 33 infants, 36% had retinopathy of prematurity. Lower birth weight or younger gestational age were associated with the higher ROP development rate. The difference in the mean serum IGF-1 levels in the ROP group was found to be statistical significant when compared with the non-ROP group (p=0.03). There was no statistically significant relationship between the serum IGF-I levels which measured weekly and the development of ROP.Conclusion: In conclusion, low mean IGF-I levels in preterm infants are associated with the development of ROP.Publication Pyelonephritis: A retrospective analysis of 190 cases(Aves, 2010-04-01) Karakeçili, Faruk; Karadağ, Sanem; Erbay, Fatma; Yılmaz, Emel; YILMAZ, EMEL; Akalın, Halis; AKALIN, EMİN HALİS; Özmen, Ahmet; ÖZMEN, AHMET TUNCER; Özakın, Cüneyt; ÖZAKIN, CÜNEYT; Helvacı, Safiye; Mistik, Reşit; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Enfeksiyon Hastalıkları ve Mikrobiyoloji Anabilim Dalı.; 0000-0002-3567-4262; 0000-0002-3894-1231; 0000-0001-7530-1279; 0000-0003-2267-2206; AAG-8392-2021; AAD-1102-2022; AAU-8952-2020Objective: In this study, we aimed to present the epidemiological, clinical and microbiological features of community acquired/onset pyelonephritis cases followed in our clinic. Methods: One hundred ninety adult patients with pyelonephritis followed in our clinic between 1989-2008 were analysed retrospectively. Results: Of the patients, 75 were male (39.5%) and 115 were female (60.5%) and mean age was 55.5 years (18-93). In total, 55 of the patients had had urinary tract infections within the last year. Complicated urinary tract infection was diagnosed in 137 patients. Escherichia coli was isolated in the urine culture of 119 (62.6%) patients and extended spectrum-beta-lactamase production was positive in 20% of the isolates. Blood culture was also positive in 44 (23.1%) patients. Nine patients (4.7%) died. Conclusions: The local epidemiology and antibiotic susceptibility results are very important for appropriate empiric antibiotic treatment of pyelonephritis. The upper urinary system ultrasonography should be carried out especially in female patients with pyelonephritis to diagnose complicated urinary tract infections.Publication Anatomical and the refractive outcomes in the eyes undergoing laser photocoagulation for retinopathy of prematurity(Galenos Yayincilik, 2009-09-01) YILDIZ, MERAL; ÖZMEN, AHMET TUNCER; YALÇINBAYIR, ÖZGÜR; Yalçınbayır, Özgür; Ayvaz, Ayse Tor; Kaderli, Berkant; Avcı, Remzi; Gelişken, Öner; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.; 0000-0002-7311-5277; 0000-0001-9581-4856; AAH-6625-2021; IYJ-9408-2023; JAC-9459-2023Aim: This study aimed to evaluate the anatomical and the refractive outcomes in cases that underwent laser photocoagulation (LP) for retinopathy of prematurity (ROP).Materials and Method: Medical records of 38 cases that underwent LP for ROP in Uludag University Department of Ophthalmology, between 2004 and 2008, were reviewed. LP was performed in 73 eyes. Stages of ROP before and after LP were recorded. Additionally gestational age and birth weight, complications, sciascopic refractions were assessed and visual acuities were evaluated with "Teller Acuity Cards".Results: Gestational age ranged from 26 to 34 weeks, (mean 29+/-2 weeks), birth weight ranged from 850 g to 1950 g (mean 1262+/-304 g). Among all cases in the study 89%, responded with a remarkable regression of ROP with LP treatment. However, 8 eyes progressed to stage 4 and 5 despite treatment. Three eyes developed vitreous hemorrhage and 2 eyes developed preretinal hemorrhage. In the 6th month of the corrected age, spherical equivalent ranged between -8D to + 2.5D, however it ranged between -11D to + 2D in the 12th month. Hyperopia was present in 53% of all eyes and myopia was present in 25.5%. Mean visual acuity was 3.5 cpd (1.3-6.4) in the 6th month of the corrected age whereas it was 6.2 cpd (1.3-9.6) in the 12th month. Conclusion: Adequate and appropriate treatment with LP, has led to fair visual outcomes and an evident regression of ROP in all cases of the study group. Besides, hypermetropia was detected more commonly.Publication Current treatments in retinopathy of prematurity(Galenos Yayincilik, 2010-08-01) YILDIZ, MERAL; Yıldız, Meral; ÖZMEN, AHMET TUNCER; Özmen, Ahmet Tuncer; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı; AAH-1885-2021Increased worldwide incidences of retinopathy of prematurity have been noted due to the recent devolepments in newborn care and increased availability of newborn intensive care units. Increased number of Newborn Intensive Care Units leads to survive of increased number of immature babies, therefore, there is an increased number of premature infants to be screened and eyes to be treated. The aim of this review is to evaluate the current improvements and treatment modalities in retinopathy of prematurity. Introduction of anti angiogenetic agents for intraocular injection with or alternative to laser photocoagulation and criotherapy for treatment may broaden our view in the management of retinopathy of prematurity.Publication The newborn conjunctival flora at the post delivery 24 hours(Wolters Kluwer Medknow Publications, 2018-12-01) Kara, Mustafa; Kıvanç, Sertaç Argun; Olcaysu, Osman Okan; Budak, Berna Akova; Özmen, Ahmet Tuncer; Kıvanç, Merih; Öz, Hatice Tuna Hormet; KIVANÇ, SERTAÇ ARGUN; Budak, Berna Akova; ÖZMEN, AHMET TUNCER; Uludağ Üniversitesi/Tıp Fakültesi/Göz Hastalıkları Anabilim Dalı.; 0000-0003-0995-5260 ; AAH-6518-2021; ABE-3033-2020; AAH-6550-2021Purpose: The aim of this study was to investigate the aerobic conjunctival flora of neonates and the effects of delivery type on conjunctival flora development in neonates who were born with normal spontaneous vaginal delivery (SVD) or elective caesarean section (C/S) and who were not given prophylactic antibiotic eye drops after birth. Methods: This cross-sectional study included 95 healthy newborns. One day after the delivery, conjunctival samples were taken from newborns who were born with normal SVD or elective C/S, and not given prophylactic antibiotic eye drops after birth. Newborns with conjunctival hyperemia and discharge were excluded from study. Samples were plated in blood agar, EMB, and chocolate agar. These cultures were incubated at 37 degrees C for 24-48 h. Antibiotic sensitivity was evaluated using Kirby-Bauer disc diffusion method. Results: Staphylococcus aureus (S.aureus) growth was observed in 7 (70%) and coagulase negative staphylococcus (CNS) growth in 2 (20%) out of 10 eyes with bacterial growth in 9 culture positive newborns born with C/S. Two S. aureus strains were resistant to methicillin. On the other hand, CNS growth was observed in the conjunctival cultures of 17 out of 19 eyes with bacterial growth in 16 culture positive newborns born with SVD. In 2 eyes with CNS growth, there was also S. aureus growth. The positive cultures for S. aureus were significantly higher in the conjunctival cultures of neonates born with C/S compared to neonates born with SVD, where CNS growth was significantly lower (P = 0.002). All isolates were susceptible to vancomycin, teicoplanin, and gatifloxacin. Two isolates were resistant to methicillin. Conclusions: In deliveries with C/S, the newborn does not contact the vagina. This may result in changes of bacterial characteristic of the flora. Culture positivity for S. aureus was higher in C/S compared to SVD, which may be important in case neonatal conjunctivitis develops. Copyright (C) 2018, Iranian Society of Ophthalmology.
