Abstract
Sezaryen girişimlerinde spinal anestezi sonrasında viseral çekilmelere bağlı bulantı-kusma, ağrı, baş dönmesi gibi reaksiyonlar önlemek amacıyla Bupivakain + Fentanil kombinasyonu uygulandı. 35 hastanın 24'üne Bupivakain % 0.5 heavy solüsyondan 7.5-10 mg + Fentanyl 10 ug, 11'ine ise sadece Bupivacaine % 0.5 heavy solüsyondan 7.5-10 mg. ile spinal anestezi verildi. Olguların kan basıncı, nabız dakika sayısı, solunum sayısı, spinal anestezi düzeyi, analjezi başlama zamanı, apgar, operasyon sırasındaki komplikasyonlar, ağrının başlama zamanı, ilk analjezik yapılma zamanı ve visuel analog skorlama yöntemi ile postoperatif ağrının şiddeti kaydedildi. Operasyon sonrası ağrının şiddeti kontrol grubunda deney grubuna göre anlamlı derecede (t = 2.62, p < 0.01) yüksek bulundu. Diğer parametreler yönünden her iki grup arasında fark yoktur. Deney grubunda görülen kaşıntı, eritem gibi minör komplikasyonlar antihistaminiğe iyi cevap verdi. Spinal anestezi sırasında viseral çekilmelere bağlı olarak görülen rahatsızlıkların giderilmesinde lokal anestezik maddeye eklenen küçük dozda Fentanilin yararlı olduğu sonucuna varıldı.
In this study Bupivacaine and Fentanyl combination was used for spinal anesthesia in Cesarean Section operations to prevent the side effects due to visceral traction. Spinal anesthesia was performed in 24 of 35 patients with 7.5-10 mg. of 0.5 % Bupivacaine heavy solution + 10 pg of fentanyl and in II. patients with only the same amount of heavy Bupivacaine. Blood pressure, heart rate, respiration rate, level and onset of spinal anesthesia, apgar scores, intraoperative complications were recorded. Also postoperative pain was evaluated using VAS (Visual Analogue Score). Postoperative pain was found to be significantly higher in the control group. There was no difference in reference to other variables between the two groups. Minor complications such as itching, erythema encountered in the experimental group responded well to antihistamines. It was concluded that fentanyl in small doses added to local anesthetics during spinal anesthesia is useful in the prevention of discomfort resulting from visceral traction.
