Abstract
Tiroid dokusu embriyolojik hayatta tiroglossal duktusun içinden geçerek normal pretrakeal lokalizasyonuna inmektedir. Tiroid dokusunun embriyolojik gelişim defekti sonucu başka lokalizasyona anormal göçü sonucu ektopik tiroid dokusu oluşur. Burada 27 yaşında submental yerleşimli lingual tiroid dokusu saptanan, çocukluk çağından beri hipotiroidi nedeniyle takip edilen bir kadın hasta sunulacaktır. Yapılan boyun ultrasonografisinde normal lojunda tiroid dokusu saptanmamış, tiroid sintigrafisinde ise submental bölgede 1.5 cm’lik radyofarmasötik aktivite tutulumu saptanmıştır. Klinik değerlendirme ve görüntüleme yöntemleri ile submental yerleşimli ektopik tiroid dokusu tanısı aldı. Bası semptomları tariflemeyen hasta L-tiroksin replasman tedavisi ile takibe alınmıştır. Hipotiroidinin nadir bir nedeni olarak ektopik tiroid, özellikle erken yaşta başlayan hipotiroidik hastalarda akla gelmelidir.
Thyroid tissue passes through thyroglossal duct during embryologic life and locates to the normal pretracheal position. Depending on the defective embryonic development and abnormal migration to another location of thyroid tissue, ectopic thyroid tissue occurs. Here we present a female patient aged 27, with submental localized lingual thyroid tissue, who presented with hypotiroidisim at childhood. There was no thyroid tissue at the normal thyroid region by ultrasonography and 1.5 cm radiopharmaceutical uptake was spotted at thyroid scintigraphy. Submental localized ectopic thyroid tissue was diagnosed with clinical evaluation and imaging methods. The patient, without any pressure symptoms, was followed up by treatment with L-thyroxine. Ectopic thyroid tissue as a rare cause of hypothyroidism should be considered especially at patient who diagnosed hypothyroidism at early ages.
