Abstract
Acil ambulans sisteminin ve sevk zincirinin uygunsuz olarak kullanılması günümüz modern sağlık sisteminin sorunlarından biridir. Halkın bu hizmeti kullanım oranı kişilerin sosyoekonomik şartlarına, sağlık hizmetlerine ulaşım süresine, yaşına ve geçirdiği kazanın veya hastalığının derecesine göre değişebilir. Çalışmamızda; üniversitemiz acil servisine 112 acil ambulans hizmetleri aracılığı ile başvuran hastaların ve hasta yakınlarının sosyokültürel, sosyoekonomik ve eğitim durumlarının incelenmesi amaçlanmıştır. Acil servisimize 112 ambulansı ile başvuran 500 hasta prospektif olarak çalışmaya dahil edildi. Hastalar, hasta yakınları ve 112 ekipleri ile görüşülerek hastaların verileri toplandı. Cinsiyet, yaş, eğitim durumu, gelir durumu, önceki ambulans kullanımları, sağlık güvencesi, çalışma durumu, şikayetleri ve tanısı gibi bilgileri toplanarak istatistiksel analizi yapıldı. Çalışmamızda ambulans kullanımının erkeklerde daha fazla olduğu ve 65 yaş üstü grupta fazla olduğu saptanmıştır. Gelir düzeyi artışının uygunsuz ambulans kullanımını azalttığı görülmüştür. Daha önceden ambulans kullanımı olmayan hasta grubunun daha fazla oranda olduğu görülmüştür. Yine bu grupta uygunsuz kullanım oranı daha yüksek oranda saptanmıştır. 112 aramalarında en yüksek oranın hasta yakınları tarafından aranma olduğu ve en düşük oranın ise hastanın kendisi tarafından aranması olduğu görülmüştür. Sonuç olarak; uygun ambulans kullanımı için hastaların bilinçlendirilmesi ve hastaların acil servis başvurularında kullanabilecekleri alternatif ulaşım sistemleri gerekmektedir.
Improper use of the emergency ambulance system and transfer systems is one of the most important problems of today's modern health care system. People's use of this service may vary depending on the socioeconomic conditions, the time for accessing to health services, the age, and the degree of the accident or disease. In our study; it was aimed to reveal the sociocultural, socioeconomic and educational status of the patients and their relatives who applied to the emergency department of our university through 112 emergency ambulance services. 500 patients who referred to our emergency department with an ambulance of 112 were included in our study. Patients, their relatives and 112 teams were interviewed and the data of the patients were collected. Statistical analysis was conducted by gathering information such as gender, age, educational status, incomes tatus, previous ambulanceusage, health insurance, working status, complaints and diagnosis. We found that the use of ambulance in our study was higher in men and in over 65 years age group. The increase in income level was found to reduce the inappropriate ambulance use. It was obtained that the patient group, which had not previously used ambulance, was found to be higher. The highest ambulance call rate was found to be called by the patient's relatives and the lowest rate was found to by the patient himself. As a result; the consciousness should be increased for appropriate ambulance use and alternative transporting systems for emergency admissions should be developed.
