Abstract
Aktinomikozis nadir görülen, fistül ve apse formasyonları gösteren süpüratif veya kronik granülomatöz bir enfeksiyondur. Enfeksiyon anaerobik, gram pozitif bir bakteri olan Actinomyces Israelii tarafından meydana getirilir. Hastalığın servikofasial, abdominopelvik ve pulmonotorasik olmak üzere üç formu mevcuttur. Servikofasial aktinomikozis fasial travma, fraktür. veya oral bakteri enfeksiyonları sonucunda gelişir ve tüm aktinomikozis bulgularının % 55'ini oluşturur. Histolojik olarak hastalıkta süpüratif materyal içerisinde sulfur granüllerinin varlığı patognomoniktir. Biyopsi ve kültür yapılmadan tüberküloz, tularemi ve diğer granülomatöz hastalıklar, bakteriyel osteomyelitis ve malignancies karışabilir. Olgumuzda sağ servikal bölgede yerleşim gösteren kitle mevcut olup, biyopsiden önce klinik olarak tüberküloz, tularemi ve malignensi düşünülmüştür.
Actinomycosis is an uncommon infection which is characterized by chronic granulomatous and suppurative disease with abscess and fistula formation. The infection is caused by the anaerobic, gram positive bacteria Actinomyces Israelii. It has three forms that are Cervicofacial, abdominopelvic and pulmonary thoracic. Cervicofacial actinomycosis which occurs following facial travma, fractures or infection with oral bacteria is seen in 55 % of the patients with actinomycosis. Histologically, the disease is characterized by the presence of sulfur granules in the suppurative material. The presence of these granules is pathognomonic. Before biopsy and culture is confused with tuberculosis, tularemia and other granulomatous disease, bacterial osteomyelitis and malignant neoplasms. In our patient, there is a mass on the right cervical region and it was thought clinically tuberculosis, tularemia and malignancy before biopsy.
