Abstract
Cerrahi yöntem ile tedavi edilen 33 travmatik AV fistül olgusu arda dönük olarak incelendi. Arteriovenöz fistül en sık alt ekstremitenin büyük damarları arasında (% 72. 7) görüldü. Venöz yetmezlik (% 63.6), arteriyel yetmezlik (% 54.5) ue kalb yetmezliği (% 30.3) sık görülen komplikasyonlar. Pulmoner emboli(% 12.1), endokardit (% 9.0) ue böbrek yetmezliği (% 6.0) bulguları daha seyrek görüldü. Mikroskobik olarak arter mediası ve elastik tabakalarda incelme saptandı. Olguların çoğunda (% 66.6) arter ve ven, safen ven interpozisyonu ile onarıldı. Daha seyrek olarak safen ven yaması ile veya primer onarım yapıldı. Ameliyattan bir hafta sonra fistülün sistemik belirtileri tamamen kayboldu. Yirmi sekiz olgu (% 85.0) iyileştirildi. Bir olgu (% 3.0) öldü. Uç olguda (% 12.0) kronik vasküler yetmezlik oluştu.
Thirty-three cases with traumatic arterio-uenous (fistula which were treated surgically have been studied retrospectively. The fistula most frequently occured between the large vessels of the lower extremities (72. 7 %). Venous insufficiency (63.6 %), arterial insufficienr:y (54.5 %) and cardiac failure ( 30.3 %) were among the major complications. Pulmonary embolism (12.1 %), endocarditis (9. O %) and renal failure (6.0 %) were seen less frequently. Microscopically arterial media and elastic layers were thin. In the majority of cases (66.6 %) the artery and vein were repaired with an interposed saphenous vein. Less frequently saphenous vein patch and primary repair were performed. The systemic manifestations of the fistula completely disappeared one week after operation. Twenty·three cases (84.8), were cured, one patient (3.0 %) died. The remaining (12.0 %) had various vascular insufficiencies.
