Abstract
Testisin primer non-Hodgkin lenfoması nadir görülen bir hastalıktır1-13. Tüm testis malignitelerinin %1-9’unu ve tüm non-Hodgkin lenfomaların yaklaşık %1-2’sini oluşturur2,4,7,8,10,13. Nadir görümesine rağmen, 60 yaşın üzerinde en sık görülen testis malignitesidir2-8,13. Genellikle ileri yaşlarda rastlanır. Ortalama yaş 60’tır6-8 . Tanı histopatolojik olarak konulur3,11. Diffüz büyük B hücreli lenfoma (DBBHL) en sık görülen histolojik alt tipidir1-6,10-13. Prognozu belirleyen en önemli parametre evre ve histolojik derecedir. Prognozu diğer testis tümörlerinden kötü olup, ilk 1 yıl içinde yayılım bulgusunun olmaması iyi prognoz işareti olarak kabul edilir1-6,9. Yaygın nodal lenfomalarda testis tutulumuna daha sık rastlanırken, testisin primer lenfoması nadirdir. Primer testis lenfoması olgularının %57’si evre I, %22’si evre II ve %21’i de evre III-IV olarak karşımıza çıkmaktadır1,2,4. Özellikle erken evre hastalıkta standart bir tedavi mevcut olmamakla birlikte, evre I ve II hastalıkta, orşiektomi sonrası doksorubisinli veya antrasiklinli kemoterapi uygulaması önerilmektedir7,8. İleri evre (evre III ve IV) hastalıkta kombinasyon kemoterapisi tercih edilen tedavi modalitesidir1,2. Bu makalede nadir görülen PTL tanısı alan olgunun klinikopatolojik bulgularının literatür bilgileri eşliğinde tartışılması amaçlanmıştır
Primary non-Hodgkin lymphoma of testis is an rare disease1-13. It accounts for 1%- 9% of all testicular neoplasms and 1-2% of all nonHodgkin lymphomas2,4,7,8,10,13. Despite this low overall incidence, it is the most common testicular malignancy in men aged over 60 years2-8,13. It is usually seen in elderly patients and mean the age of 606-8 . The diagnosis is done through histological examination3,11. The most commonly encountered histological type is diffuse large B celll ymphoma1-6 . Clinical stage and histological grade is the most important prognostic factors. The prognosis is worse than other testicular tumors and, the evidence of no extension within a year suggest a good prognosis1-6,9. Primary testicular lymphoma (PTL) is infrequent whereas involvement of testis is common in nodal lymphomas. Fifty seven percent of cases with PTL present with stage I, 22% with stage II and 21% with stage III-IV1,2,4. Although there is not a standart therapy especially for stage I and II disease, treatment with doksorubicin or antrasiklin based chemotherapy following orchiectomy is recommend in these stages7,8. In advanced stages (III and IV), combination chemotherapy is the treatment of choise1,2. In this article we aimed to discuss the clinicopathological features of the cases diagnosed as PTL which is an rare disease with the review of the literature.
