Item 65 yaş ve üzeri ürolojik cerrahi geçiren hastalarda üç farklı kırılganlık ölçeğinin postoperatif sonuçları öngörme yeteneğinin değerlendirilmesi(Bursa Uludağ Üniversitesi, 2023) Yıldız, Simge; Gören, Suna; Bursa Uludağ Üniversitesi/Tıp Fakültesi/Anesteziyoloji ve Reanimasyon Anabilim Dalı.Çalışmamızda ürolojik cerrahi geçiren 65 yaş ve üzeri hastalarda kırılganlığın FRAIL Ölçeği (FRAIL), Edmonton Kırılganlık Ölçeği (EKÖ) ve Klinik Kırılganlık Skalası (KKS) ile belirlenmesi, postoperatif 30 günlük dönemde olumsuz sonuçlarla ilişkisinin araştırılması ve ölçeklerin bu konulardaki tahmin güçlerinin kıyaslanması amaçlanmıştır. Ürolojik cerrahi geçiren ≥65 yaşında 234 hastada gerçekleştirilen prospektif çalışmada preoperatif dönemde sosyo-demografik verileri, son bir yıl içerisindeki acil servis başvuruları, düşme sayıları, Amerikan Anestezistler Derneği Fiziksel Durum Sınıflaması (ASA), Charlson Komorbidite İndeksi (CKİ), albümin, hemoglobin, hematokrit değerleri, kırılganlık ölçek puanları kaydedilmiştir. Hastaların postoperatif 30 gün boyunca komplikasyonlar, yoğun bakım yatışı, yoğun bakımda ve klinikte yatış süresi, acil servis başvurusu takip edilmiştir. Çalışmamızda kırılganlığın (sırasıyla FRAIL, EKÖ ve KKS) postoperatif komplikasyon görülme ihtimalini arttırdığı gösterilmiştir (sırasıyla p<0,001, OR:10,214; p<0,001, OR:3,663; p<0,001, OR:5,029). ASA ve kırılganlık ölçeklerinin postoperatif komplikasyonları öngörme yetenekleri benzerdir (ASA AUC:0,611, p=0,002 ve sırasıyla AUC:0,665, p<0,001; AUC:0,606, p=0,014; AUC:0,640, p=0,003). Yoğun bakım ihtiyacı olan hastaların ASA skorlarının daha yüksek olduğu (p=0,001) ve üç ölçeğe göre de daha kırılgan olduğu saptanmıştır (sırasıyla p<0,001; p=0,010; p<0,001). ASA ve kırılganlık ölçek puanları ile postoperatif yoğun bakım yatış süresi arasında anlamlı bir korelasyon gösterilememiştir. ASA skoru daha yüksek olan (p=0,001); FRAIL (p=0,004) ve KKS'ye (p=0,022) göre kırılgan olan hastaların klinik yatış süreleri daha uzun iken EKÖ'ye (p=0,070) göre kırılgan olan hastalarla klinik yatış süresi arasında anlamlı bir ilişki saptanmamıştır. EKÖ (AUC:0,702, p<0,001) ve KKS (AUC:0,635, p=0,008) postoperatif acil servis başvurusunu öngörmede anlamlı ölçeklerdir, ancak EKÖ'nün prediktif yeteneği KKS'ye daha üstün bulunmuştur (p=0,045). Çalışmamızda kırılganlığın ürolojik cerrahi hastalarında postoperatif olumsuz sonuçlarla (komplikasyonlar, yoğun bakım yatışı, klinik yatış süresinde uzama, acil servis başvurusu) ilişkili olduğu gösterilmiştir. Postoperatif komplikasyonları öngörmede FRAIL, postoperatif acil servis başvurularını öngörmede EKÖ öne çıkmaktadır. As an alternative to the standard surgical method, percutaneous techniques are available. Seventy-two patients were electively selected for percutaneous tracheostomy (PCT) in a nine-bed combined medical-surgical intensive care unit. PCT was performed at bedside with the Porter Percutaneous Tracheostomy Kit that uses the Griggs technique. The procedure time and early complications were recorded. The procedure was successful in all patients. The average duration of placement was 7.4 min. There were no tracheostomy-related deaths. Major bleeding occurred in three patients and required surgical intervention. In one patient, minor bleeding occurred at the stoma site that resolved with applied pressure. Wound infections were treated with local antiseptics in two patients. These findings suggest that PCT is a simple, quick and safe procedure.Item Çarpan kalpte koroner arter bypass cerrahisinde farklı anestezik ilaç kombinasyonlarının karşılaştırılması: hemodinami, oksijenasyon ve derlenme profiline etkileri(Uludağ Üniversitesi, 2005-06-13) Kaya, Fatma Nur; Gören, Suna; Şahin, Şükran; Korfalı, Gülsen; Canbulat, Atilla; Uludağ Üniversitesi/Tıp Fakültesi/Anesteziyoloji ve Reanimasyon Anabilim Dalı.Çarpan kalpte koroner arter bypass (ÇKKAB) cerrahisinde farklı anestezik ilaç kombinasyonlarının hemodinami, oksijenasyon ve derlenme profiline etkilerini karşılaştırmayı amaçladık. ÇKKAB cerrahisi planlanan, 60 olgu (EF>%40) rastgele beş gruba ayrıldı (n=12). Grup I’de anestezi indüksiyonu ketamin (2 mg/kg) ve fentanil (5 µg/kg), idamesi izofluran (%0.5-1.2) ile sağlandı. Grup II ve V’de indüksiyonda etomidat (0.3 mg/kg) ve fentanil (5 µg/kg), idamede Grup II’de izofluran (%0.5-1.2), Grup V’de ise sevofluran (%0.8-2.5) kullanıldı. Grup III ve IV’de indüksiyonda midazolam (0.15 mg/kg) ve fentanil (5-7 µg/kg), idamede Grup IV’de izofluran (%0.5-1.2), Grup III’de midazolam infüzyonu (0.1 µg/kg/dk) verildi. Grup I, II ve V’de indüksiyon öncesi sedasyon amacıyla midazolam, bölünmüş dozlar halinde (0.1 mg/kg’e kadar) uygulanırken, Grup III ve IV’de 1-3 mg verildi. Tüm gruplarda idamede fentanil infüzyonu (2-5 µg/kg/sa) uygulandı. Hemodinamik parametreler, oksijen tüketimi ve şant fraksiyonu farklı dönemlerde kaydedildi. Spontan solunum, ekstübasyon, mobilizasyon ve yoğun bakımdan çıkış için geçen süreler kaydedildi. İndüksiyon sonrası kardiyak debideki azalma, Grup I ve IV’e göre Grup II’de daha az (p<0.01 ve p<0.05), diğer parametreler ise benzer bulundu. Sonuç olarak, ÇKKAB cerrahisinde farklı anestezik ilaç kombinasyonları ile benzer hemodinamik ve postoperatif derlenme özellikleri gözlenmesine rağmen, anestezi indüksiyonu sonrası gözlenen kardiyak debi değişiklikleri dikkate alındığında, ventrikül fonksiyonları korunmuş kardiyak olgularda, etomidat, fentanil ve izofluran kombinasyonunun iyi bir seçim olacağı kanısındayız.Item Combination of a psoas compartment, sciatic nerve, and T12-L1 paravertebral blocks for femoropopliteal bypass surgery in a high-risk patient(W. Design: Prospective, randomized, controlled, blinded clinical study. Setting: University hospital. Participants: Forty-six patients who underwent elective off-pump coronary artery bypass grafting. Interventions: Patients were randomly assigned to receive remifentanil infusion alone (control group, n=23) or remifentanil infusion plus 10 mu g/kg of intrathecal morphine (ITM group, n=23). Induction and maintenance anesthesia were the same in both groups. Maintenance therapy was remifentanil infusion (0.25-1 mu g/kg/min) and 0.5% to 1.5% isoflurane, with adjustments according to hemodynamics. After extubation, intravenous patient-controlled analgesia with morphine (1-mg bolus and 5-minute lockout) was administered, and Wilson sedation scores, visual analog pain scores (scale, 0-100 mm) at rest and during coughing, and cumulative morphine consumption were assessed at 1, 2, 4, 8,12,24, and 48 hours. Examiners were unaware of patients' group identities. Anesthetic recovery parameters and opioid-related, spinal anesthesia-related, and cardiac complications were recorded. Measurements and Main Results: There were no differences between the groups' intraoperative hemodynamic or anesthetic recovery findings. Pain scores and morphine consumption were significantly lower in the ITM group at all time points after extubation (p=0.0001-0.05). Group frequencies of opioid-related and cardiac complications were similar. No patient had central neuroaxial hematoma or post-spinal tap headache. Conclusion: In the setting of isoflurane anesthesia for off-pump coronary artery bypass grafting, ITM combined with remifentanil infusion provides better postoperative analgesia than does remifentanil infusion alone, and does not improve or negatively affect fast-tracking.Item Comparison of analgesic effects of morphine, fentanyl, and remifentanil with intravenous patient-controlled analgesia after cardiac surgery(W.B. Design: Prospective, randomized, and double-blinded study. Settings: This study was performed at Uludag University hospital. Participants: Seventy-five patients undergoing off-pump coronary artery bypass surgery were included in the study. Interventions: Anesthesia was standardized. Cases were randomized into 3 groups consisting of 25 patients in each. Groups M, F, and R were given morphine HCl (1 mg/mL) with an infusion rate of 0.3 mg/h and 1-mg bolus doses; fentanyl (50 mug/mL.) with an infusion rate of 1 mug/kg/h and 10-mug bolus; and, remifentanil (50 mug/mL) with an infusion rate of 0.05 mug/kg/min and 0.5-mug/kg bolus, respectively. Continuous infusion was started immediately after the completion of the surgery. Measurements and Main Results: Pain was assessed by using a visual analog scale (0-10), and sedation was assessed with the Ramsey sedation score (1-6) 30 minutes, 1, 2, 4, 12, and 24 hours after extubation. The number of boluses and demands, time to extubation, and side effects were analyzed. Visual analog scale, sedation scores, and mean extubation times were similar in all groups. Total number of boluses and demands were statistically more in the remifentanil group. Regarding the side effects, nausea and vomiting was higher in group M (p < 0.05), whereas itching was prominent in group F (p < 0.05). Design: Prospective, randomized, double-blind, clinical study. Setting: Single university hospital. Participants: Forty patients who underwent elective muscle-sparing thoracotomy. Interventions: Before anesthesia induction, an epidural catheter was placed in the L2-3 or L3-4 interspace using the loss-of-resistance technique. On arrival at the intensive care unit, patients were randomized to receive doses of either 100 mg of tramadol (group T) or 4 mg of morphine (group M) via the lumbar epidural catheter. Each dose was diluted in 10 mL of normal saline. On awakening from anesthesia, if the patient's pain score on a 0- to 100-mm visual analog scale was above 40 mm, the initial epidural drug dose was administered. The initial injection in each case was taken as time 0. Subsequent pain scores above 40 mm were considered indications for epidural dosing; each patient was allowed 2 doses in the first 12 hours postoperatively and 2 more in the second 12 hours. Measurements and Main Results: The groups' analgesia onset times were similar, but duration of analgesia was significantly shorter in group T than in group M (p < 0.01). There were no differences between the groups with respect to pain scores at rest or during coughing at any of the time points investigated. Sedation scores were lower in group T than in group M at 1, 2, 3, 4, and 8 hours (p value range, 0.0001-0.05). Compared with group T, group M showed significantly greater drops in arterial oxygen tension from baseline at 3, 4, 8, and 12 hours (p value range, 0.0001-0.05). The group means for arterial carbon dioxide tension and respiratory rate were similar at all time points investigated. Conclusion: The study revealed that the quality of analgesia achieved with repeated doses of lumbar epidural tramadol after muscle-sparing thoracotomy is comparable to that achieved with repeated doses of lumbar epidural morphine. Despite the theoretical advantage ofrequiring smaller amounts of drug to provide analgesia solely via thoracic dermatomes, the superiority of thoracic epidural analgesiaover lumbar epidural analgesia has been called into question. In the meta-analysis of 8 studies comparing thoracic versus lumbarepidural opioid administration, Ballantyne and associates1found no differences in pain scores, pulmonary function, and theincidence of side effects. As a result of limited evidence confirming the benefits of thoracic versus lumbar epidural analgesia, someauthors have expressed caution in routinely using thoracic epidural analgesia.2-4Furthermore, most anesthesiologists are morecomfortable placing lumbar epidural catheters because these catheters are placed below the conus medullaris, avoiding the risk ofinjuring the spinal cord if the dura is inadvertently punctured during thoracic epidural placement. Furthermore, thoracic epiduralcatheter placement can be technically more difficult because of the greater caudad angulation of the spinous processes.Immediately after awakening from anesthesia, all patients complained of pain, with a visual analog scale score higher than 40mm at rest, and received the first dose of study drug via epidural catheter. Therefore, we did not discuss analgesia for the patientswho scored less than 40 mm in this period. In 20 of the 75 patients we used only an intraparenchymal fiberoptic ICP monitoring transducer, while, in the remaining 55 patients, who required CSF drainage, a ventricular drainage set (VDS) was used in addition to ICP monitoring. The duration of monitoring with the ICP transducer alone was approximately 5.1 +/- 2.6 das (min. 1, max. 13) and that of ICP monitoring with VDS was 6.2 +/- 3.1 days (min. 1, max. 13). In 8 cases a total of 9 complications were experienced (12 %). These complications were infection in 3 cases (4 %), epidural hematoma in 2 cases (2.7 %): disconnection in 2 cases (2.7 %) and contusion in 2 cases (2.7 %). Although none of the 44 patients who were monitored for less than 5 days experienced infection, 3 of the 31 patients monitored for longer than 5 days did experience infection (9.7 %) (p < 0.05). None of the 20 patients who underwent ICP monitoring only experienced infection. However, 3 of the 55 patients in whom the ventricular drainage set was implanted in addition to the transducer for ICP monitoring experienced infection (p < 0.05). Preoperatively, the mean serum free and phospholipid-bound choline concentrations in patients scheduled for abdominal hysterectomy (n=26), off-pump coronary artery grafting surgery (n=34) or brain tumor surgery (n=24) were 12.3+/-0.5, 12.1+/-0.4 and 11.4+/-0.4 mumol/l, and 2495+/-75, 2590+/-115 and 2625+/-80 mumol/l, respectively. Serum free choline and phospholipid-bound choline concentrations decreased from these baseline values to 8.8+/-0.7 (p<0.001), 8.8 +/- 0.5 (p<0.001) and 8.2+/-0.4 mumol/l (p<0.001), and 2050 +/- 108 (p<0.001), 2166+/-59 (p<0.001) and 1884 +/- 104 mu mol/l (p<0.001) at 1 h after hysterectomy, off-pump bypass graft surgery or brain tumor surgery, respectively. They remained at these low levels for 24 h and then gradually increased towards the preoperative values at 48-72 h postoperatively. Serum cortisol increased postoperatively in all surgical patients for 24 h and its levels were inversely correlated with serum free and bound choline concentrations. Bu amaçla Bupivakain ve yüksek lipid solubilitesi ve hızlı doku alınımına sahip bir opiat olan Fentanil ile hazırladığımız solüsyonla torakotomi sonrası ağrıyı durdurmaya çalıştık. Çeşitli nedenlerle torakotomi uygulanan 16-72 yaşları arasında 5 kadın, 28 erkek toplam 33 hastaya T10 L4 aralıklar arasından epidural kateter yerleştirildi. Operasyon sonunda 10 ml. bolus (Bupivakain % 0.5 2 ml + fentanil 0.05 mg + SF) dozunu takiben 1 mg/ml Bupivakain + 5 ug/ml Fentanil solüsyonu ile infüzyona başlandı. İnfüzyon hızı hastanın gereksinimine göre gerektikçe arttırılıp, azaltıldı. Hastalar kateter çıkarılıncaya kadar yoğun bakımda VAS (Görsel Analog Skorlama) ile ağrının kontrolü bakımından izlendiler, torakostomi tüpü çıkarıldıktan sonra epidural kateter çekildi. Olgularımızın hepsinde torakotomiye bağlı ağrı oldukça etkin olarak giderildi. Hiç bir hastada istenmeyen etki görülmedi. Epidural kateter en az 3, en fazla 13 gün yerinde kaldı. Posttorakotomi ağrılarının kontrolünde bu yöntemin etkin, emniyetli ve kolay bir yol olduğu sonucuna varıldı.Item Epidural narkotik ve lokal anestezik kombinasyonu ile postoperatif ağrı tedavisi(Uludağ Üniversitesi, 1992) Gören, Suna; Uludağ Üniversitesi/Tıp Fakültesi/Anesteziyoloji ve Reanimasyon Anabilim Dalı.Çalışmamızda, Jinekolojik Calt abdoMİnal) ve ortopedik Calt ekstrenite) girişin yapılan, ASA I-II grubuna giren 30 olgu, 10 kişilik üç eşit gruba ayrıldı. Postoperatif ağrı tedavisi anacıyla epidural yolla uygulanan Bupivakain-Morfin bolus, Bupivakain-Morfin infüzyonu ve Bupivakain-Fentani 1 infüzyonunun etkileri karşılaştırıldı. Preoperatif donende yerleştirilen epidural kataterden bu anaçla 1. gruba 3 ng Morfin + 30 ng Bupivakain Ctotal 10 cc) ile gereğinde tekrarlanan bolus tedavisi, 2. gruba 2 ng Morfin + 20 ng Bupivakain Ctotal 10 cc3 bolus dozunu takiben X 0.125'lik Bupivakain + 7, 0.01'lik Morfin ile 2-6 cc/st hızla infüzyon tedavisi ve 3. gruba 50 jug Fentanil + 20 ng Bupivakain (total 10 cc) bolus dozunu takiben 7. 0.125 Bupivakain + 7. 0.005 Fentanil 4-8 cc/st hızla infüzyonu uygulandı. Preoperatif ve postoperatif çeşitli döneMİende VAS, NDS, sistolik ve diastolik kan basıncı, solunum sayısı, pH, pC02, p02, Sa02, FVC ve FEVİ değerleri ile gözlenen yan etkiler kaydedildi. VAS değerlerine göre her 3 grupta da etkin bir postoperatif analjezi elde edildi. Ancak 1. grupta CBMB) verilen ilacın etkisi geçtikten sonra VAS'daki yükselme, istatistiksel açıdan anlamsız olmakla birlikte daha fazla idi. 1.grupta CBMB) solunum sayısında 8. ve 24. saatlerde % 10' Iuk düşüşler ile FVC de postoperatif 1. ve 2. günlerdeki % 17.7 ve % 11'lik düşüşler anlamlı idi. 2. grupta CBM1J, sistolik kan basıncında 2, 4, 12 ve 16. saatlerde önemli azalmalar gözlendi. 3. grupta ise solunum sayısının 4. ve 24. saatlerde, Sa02'nun 12, saatte artMası dışında belirgin bir farklılık saptanmadı. Bu farklılıklar klinik olarak herhangi bir tedavi gerektirecek düzeyde değildi. NDS, diastolik kan basıncı, pH, pCO2, p02, ve FEVİ değerlerinde gruplar arasındaki değişiklikler önemli bulunmadı. 3. grupta yan etkilerin diğer gruplara oranla daha az olduğu gözlendi. Hastane etik kurul onayı sonrasında, Aralık 2014-Eylül 2015 tarihleri arasında Anestezi Polikliniği'ne başvuran hastalardan preoperatif anket formunu doldurmaları istendi. Postoperatif dönemde takip edildikleri klinikte ziyaret edilerek 2. anket formunu doldurmaları, bilgi düzeylerindeki değişikliği değerlendirmek amacıyla preoperatif formdaki 5-17. soruları tekrar cevaplandırmaları istendi. Çalışmaya ≥18 yaş, okuma-yazma bilen, yoğun bakım takibi planlanmayan, elektif cerrahi geçirecek hastalar dahil edildi. Preoperatif dönemde, anestezi polikliniğinin işlevini bilme ve bilgilendirmenin endişeleri azaltacağı düşüncesi; postoperatif dönemde, korku ve endişelerimde azalma oldu cevabı ile anestezistin adını bilme insidansı yüksektir (p<0,001, p=0,010, p=0,006, p=0,012). Preoperatif dönem ile karşılaştırıldığında anestezi uygulama şekilleri, peroperatif hasta takibi, derlenme, preoperatif açlık süreci ve ilaç kullanımı ile ilgili bilgi düzeylerinin arttığı bulunmuştur (p=0,011, p=0,011, p=0,001, p<0,001, p=0,009). Anestezistin ameliyathane dışındaki çalışma alanları ve preoperatif açlık süresi ile ilgili bilgi düzeylerinde azalma izlenirken, ameliyat süresince yapılanlar ile ilgili bir değişiklik izlenmemiştir (p<0,001, p=0,015, p=0,927). Olumlu sonuçların varlığına rağmen çalışmamız, toplumun büyük bir kesiminde bu uzmanlık alanının varlığı, görev tanımı, yetkinlik ve yeterliliği konularında bilgi eksikliği olduğunu, günümüzde hastaların hala anestezist ve anestezi süreci ile ilgili bilgilendirmeye ihtiyaç duyduklarını göstermektedir. INTRODUCTION: Anesthetic induction and maintenance may induce myocardial ischemia in patients with coronary artery disease. A primary goal in the anesthesia of patients undergoing coronary artery bypass grafting surgery is both the attenuation of sympathetic responses to noxious stimuli and the prevention of hypotension. METHODS: Thirty patients undergoing coronary artery bypass grafting surgery were randomized to receive either ketamine 2 mg.kg(-1) (Group K) or propofol 0.5 mg.kg(-1) (Group P) during induction of anesthesia. Patients also received standardized doses of midazolam, fentanyl, and rocuronium in the induction sequence. The duration of anesthesia from induction to skin incision and sternotomy, as well as the supplemental doses of fentanyl and sevoflurane, were recorded. Heart rate, mean arterial pressure, central venous pressure, pulmonary arterial pressure, pulmonary capillary wedge pressure, cardiac index, systemic and pulmonary vascular resistance indices, stroke work index, and left and right ventricular stroke work indices were obtained before induction of anesthesia; one minute after induction; one, three, five, and ten minutes after intubation; one minute after skin incision; and at one minute after sternotomy. RESULTS: There were significant changes in the measured and calculated hemodynamic variables when compared to their values before induction. One minute after induction, mean arterial pressure and the systemic vascular resistance index decreased significantly in group P (p<0.01). CONCLUSION: There were no differences between groups in the consumption of sevoflurane or in the use of additional fentanyl. The combination of ketamine, midazolam, and fentanyl for the induction of anesthesia provided better hemodynamic stability during induction and until the end of sternotomy in patients undergoing coronary artery bypass grafting surgery.Item İntravenöz anesteziklerin sistemik vasküler rezistans üzerine etkileri(Uludağ Üniversitesi, 2000) Gören, Suna; Moğol, Elif; Şahin, Şükran; Korfalı, Gülsen; Özsaraç, Özer; Cevheroğlu, Demet; Uludağ Üniversitesi/Tıp Fakültesi/Anesteziyoloji ve Reanimasyon Anabilim Dalı.Elektif koroner arter bypass cerrahisi uygulanacak 62 olguda tiyopental sodyum (Grup T; n=20), propofol (Gıup P; n=22) ve etomidatın (Grup E; n=20), hipotermik kardiyopulmoner bypass sırasında perfüzyon akımı sabit iken perfüzyon basıncı ve sistemik vasküler rezistans (SVR) olan etkilerini araştırdık. 10 dakikalık takip süresince ortalama arter basıncı (MAP) ve rezervuar volümünde meydana gelen maksimum değişiklikler kaydedildi. SVR hesaplandı. Grup P de daha fazla olmak üzere, her 3 grupta da MAP da anlamlı düşme saptanmıştır (Grup T, Grup P, Grup E'de sırasıyla p<0.05, p<0.001, p<0.05). Fakat SVR da her üç grup arasında anlamlı bir değişiklik gözlenmemiştir. Sonuç olarak, KPB sırasında propofol grubunda daha belirgin olmak üzere her üç iv anestezik ajan perfüzyon basıncı ve SVR'da benzer değişiklikler meydana getirmiştir.Item İntravenöz parasetamolün torakoskopik cerrahi sonrası postoperatif analjezi üzerine etkinliği(Uludağ Üniversitesi, 2010-05-07) Çınar, Vildan; Kaya, Fatma Nur; Yavaşçaoğlu, Belgin; Gören, Suna; Bayram, Sami; Moğol, Elif Başağan; Kaya, Atilla; Uludağ Üniversitesi/Tıp Fakültesi/Anesteziyoloji ve Reanimasyon Anabilim Dalı.; Uludağ Üniversitesi/Tıp Fakültesi/Göğüs Cerrahisi Anabilim Dalı.Çalışmamızda, torakoskopik cerrahi sonrası intravenöz parasetamol uygulamasının postoperatif analjezi üzerine etkinliğini değerlendirmeyi amaçladık. ASA I-II grubu 40 hasta, anestezi indüksiyonundan önce ve postoperatif 24 saat içinde her 6 saatte bir intravenöz 1 gr parasetamol veya 100 ml serum fizyolojik verilmek üzere randomize olarak ayrıldı. Dinlenme ve hareket sırasındaki ağrı skorları, sedasyon skorları, hemodinamik parametreler, morfin kullanımı, hasta/hemşire memnuniyeti ve yan etkiler değerlendirildi. İlk analjezik gereksinim zamanı, ilk mobilizasyon ve hastaneden çıkış zamanları kaydedildi. Parasetamol grubunda; morfin tüketimi postoperatif 2., 4., 8., 12. ve 24. saatlerde daha düşük (p=0.017, p=0.021, p<0.001, p<0.001, p<0.001), bulantı ve kusma sıklığı daha az (p=0.001, p<0.001); ilk analjezi gereksinim zamanı daha uzun (p=0.0032), ilk mobilizasyon ve hastaneden çıkış süreleri daha kısa bulundu (p=0.0031, p=0.0015). Analjezi açısından hasta/hemşire memnuniyeti anlamlı olarak fazlaydı (p<0.001, p<0.001). Sonuç olarak, parasetamol kullanımı daha az yan etki ile postoperatif morfin gereksinimini azalttı ve hasta/hemşire memnuniyetini artırdı. Bu yaklaşım torakoskopik cerrahi sonrası erken postoperatif mobilizasyona da katkıda bulunabilir.
