Abstract
Bu araştırma Uludağ Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Göz Kliniğinde günübirlik cerrahi ile katarakt ameliyatı olan hastalara (n=72 müdahale; n=72 kontrol grubu) verilen taburculuk eğitimi ile telefonla hasta izleminin hastaların iyileşme sürecine ve günlük yaşam aktivitelerine etkisinin değerlendirilmesi amacıyla yapılmıştır. Araştırmanın verileri Şubat-Haziran 2017 tarihlerinde veri formu ve hasta izlem formu ile toplanmıştır. Hastaların taburculuk eğitiminde yaşam modeline göre hazırlanmış bilgisayar destekli eğitim ve eğitim kitapçığı kullanılmıştır. Veriler SPSS-20 ile değerlendirilmiştir. Bulgular sayı ve yüzde, ortalama ve standart sapma olarak verilmiş, analizlerde Pearson Ki- Kare, Mann Whitney U testleri kullanılmıştır. Müdahale grubundaki hastaların yaş ortalaması 67,74±11,42 bulunmuştur; kontrol grubundaki hastaların yaş ortalması ise 69,74±10,8'dir. Hastaların katarakt ameliyatı sonrasına ilişkin bilgi düzeyleri ortalamaları müdahale grubunda 1,38±1,74;kontrol grubundaki ise1,04 ± 1,16'dir. Hastalara planlı olarak verilen taburculuk eğitiminin yaşam modelinde yer alan bazı parametreleri olumlu etkilediği görülmüştür. Göz damlalarını damlatma, eski gözlüklerini kullanma, gözün zarar görmesini önlemek için yapmaları gerekenleri bilme, doktoru aramaları gereken durumları ayırt edebilme, ameliyatlı gözün etkilenmesine yol açacak durumların farkında olma, bireysel hijyen, hareket ve uyku ile ilgili parametrelerde gruplar arasındaki fark anlamlıdır(p<0,05).Hareketle ilgili tüm parametrelerde müdahale ve kontrol grubundaki hastaların endişe durumları arasındaki fark anlamlıdır ve müdahale grubundaki hastaların endişelenme durumu yüksek bulunmuştur (p<0,05).İstatistiksel olarak anlamlı olmamakla birlikte eğitim verilen grupta ağrı skorlarının tüm günlerde (1., 3. ve 10. gün) daha yüksek olduğu; eğitimin iletişim kurma ve telefon etmeyi, yemek yemeyi ve boşaltımı, cinsel aktiviteyi etkilemediği ve olumsuz bir gelişme olabileceği konusunda endişe yaratmadığı (p>0,05) görülmüştür.
This study was carried out with patients that had cataract surgery (study group =72, control group=72) in Uludağ University Medical Hospital Eye Clinic. The aim of the study was to determine the impact of the discharge education given to the patients and telephone follow up on their recovery. Data were collected between February- June 2017 using data collection form and patient follow up form designed for this study. Computer assisted discharge education along with patient education booklet were designed according to Model of Living used for patient education of the study group. Data were evaluated using SPSS 20 statistical software. Results were given in numbers, percentages, means and standart deviations. Statistical analyisis were done by using Pearson Chi Square, Mann Whitney U tests. The mean age of the patients that were included in study group was 67,74±11,42. Mean age of the control group patients was calculated as 69,74±10,8. The knowledge related to care after cataract surgery was 1,38±1,74 for study group and 1,04 ± 1,16 was for control group. The discharge education given to patients was found to be effective on some parameters of the Living Model. Statistically significant differences were found between study and control group on applying eye drops, knowledge on using previous eye glasses, knowledge related to protection of the affected eye, differenciate the conditions which requires to call the doctor, conditions that may affect the eye negatively, personal hygene, mobilization and sleeping (p<0,05). All of the parameters related to mobilization were statistically significant. It was found that concerns of the study group patients was higher than control group in parameters of mobilization (p<0,05). Although it was statistically insignificant, pain scores of the patients in study group were high in the 1st, 3rd and 10 th days after cataract surgery. It was also found that discharge education did not have any significant influence on patients' communication, eating and drinking, elimination, sexuality and death activities (p>0,05).
