Abstract
Kemik yaşı tayininin tıbbi ve adli uygulamalarda önemli bir yeri vardır. Günümüzde 18 yaş altı bireylerde tamamlanmış el ve el bileği kemik osifikasyonularına bakılarak yaş tayini yapılabilmektedir. Ancak tamamlanmış el ve el bileği osifikasyonları nedeni ile 18 yaş üstü vakalarda yaş tayini yapmak oldukça zordur. Bu pilot çalışma, 18 yaş üstü bireylerde sakral vertebra korpusları arasındaki füzyon derecesini skorlayarak, yaş tayini yapılabilmesini amaçlamaktadır. Çalışmamızda, lomber ya da sakrum bilgisayarlı tomografi (BT) tetkiki yapılmış, yaşları 15- 64 arasında değişen 174 erkek, 179 kadın toplamda 353 hastanın sagittal reformat BT görüntüleri iki radyolog tarafından retrospektif olarak çift kör değerlendirilmiştir. Sakral vertebra korpusları arasındaki füzyon dereceleri Belcastro ve ark.’nın tanımladığı 4’lü evreleme sistemine göre evrelendirilmiştir. Çalışmamızda erkek olgularda yaş ile sakral füzyon skorları arasında istatistiksel olarak anlamlı ilişki saptanmıştır (p<0,05). Kadın olgularda ise S2-S3 ve S3-S4 düzeyleri için yaş ile sakral füzyon düzeyleri arasında istatistiksel olarak anlamlı farklılık mevcuttur (p<0,05). Ayrıca kadın olgularda genel olarak aynı yaş grubundaki erkeklere kıyasla daha ileri füzyon dereceleri izlenmiştir. Ancak her iki cinsiyette de Spearman korelasyon katsayısı ile yapılan incelemede sakral vertebral füzyon dereceleri ile yaş arasında düşük derecede uyum olması, bu tekniğin pratikte yaş tayininde uygulanabilirliğinin önünde engel oluşturmaktadır. Bu konuda objektif bir değerlendirmenin yapılabilmesi için daha fazla olgu sayısı ile yeni araştırmaların yapılması gerekmektedir.
Bone age estimation has an important role in both medical and forensic studies. The age of individuals below 18 years can be determined with a low degree of error in regard to narrow age ranges associated with ossification of hand and wrist bones. In cases over 18 years old, completion of hand and wrist bone ossification makes age estimation a difficult process. In this pilot study, it was aimed to detect whether sacral vertebra corpus fusions (SVF) can be used for age estimation. Sagittal reformatted computed tomography images from 174 male and 179 female, a total of 353 patients, age ranging from 15 to 64, who admitted to radiology department of Uludag University Faculty of Medicine were examined by two radiologists independently and retrospectively. Sacral vertebra corpus fusions were evaluated by using four-stage scoring method defined by Belcastro et al. in 2008. In male subjects there was a stastically significant relationship between age and SVF scores (p<0,05); in female subjects there was a statistically significant relationship between age and SVF scores for stages S2-S3 and S3-S4 (p<0,05). Sex differences in SVF scores were found, with females showing earlier fusion than males. Even there is statistically significant relationship in males and females for some stages between age and SVF, in both sexes Spearman’s correlation coefficient corresponds a low grade relationship between age and SVF, that prevents practical use of this method for age estimation in routine. Furt her studies with more number of subjects should be made for a clear determination of relationship between SVF and age.
