Abstract
Çalışmada kardiyak otonom nöropati (KON) pozitif diyabetes mellitus (DM)’lu olgular ile sağlıklı kontrol grubunda arteriyel sertlik ölçülerek karşılaştırıldı. Çalışmaya 26 tip II DM’li hasta ve 12 sağlıklı gönüllü alındı. Tip II DM’li olgulara Ewing testi uygulanarak ≥1 puan alanlar KON pozitif kabul edildi. Radiyal arter nabız dalga hızı (NDH) “Pulse Wave Sensor HDI system” ile ölçüldü. Büyük arter (BAEİ) ve küçük arter elastisite indeksleri (KAEİ) nabız dalgasının otomatik analizi ile hesaplandı. KON tip II DM’li olguların 21’inde pozitifti. KON pozitif tip II DM’li olgularda sağlıklı kontrol grubu ile karşılaştırıldığında hem BAEİ (sırasıyla 7.72±2.77, 14.8±3.31 ml/mmHgx10, p<0.001) ve hem de KAEİ (3.4±1.55, 5.9±2.99 ml/mmHgx100, p=0.033) istatistiksel olarak anlamlı düzeyde daha düşüktü. Bu bulgular girişimsel olmayan ve kolay uygulanabilir bir tetkik yöntemi olan NDH ölçülmesinin, DM seyrindeki kardiyovasküler olayların ön görülmesinde faydalı bir yöntem olabileceğine işaret etmektedir.
Introduction: In this study, arterial stiffness was measured and the levels of cases with Cardi¬ac Autonomic Neuropathy (CON) positive type II diabetes mellitus (DM) patients were compared with healthy controls. Method: In the study, 26 type II DM patients and 12 healthy volunteers were controlled. By per¬forming Ewing test in type II diabetes mellitus patients, the cases with ≥1 scores were regarded as being CON (+). Radial artery pulse wave rate was measured using “Pulse Wave Sensor HDI system”. Great Artery Elasticity index (GAEI) and small artery Elasti¬city Index (SAEI) was calculated with automatic analysis of pulse wave. Results: CON was positive in 21 of type II patients. When CON positive type II DM patients were compared to healthy controls, both GAEI (7.72±2.77 vs 14.8±3.31 ml/mmHgx10, p<0.001) and SAEI (3.4±1.55 vs 5.9±2.99 ml/mmHgx100, p=0.033) were found to be significantly lower. Conclusion: These findings suggest that the pulse wave mea¬surement (a noninvasive, easily applicable test) may be useful method for prediction cardiovascular events in the course of DM.
