Abstract
Bu çalışmanın amacı, hemşirelerin mesleki yaşamları boyunca karşılaşabilecekleri etik problemler, etik ikilemler ve etik ihlallere ilişkin farkındalık düzeylerini belirlemek için geçerli ve güvenilir bir ölçüm aracı geliştirmektir. Araştırma metodolojik bir çalışmadır 15.04.2022 (Mayıs, Haziran, Temmuz) tarihinde SBÜ Bursa Yüksek İhtisas Eğitim Araştırma Hastanesinde 453 hemşirenin katılımıyla yapılmıştır. Veriler Sosyo demografik Bilgi Formu ve 5’li Likert tipinde hazırlanan 58maddelik ölçek taslak formu kullanılarak toplanmıştır. Verilerin analizinde IBM SPSS22.0 ve AMOS paket programı kullanılmıştır. Verilerin normal dağılıp dağılmadığı normallik testlerinin yanı sıra histogram, Q-Q grafiği ve kutu-çizgi (Box-Plot)grafikleri ile çarpıklık ve basıklık varyasyon katsayısı gibi dağılım ölçüleriyle değerlendirilmiştir. Kapsam geçerliği analizinde Kendall’s W iyi uyum analizi kullanılmıştır. Ölçek alt boyutları Açımlayıcı ve Doğrulayıcı Faktör Analizi ile belirlenmiş; güvenirliği Cronbach’s Alfa Katsayısı ve Madde Toplam Puan Analizi için de Pearson Momentler Çarpımı Korelasyonu kullanılmıştır. HMEİFÖ ve alt boyutlarına ait toplam puanlar arasındaki farklılıklar diğer özellikler açısından birbirleriyle Tek Yönlü Varyans Analizi (ANOVA) ve Bağımsız Gruplar T Testi ile karşılaştırılmıştır. Doğrulayıcı Faktör Analizine göre HMEİFÖ’inin yapısal denklem model sonucunun (Structural Equation Modeling Results) p≤0,001 düzeyinde anlamlı olduğu, ölçeği oluşturan 27 madde ve 4 faktörlü ölçek yapısıyla ilişkili olduğu belirlenmiştir. HMEİFÖ’nin Kaiser-Meyer-Olkin katsayısı (KMO) değeri 0,95,Bartlett's Test of Sphericity Approx. Chi-Square değeri 6953,231 ve p≤0,001 olarak anlamlı olarak bulunmuştur. Ölçeğin Kapsam Geçerliliği İndeksi (KGİ) aralığı 0,92-1,00; faktör yükleri 0,452-0,8070, madde toplam korelasyonları p<0,05 ve alt boyutların Cronbach Alpha Güvenirlik katsayıları (Ahlaki Düşünce: 0,94; Erdemli Olma: 0,89; Görev Bilinci: 0,74; Mesleki Yükümlülük: 0,63) ölçeğin genel puanı 0,95olup yüksek güvenirliktedir. Birinci düzey çok faktör analizi sonuçlarına göre HMEİFÖ Model uyum iyiliği indeksi: x2 (Cmin/df) =2,584≤3, RMSEA=0, 059≤0,08,CFI=0,925≥0,90, RMR=0,041≤0,05 olarak bulunmuş ve modelin istatistiksel uyum gösterdiği tespit edilmiştir (p≤0,001). Sonuç olarak, hemşirelerin mesleki etik ilkeler farkındalık düzeylerini belirlemede HMEİFÖ’nin geçerli ve güvenilir bir ölçüm aracı olduğu gösterilmiştir.
The aim of this study is to develop a valid and reliable measurement tool to determine the level of awareness of nurses about ethical problems, ethical dilemmas and ethical violations that they may encounter throughout their professional lives.The research is a methodological study. It was conducted on 15.04.2022 (May, June, July) with the participation of 453 nurses at SBU Bursa Yüksek İhtisas Training and Research Hospital. The data were collected using the Sociodemographic Information Form and the 58-item draft scale prepared in 5-point Likert type. IBM SPSS 22.0 and AMOS package program were used in the analysis of the data. Whether the data were normally distributed or not was evaluated by means of distribution measures such as histogram, Q-Q plot, box-plot graphs, skewness and kurtosis coefficient of variation, as well as normality tests. Kendall's W good fit analysis was used in the content validity analysis. Scale sub-dimensions were determined by Exploratory and Confirmatory Factor Analysis; Reliability Cronbach's Alpha Coefficient and Pearson Product-Moment Correlation were used for Item Total Score Analysis. The differences between the total scores of the HMEIS and its sub-dimensions were compared with each other in terms of other characteristics with One-Way Analysis of Variance (ANOVA) and Independent Groups T-Test. According to the Confirmatory Factor Analysis, it was determined that the structural equation modeling results (Structural Equation Modeling Results) of the HMEIS were significant at the p≤0.001 level and were associated with the scale structure with 27 items and 4 factors. The Kaiser-Meyer-Olkin coefficient (KMO) value of the HMEIO was 0.95, Bartlett's Test of Sphericity Approx. The Chi-Square value was found to be 6953.231 and p≤0.001 as significant. Scope Validity Index (CGI) range of the scale was 0.92-1.00; factor loads 0.452-0.8070, item total correlations p<0.05, and Cronbach Alpha reliability coefficients of sub-dimensions (Moral Thinking: 0.94; Virtuousness: 0.89; Duty Consciousness: 0.74; Professional Obligation: 0, 63) the overall score of the scale is 0.95, with high reliability. According to the results of the first level multi-factor analysis, the HMEIE Goodness of Fit index was found to be: x2 (Cmin/df) =2.584≤3, RMSEA=0.059≤0.08, CFI=0.925≥0.90, RMR=0.041≤0.05, and it was determined that the model showed statistical agreement (p≤0.001). As a result, it has been shown that HMEİFÖ is a valid and reliable measurement tool in determining nurses' awareness of professional ethical principles.
