Varikosel tedavisinde farklı bir yaklaşım: Spermatiko-kollateral safen ven şantı (bir olgu nedeniyle)

Abstract
Günümüzde varikosel tedavisinde en sık olarak kullanılan yöntem internal spermatik venin cerrahi olarak bağlanmasıdır. Sol skrotal ağrı ile başvuran ve sol grade 3 varikoselin yanısıra spermiogramında belirgin astenospermi saptadığımız bir hastamıza spermatik-kollateral safen ven şant drenaj uyguladık. Ameliyat sonrası 7. günde varikoselin tamayakın kaybolduğu, skrotal ağrı yakınmasının geçtiği ve ameliyat sonrası 1. günden itibaren normal günlük yaşama geçebildiği gözlendi. Ayrıca ameliyat sonrası 6. ayda yapılan spermiogramında astenosperminin düzeldiği saptandı. Erken semptomatik ve kozmetik iyileşme ve daha önemli olarak bozuk semen parametreleri düzelmesi gibi nedenlerle bu yöntemin diğer klasik varikoselektomi yöntemleri arasında yer olabileceğini düşünmekteyiz.
The most frequent current treatment modality of varicocele is surgical high ligation of internal spermatic vein. We performed a spermatico-saphenous collateral venous shunt operation to a patient having grade 3 left varicocele with symptoms of left scrotal pain as well as severe astheno-spermia on his spermiogram. Up to complete resolution of his varicocele and scrotal pain on postoperative day seven and ability to return to normal daily activity on postoperative day one were observed. In addition, 6 months later his spermiogram revealed that asthenospermia resolved. For the reasons of early symptomatic and cosmetic relief and more importantly the improvement of altered seminal parameters, we think that this form of surgical therapy may be encountered with the other classic treatment modalities. lt is planned to continue performing this surgical approach.
