Abstract
Bu çalışmada, spesifik bir H reseptör blokeri olan Cimetidine (CM) ile tedavi edilen 37 idiopatik hirsutismus olgusunun sonuçlan sunulmuştur. CM, kıl foliküllerinde bulunan androjen reseptörlerine bağlanarak kalın büyümesini engellemektedir. CM tedavisi ile kıl çaplarında istatistiksel olarak anlamlı bir (p < 0.005) azalma saptanmıştır. Diğer taraftan CM, hirsut kadınlarda genellikle yüksek bulunan LH'un supresyonu aracılığıyla overlerde androjen sentezini de inhibe etmektedir. olgularımızda CM tedavisi süresince serum total testosteron (p < 0.01), serbest testosteron (p < 0.001) ve androstendion (p < 0.025) düzeylerinde istatistiksel olarak anlamlı bir azalma; sex hormone binding globulin (p < 0.001) düzeylerinde ise anlamlı bir yükselme saptanmıştır. Östradiol kortizol ve DHEAS'un serum düzeylerinde anlamlı bir değişiklik görülmemiştir. Sonuç olarak, CM'in idiopatik hirsutismus tedavisinde etkili ve zararsız bir ilaç olduğu kanısına varılmıştır.
In this study, we present the results obtained in 37 idiopatic hirsute women who were treated with Cimetidine (CM), a specific H2-reseptor blocker. CM binds to tlıe androgen reseptors which are present on the hair follicle and prevents the hair growth. We found a statistically significant (p < 0.005) reduction of the hair diameter with CM therapy. In addition, CM inhibits the ovarian androgen synthesis by supression of the pituitary LH level which is elevated in hirsute women. A significant decrease of serum levels of total (p < 0.01) and free testosterone (p < 0.001), androstenedione (p < 0.025), and a significant increase of sex hormone binding globulin (p < 0.001) was determined in our cases during CM therapy. There were no statistically significant changes in serum levels of estradiol, cortisol and DHEAS. The present data suggest that CM may be considered a safe, but more effective agent in the treatment of idiopathic hirsutism.
