Current evidence does not conclude any superiority between fixed pressure support (PS) and aver-age volume-assured pressure support (AVAPS) modes. However, given the ability of rapid PaCO2 decline in AVAPS mode, we hypothesized that COPD patients with AHRF who did not show the desired reduction in PaCO2 with fixed-level PS-NIV might benefit from the AVAPS mode.Materials and Methods: Patients admitted to the non-ICU pulmonary ward with acute exacerbation of COPD (AECOPD) and AHRF were included con-secutively in this observational study. Patients with hypercapnic respiratory failure due to obesity-hypoventilation, neurological diseases, or chest wall deformities were excluded. All patients started NIV treatment with fixed pres-sure support (PS) and patients who did not reach clinical and laboratory stability under PS-NIV treatment were switched to the average volume -as-sured pressure support (AVAPS) mode of NIV.Results: Thirty-five COPD patients with hypercapnic respiratory failure were included. Under PS-NIV treatment, 14 (40%) patients showed a 17.9 (-0.0-29.2) percent change in terms of PaCO2 , meaning no improvement or worsening. Therefore, these patients were treated with AVAPS mode. Arterial PaCO2 and pH levels significantly improved after AVAPS-NIV administration. AVAPS-NIV treatment created a significantly better PaCO2 change rate than using PS-NIV (-11.4 (-22.0 --0.5)vs 8.2 (-5.3-19.5), p= 0.02]. Independent predictors of AVAPS mode requirement were higher Charlson Comorbidity Index (OR= 1.74 (95% CI= 1.02-2.97)] and higher PaCO2 upon admission (OR= 1.18 (95% CI= 1.03-1.35)]. Therefore, public health policies with respect to smoking and education of the general population regarding the harmful effects of active and second-hand smoking may not have received adequate attention during this period. More specifically, certain subpopulations suffering from chronic diseases may not have received adequate information about the effects of smoking on the course and outcome of their disease; high-level, evidence-based pharmaceutical therapies; and the potential for follow-up. However, adequate education and awareness regarding short- and long-term health benefits from smoking cessation for the general population as well as special subgroups remains of utmost importance. In this study, we investigated the comorbid medical conditions and sociodemographic features that contribute to disparities in tobacco smoking prevalence in the adult population. We induded volunteers who participated in events for the "World COPD day" in 4 consecutive years (i.e., 2014, 2015, 2016, and 2017) held in Bursa, Turkey. Sociodemographic features, smoking characteristics, and comorbidities were recorded. A total of 747 volunteers who were 47.1 +/- 14.5 [16-84] years of age and smoked 24.1 +/- 20.91 pack-years were included. The study population consisted of 405 (54.2%) males, 36.3% of which were current smokers, 23.6% exsmokers, and 40.2% nonsmokers. Moreover, 15.7% of participants with asthma were nonsmokers and 9.4% ever-smoker (p=0.011), while 2% of participants with coronary artery disease (CAD) were nonsmoker and 6.7% were ever-smoker (p=0.003). Current smoker participants exhibited more symptoms such as cough. sputum, and wheezing (p<0.001, in both cases). This study aims to find a predictive scoring model to estimate the COVID-19 diagnosis.Materials and Methods: Patients who applied to the emergency pandemic clinic between April 2020 and March 2021 were enrolled in this retrospective study. At admission, demographic characteristics, symptoms, comorbid diseases, chest computed tomography (CT), and laboratory findings were all recorded. Development and validation datasets were created. The scoring system was performed using the coefficients of the odds ratios obtained from the multivariable logistic regression analysis.Results: Among 1187 patients admitted to the hospital, the median age was 58 years old (22-96), and 52.7% were male. In a multivariable analysis, typical radiological findings (OR= 8.47, CI= 5.48-13.10, p< 0.001) and dyspnea (OR= 2.85, CI= 1.71-4.74, p< 0.001) were found to be the two important risk factors for COVID-19 diagnosis, followed by myalgia (OR= 1.80, CI= 1.082.99, p= 0.023), cough (OR= 1.65, CI= 1.16-2.26, p= 0.006) and fatigue symptoms (OR= 1.57, CI= 1.06-2.30, p= 0.023). In our scoring system, dyspnea was scored as 2 points, cough as 1 point, fatigue as 1 point, myalgia as 1 point, and typical radiological findings were scored as 5 points. This scoring system had a sensitivity of 71% and a specificity of 76.3% for a cut-off value of >2, with a total score of 10 (p< 0.001).Conclusion: The predictive scoring system could accurately predict the diagnosis of COVID-19 infection, which gave clinicians a theoretical basis for devising immediate treatment options. Although the STOP-BANG questionnaire is an attractive screening tool because of high sensitivity, it lacks power in specificity. The aim of the present study was to evaluate and compare the diagnostic accuracy of standard STOP-BANG and a modified STOP-BANG questionnaire, using national cut-off values for neck circumference that determined OSA, in a sleep center population.MATERIALS AND METHODS: One hundred eighty-five participants who were referred to the sleep-disordered breathing clinic were consecutively enrolled. We used 40 cm and 36 cm as the cut-off values for neck circumference, thus scoring patients accordingly and creating a modified STOP-BANG score with national anthropometric obesity indexes.RESULTS: The median neck circumferences were 41 (39-44) cm, 40 (37-42) cm, and 43 (40-45) cm for total population, female gender, and male gender, respectively. The mean STOP-BANG score was 4.5 +/- 1.5, and the mean modified STOP-BANG score was 4.9 +/- 1.5. Discrimination of OSA measured by area under the curve for both questionnaires is comparable (p>0.05). Sensitivity to define OSA (apnea-hypopnea index (AHI)>= 1.5) was 92.2% and 93.8% for original and modified STOP-BANG questionnaire, respectively. Sensitivity for moderate (AHI >= 15) and severe OSA (AHI >= 30) was identical for both questionnaires.CONCLUSION: The STOP-BANG questionnaire has an excellent sensitivity, but modest specificity and adding national obesity indexes for neck circumference achieved similar results in terms of sensitivity and specificity with the original questionnaire.Publication Did the smokers level of knowledge on copd changed in the last decade?
