Abstract
Kliniğimize kronik kusma, regurgitasyon ve kilo kaybı gibi şikayetlerle getirilen, 1.5 yaşlı, St. Bernard ırkı köpeğe yapılan klinik ve laboratuvar muayeneleri sonucunda idiopatik megaezefagus tanısı kondu. Klinik ve hematolojik olarak patolojik akciğer sesleri ve nötrofili haricinde bir anormallik gözlenmezken, radyolojik olarak megaözefagus belirlendi. İdrar sedimenti lökosit ve böbrek epitel kümeleri yönünden zengindi. EKG'de sinus aritmi ile birlikte ORS kompleksinin süresinin uzadığı gözlendi. Serum total protein, kreatinin, kortizol ve kolesterol düzeyleri ile GPT ve pankreatik amilaz aktiviteleri normal sınırlarda iken, üre düzeyi artmıştı. Serolojik olarak ANA, AdsDNA, ASMA, APA, ABMA ve AMA testleri negatif, ARF ise hafif pozitif olarak belirlendi. idiopatik megaözefagusa yönelik olarak semptomatik tedavi uygulanmasına rağmen, 5. günde ölen köpeğin nekropsisi yapıldı; makroskobik olarak megaözefagus, gastroözefagal sifinkter daralması (achalasia) ve bronkopnömoni, histopatolojik olarak ta eroziv-ülseratif özefagitis, aspirasyon pnömonisi ve intersitiel nefritis tespit edildi.
In the St. Bernard-breed-dog, 1.5 year old, referred to the clinic with history of chronic vomiting, regurgitation and weight loss, the diagnosis of idiopathic megaesophagus was made as a result of clinical and laboratory examination. While clinically and hematologically no abnormalities were detected with the exception of pathologic lung sounds and neutrophilia, megaesophagus was observed radiologically. There were many leukocyt and renal epithelial casts in urine sediment. On the ECG, sinus aritmia and prolongation of duration of ORS complex were monitored. While the levels of serum total protein, creatinin, cortisole and cholesterole, and GPT and P-Amylase enzyme activities were within normal limits, the level of urea increased. Serologically, it was observed that ANA, AdsDNA, ASMA, APA, ABMA and AMA test were negative, but ARF was mild positive. In spite of semptomatic treatment for idiopathic megaesophagus, necropsy of dog died in the 5th day was performed; macroscopically megaesophagus, narrowed gastroesophagal sphincter (achalasia) and bronchopneumonia, histopatologically, erosiv and ülcerativ eosophagitis, aspiration pneumonia and interstitiel nephritis were observed.
