    Road traffic collisions in Bursa, Turkey, during 2003, 2004 and 2005

    Aim: To highlight the demographic pattern of road traffic injuries in Bursa and to improve emergency care and health facility-based treatment.Methods: Records of all registered road traffic collisions maintained by the Uludag University Emergency Department in Bursa for the period 2003 to 2005 and trauma files of 1307 road traffic collision victims were examined in this study. A form was designed to document patients' age, gender, type of trauma, time of trauma (hour, day, month and year), type and time to hospital of transport, presence of safety devices, presence of alcohol and blood alcohol concentration, localisations of wounds, trauma scores and outcomes.Results: Of the 1307 cases, 418 (32%) were female and 889 (68%) were mate. Ages ranged from 3 months to 87 years; mean age (+/-S.D.) was 34.8 (+/-17.3) years. The mean (+/-S.D.) ages of women and men were 35 +/- 17.6 and 34.7 +/- 17.2 years, respectively. Trauma was most commonly sustained within the motor vehicle (72.2%), followed by pedestrian injuries (21.7%), motorcycle injuries (5.5%) and bicycle injuries (0.6%). Collisions were most frequent in summer (34.7%) and on Fridays and weekends (48.5%). Seat belts were used in only 1.8% of incidents. In 90 cases (6.9%) alcohol was found in the blood; mean blood alcohol concentration was 139.2 +/- 88.3 mg/dl.Conclusions: The identification of RTC characteristics may contribute to the development of injury prevention measures. Road travel requires extra attentiveness at peak accident times. Seat-belt use should increase, as well as use of other safety equipment such as collision helmets. Shorter transportation time of casualties to hospital would improve outcome. (C) 2007 Elsevier Ltd. All rights reserved.

    Protective role of intracoronary shunt in off-pump coronary bypass operations

    Objective: To investigate if there are any advantages in using intracoronary shunts compared to shuntless operations, in the context of whether it has a protective role for the myocardia.Methods: This prospective study, included 100 patients who underwent off-pump coronary bypass surgery at 2 different cardiovascular surgery departments, namely, the Social Security Ankara Ihtisas Hospital, and Hacettepe University Hospital, Turkey, between September 2002 and July 2006. Patients were divided into 2 groups. In group 1 (n=50) off-pump coronary bypass operations were performed with intracoronary shunts. In group 2 (n=50) shunts were not used during off-pump. Serum creatine kinase, myoglobin, and troponin were studied.Results: There were significant increases in serum creatine kinase levels in group 2 at postoperative 6th, 12th, and 24th hours. In group 2, the increase of myoglobin was statistically significant at only the postoperative 24th hour. Troponin levels were significantly higher in group 2 at postoperative 6th, 12th, and 24th hours.Conclusion: There are some questions regarding myocardial protection while maintaining a bloodless secure surgical field in off-pump coronary surgery. However, use of intracoronary shunts provides distal coronary flow, and reduces the risk of myocardial ischemia, while maintaining a comfortable blood free anastomosis area.

    The performance of some walnut (juglans regia) cultivars in the conditions of Bursa, Turkey

    The present study aimed to evaluate the pomological and phenological traits, as well as yield potential and incidence of external defects of some local ('Bilecik', 'Mara 12', 'Mara 18', 'Sebin', 'Sen 1') and foreign ('Chandler', 'Fernette', 'Fernor', 'Howard', 'Pedro', 'Serr') walnut cultivars grown in Bursa (north-west in Turkey) conditions between 2014 and 2020. The first leafing was observed in Mara 12, Serr, Sebin, and Mara 18 cultivars, while Fernette and Fernor were the last leafing cultivars. The female blooming period of the cultivars was recorded to be the longest in Mara 18 (15 days) and Sebin (16 days) and that was the shortest in Bilecik and Serr (10 days). The male blooming period was recorded as the longest in Chandler, Fernette, Bilecik, and Sebin (16-18 days) and that was the shortest in Mara 12 and Serr (10 days). The average yield per tree of cultivars ranged between 1.87 kg (Sebin) and 14.21 kg (Pedro). Walnut blight was observed on average in 15.85%, sunburn in 10.21%, codling moth in 5.11%, anthracnose in 3.23%, and bird damage in 2.35% of the fruits. The healthy nut percentage ranged from 40.06% (Sen 1) to 76.50% (Chandler). The nut weight of cultivars was between 9.91 g (Mara 12) and 16.27 g (Sen 1). Kernel percentage varied from 36.31% (Sen 1) to 53.56% (Mara 12). The principal component analysis conducted to distinguish the cultivars suggests that the most important factors could be reduced to five components. The results showed that Mara 12, Chandler, Fernor, and Howard were suitable cultivars for commercial cultivation in Bursa conditions.

    Ruptured baker's cycst mimicking the symptoms of deep vein thrombosis in a rheumatoid arthritis patient

    A 50-year-old male patient who had a diagnosis of rheumatoid arthritis was admitted to our clinic with the complaint of sudden onset of severe pain and swelling in his right knee and calf. An initial diagnosis of deep vein thrombosis was made, however, ultrasound imaging revealed bilateral popliteal Baker's cysts. The cyst on the right side was ruptured. Both of the cysts were aspirated. Concomitant occurrence of rheumatoid arthritis and Baker's cyst which could be confused with deep venous thrombosis was discussed in this case presentation.

    The regulatory effect of cabazitaxel on epithelial-mesenchymal transition in metastatic prostate cancer

    Introduction:Epithelial-mesenchymal transition (EMT) is a critical mechanism that promotes cancer cells to metastasis. Therefore, EMT regulation has become an important target in anticancer therapy approaches in recent years. However, in metastatic prostate cancer (PC), the EMT regulatory effect has not fully understood for cabazitaxel (Cbx), a third line taxane-based chemotherapeutic for metastatic castration-resistant PC.Aim:In this study, we investigated the antimetastatic and EMT-regulatory effects of Cbx on hormone-sensitive metastatic PC cells.Materials and Methods:The anticancer effects of Cbx were assessed by WST-1 and Annexin V analysis. The antimetastatic effect of Cbx was evaluated by wound healing and quantitative reverse transcription polymerase chain reaction through EMT-mesenchymal-to-epithelial transition (MET) markers as well as EMT-repressor microRNAs (miRNAs) in Cbx-treated LNCaP cells.Results:Our results showed that, in addition to its apoptotic and anti-migratory activities, Cbx exhibited the EMT-repressor effects through the prominent downregulation of matrix metalloproteinase-9 and Snail levels as EMT-promoting factors, and the significant upregulation of the certain miRNAs, including miR-205, miR-524, and miR-124, which play a role in EMT-repressing by targeting regulators of the EMT-associated genes.Conclusion:Although further evaluations are needed to improve the findings, we showed that, in addition to its classical taxane function, Cbx has a regulatory effect on EMT-MET cycling in hormone-sensitive metastatic PC.

    The role of femoral head size and femoral head coverage in dogs with and without hip dysplasia

    Simple Summary Canine hip dysplasia is a nonhealing developmental orthopedic disorder resulting in osteoarthrosis of the hip joints and lameness. Radiography is an important tool to diagnose, grade and assess prognosis in hip dysplasia. The purpose of this study was to investigate radiographically detectable and measurable parameters that could indicate a predisposition to hip dysplasia. Radiographs of 264 dogs presented for canine hip dysplasia screening were evaluated for femoral head size, coverage of the femoral head by the acetabulum and acetabular length in relation to dysplasia status according to the Federation Cynologique Internationale. No significant relationship between femoral head area and Federation Cynologique Internationale assessment was detected. Femoral head area was breed-specific and larger in non-dysplastic dogs, males and German wirehaired pointers. Coverage of the femoral head by the acetabulum was significantly affected by presence of dysplasia and breed. All breeds and both sexes showed strong positive correlations between femoral head area and acetabular length. The subject of hip dysplasia in dogs is still current and preoccupies both animal owners and veterinarians. Major factors affecting the development of the disorder are hip laxity and incongruent joints. Many studies on etiology, pathogenesis, and early diagnosis have been performed to reduce prevalence and select healthy dogs for breeding. The purpose of the present study was to investigate a possible relationship between dysplasia and femoral head area (FHA), femoral coverage by the acetabulum (CFH) and cranio-caudal distance of the dorsal acetabular rim (CrCdAR). Radiographs of a total of 264 skeletally mature dogs with similar physical characteristics (German wirehaired pointers (GWP), German shepherd dogs (GSD) and Labrador retrievers (LAB)) presented for routine hip dysplasia screening were recruited for the study. FHA, CFH and CrCdAR were measured and related to dysplasia status. Evaluations of FHA (p = 0.011), CFH (p < 0.001) and CrCdAR length (p = 0.003) measurements revealed significant interactions between breed, sex and FCI scores, so they had to be assessed separately. The results revealed that FHA tends to decrease as the hip dysplasia score worsens. There was no significant relationship between FHA and dysplasia assessment. FHA is breed-specific and is larger in normal and near-normal male (p = 0.001, p = 0.020) and female (p = 0.001, p = 0.013) GWP compared to GSD, respectively. FHA is greater in normal male GWP (p = 0.011) and GSD (p = 0.040) compared to females. There was a significant and strong positive correlation between FHA and CrCdAR in all breeds and sexes. Additionally, FCI scoring had a medium (GWP, GSD) to strong (LAB) negative correlation with CFH.

    The use of acellular matrices obtained by the esophagus, intestine, and trachea for esophageal wall repair: An experimental study on a rat model

    This study aimed to evaluate the procurement, efficacy, and applications of different acellular matrices (AMs) on rat esophagi. Sixteen rats were used. Following tissue harvesting from donor rats, AMs were obtained by detergent-enzymatic treatment (DET) from the esophagus, jejunum, and trachea. Fifteen rats were allocated into groups defined by implemented AM types: esophageal (EAM), intestinal (IAM), and tracheal (TAM). According to groups, 5 x 5 mm esophageal wall defect repair was performed with AMs. Samples were examined for hyperemia, edema, inflammatory cells, and neovascularization by scoring. The existence of muscle cells was investigated with immunohistochemical assessment. The procurement of AM in vitro and its implementation on the damaged esophageal wall was achieved successfully. There was a significant difference between groups regarding hyperemia, inflammatory cells, and neovascularization (p < 0.05). Hyperemia and cell infiltration were higher, and neovascularization was poorer in TAM than in others (p < 0.05). The neovascularization was significantly higher in the EAM group (p < 0.05). Increased edema was detected in the IAM group than others, albeit with no significant difference. Staining with Masson's trichrome and desmin antibodies, showing increased inflammatory response on the matrix site, was not detected to regenerate muscle cells. Our DET approaches provided that AMs with potential for regeneration have been obtained from different tissues. Grafting by TAM caused higher grades of inflammation with poor angiogenesis and existing chondrocytes. Decreased inflammatory response of EAM was thought to be due to the native tissue-derived matrix. These results suggest that promoting self-regeneration with native tissue-derived AM can be improved and more convenient in damaged tissue repair.

    The π-extending property via singular quotient submodules

    A module is said to be pi-extending provided that every projection invariant submodule is essential in a direct summand of the module. In this article, we focus on the class of modules having the pi-extending property by looking at the singularity of quotient submodules. By doing so, we provide counterexamples, using hypersurfaces in projective spaces over complex numbers, to show that being generalized pi-extending is not inherited by direct summands. Moreover, it is shown that the direct sums of generalized pi-extending modules are generalized pi-extending.

    The √3 rectangle in islamic geometric patterns

    In this research, eleven different six-fold Islamic geometric patterns were analyzed geometrically, and the geometric relationships between the patterns were revealed. In the context of the Girih mode, the contributions of the small pattern mold constructed by using the symmetry axes to the geometric analysis of the patterns are emphasized. Patterns were considered whole, and small pattern molds were found using axes of symmetry. We have demonstrated that the rectangles of the small pattern mold of the six-fold patterns are root 3 rectangle. This rectangle is one of the mathematical concepts used in Islamic geometric patterns.

    Thermodynamic analysis of basic and regenerative organic rankine cycles using dry fluids from waste heat recovery

    The organic Rankine cycle (ORC), which generates electric energy using low temperature heat sources, is a promising technology in energy production sector. The ORC, which uses an organic fluid with its lower boiling point and higher vapor pressure than water-steam as a working fluid. The thermal efficiency of an ORC showes the performance of system, depends on system compenents, working fluid and operating conditions. This paper presents an thermodynamics examination of basic ORC and regenerative ORC for waste heat recovery applications using dry organic fluids. R113, R114, R227ea, R245fa and R600a with the boiling points from -16 degrees C to 48 degrees C are selected in the analyses. The relationships between the ORC's performance parameters for basic and regenerative technologies and the properties of working fluids are evaluated based on various turbine inlet pressure values. Results show that regenerative ORC has higher thermal efficiency compared with basic ORC. Also, the thermal efficiency increases with the increment of the turbine inlet pressure for both basic ORC and regenerative ORC.

    Thermomechanical vibration analysis of a restrained nanobeam

    In this study, thermomechanical vibration analysis of a nanobeam model of boron nitride nanotube is performed. Displacement is allowed by placing elastic springs in the boundary conditions. The end displacements are represented by two different coefficients and the variable in the region is represented by an analytical Fourier sine function. The Stokes' transformation is used to force the boundary conditions to resemble the desired support shape. It is aimed in this study, a general eigenvalue problem that gives the thermal vibrational frequencies of a single-walled boron nitride nanotube under deformable boundary conditions is established. Unlike the studies with rigid boundary conditions found in the literature, in this study, there is no need to re-derive the system equations for each boundary condition change. The results are compared with similar results in the literature and excellent agreement is obtained. In addition, the effects of thermal load, elastic springs, small-scale parameters and elastic foundation are investigated and it is found that these effects have a negligible influences on the vibration frequencies of boron nitride nanotubes.

    Trends in attitudes of young people about to begin their studies at university: Research to develop a measuring instrument

    Objective: In our study we aimed to develop a scale to evaluate the effective factors in the choice of faculty by students who are about to start university within the scope of the "Wholistic Approach" one of the core competences recommended by WONCA.Methods: In the study 434 students completed a questionnaire. In the pilot study, we evaluated the form we had developed containing 25 attitude statements with a positive and negative direction under certain conditions. Before use, the questionnaire was evaluated for validity of content and scope. In order to investigate the structural validity of the questionnaire, it was then given to students who had gained places in various faculties of Uludag University in Bursa who were asked to mark the items according to their own preferences. The reliability and factor analysis of the scores was calculated using the SPSS statistical programme and the validity was evaluated.Results: After removing the items which reduced the reliability of the questionnaire, the general Cronbach-Alpha value of the remaining 18 items was 0.779 (corrected to 0.811). Upon factor analysis of these items, we obtained a scale containing 5 factors and 18 statements. We named this scale "The Goktas Faculty Orientation Scale." We determined that the factors of the scale were "Expectations of the Faculty", "Aims and Ideals", "Previous Schooling", "Family Status" and "Faculty Choices".Conclusion: As a result, we have developed a scale with high reliability which can be used in a wide range of future research on student orientation and attitudes.

    The colloboration of maurice!nicolle et adil!Mustafa -: The discovery of rinderpest agent

    The earliest relationship between Turkey and European countries in the veterinary field was initiated with the establishment of the first veterinary school in Turkey in 1842. Through the 19th and 20th centuries, many scientists, researchers and specialists from different part of Europe came to Turkey to contribute educational activities, institutionalization, organisation and legal restructuring of services related to veterinary medicine. One of the prominent results of these co-operations is seen in the researches of Dr. Nicolle, founder of the Imperial Institute of Bacteriology (1894), and Military Veterinarian Adil Mustafa, founder of the first Veterinary Bacteriology Institute in Turkey. They first succeeded in preparing therapeutically rinderpest serum and then discovered the rinderpest agent as filter-passing virus. This discovery, which was the success of joint effort of the, veterinary and medical sciences, as well as Turkish-French collaboration, made Nicolle's and Adil's names immortal in veterinary and science history. This review deals with the historical roots of veterinary bacteriology in Turkey and the international collaboration of Adil Mustafa and Maurice Nicolle, resulted the discovery of rinderpest agent.

    Introduction: Noninvasive ventilation (NIV) for acute hypercapnic respiratory failure (AHRF) is an established treatment modality. Current evidence does not conclude any superiority between fixed pressure support (PS) and aver-age volume-assured pressure support (AVAPS) modes. However, given the ability of rapid PaCO2 decline in AVAPS mode, we hypothesized that COPD patients with AHRF who did not show the desired reduction in PaCO2 with fixed-level PS-NIV might benefit from the AVAPS mode.Materials and Methods: Patients admitted to the non-ICU pulmonary ward with acute exacerbation of COPD (AECOPD) and AHRF were included con-secutively in this observational study. Patients with hypercapnic respiratory failure due to obesity-hypoventilation, neurological diseases, or chest wall deformities were excluded. All patients started NIV treatment with fixed pres-sure support (PS) and patients who did not reach clinical and laboratory stability under PS-NIV treatment were switched to the average volume -as-sured pressure support (AVAPS) mode of NIV.Results: Thirty-five COPD patients with hypercapnic respiratory failure were included. Under PS-NIV treatment, 14 (40%) patients showed a 17.9 (-0.0-29.2) percent change in terms of PaCO2 , meaning no improvement or worsening. Therefore, these patients were treated with AVAPS mode. Arterial PaCO2 and pH levels significantly improved after AVAPS-NIV administration. AVAPS-NIV treatment created a significantly better PaCO2 change rate than using PS-NIV (-11.4 (-22.0 --0.5)vs 8.2 (-5.3-19.5), p= 0.02]. Independent predictors of AVAPS mode requirement were higher Charlson Comorbidity Index (OR= 1.74 (95% CI= 1.02-2.97)] and higher PaCO2 upon admission (OR= 1.18 (95% CI= 1.03-1.35)]. Thirteen (92.8%) patients reaching signif-icant clinical stability with AVAPS-NIV were able to return to fixed-level PS-NIV and maintain acceptable PaCO2 levels.Conclusion: Our study demonstrated that patients can benefit from AVAPS-NIV despite insufficient response to fixed-level PS-NIV.

    Understanding the impact of pulmonary rehabilitation on airway resistance in patients with severe copd: A single-center retrospective study

    Purpose: We investigated the effect of pulmonary rehabilitation (PR) on airway resistance in chronic obstructive pulmonary disease (COPD) patients with severe airway obstruction and hyperinflation. Patients and Methods: This retrospective cohort study was conducted with data from severe COPD cases with those who underwent an 8-week PR program. Main inclusion criteria were having severe airflow obstruction (defined as a forced expiratory volume in one second (FEV1) <50%) and plethysmographic evaluation findings being compatible with hyperinflation supporting the diagnosis of emphysema (presence of hyperinflation defined as functional residual capacity ratio of residual volume to total lung capacity (RV/TLC) >120%). Primary outcomes were airway resistance (Raw) and airway conductance (Gaw) which were measured by body plethysmography, and other measurements were performed, including 6-minute walk test (6-MWT), modified Medical Research Council dyspnea scale (mMRC) and COPD assessment test (CAT).Results: Twenty-six severe and very severe COPD patients (FEV1, 35.0 +/- 13.1%; RV/TLC, 163.5 +/- 29.4) were included in the analyses, mean age 62.6 +/- 5.8 years and 88.5% males. Following rehabilitation, significant improvements in total specific airway resistance percentage (sRawtot%, p = 0.040) and total specific airway conductance percentage (sGawtot%; p = 0.010) were observed. The post-rehabilitation mMRC scores and CAT values were significantly decreased compared to baseline results (p < 0.001 and p < 0.001, respectively). Although there were significant improvements in 6-MWT value (p < 0.001), exercise desaturation (Delta SaO2, p = 0.026), the changes in measured lung capacity and volume values were not significant.Conclusion: We concluded that PR may have a positive effect on airway resistance and airway conductance in COPD patients with severe airflow obstruction.

    Usability and accessibility of urban service areas with increasing epidemics: The case of bursa/turkey the effect of urban service areas on the quality of life in the post-pandemic period

    The concept of urban life quality, which comes to the fore with urbanization and evaluates the effects of environmental variables on the well-being of the citizens in every respect, is directly related to the quality, quantity, and sufficiency of the urban service areas of the cities. In this study, cultural spaces, open spaces, and green areas in the central district of Bursa are classified as 12 types of urban service areas were examined. Within the framework of the method setup, the existence and distribution of urban services were evaluated within the scope of pre-and post-pandemic conditions, and solution suggestions were developed to increase the quality of life. In this context, it was determined that 47 out of 136 neighborhoods did not have urban service areas, and per capita values were found to be sufficient in only 10 neighborhoods. It is seen that these 10 neighborhoods have more recreational activity opportunities compared to other neighborhoods. The systematic method in the study was created in order to evaluate it as a plan base for physical planning studies in the study area and to shed light on the development of suggestions for increasing the quality of life for each city in general.

    Regulation of the acid inducible rcfB promoter in Lactococcus lactis subsp lactis

    Although Lactococcus is one of the most extensively studied lactic acid bacteria, little information is available on the regulation of the catabolite receptor protein (CRP) and acid induced gene expression. The rcfB gene, encoding RcfB protein which has unknown function in Lactococcus lactis IL1403, was analysed. Promoter region contains consensus CRP binding site and acid inducible sequence at upstream of start site. A 402 bp rcfB promoter was fused to upstream of b-galactosidase gene by cloning into promoterless pAK80 plasmid. Promoter activity corresponding to b-galactosidase was measured from Lactococcus lactis IL1403 transformant grown in medium M17 containing different carbon sources. We have found that the rcfB promoter expression was highly induced by acidity and carbohydrate sources used in medium and growth phase of the culture.

    Validity, reliability, and factorial structure of the Turkish version of the patient-reported outcomes in obesity questionnaire

    Background: The prevalence of being overweight and obesity is increasing at an alarming rate throughout the world. Health-related quality of life (HRQoL) is one of the most important issues to be investigated in obesity. The purpose of this study was to investigate the validity, reliability, and factorial structure of the Turkish version of the Patient Reported Outcomes in Obesity questionnaire (PROS).Methods: A total of 80 participants (40 overweight, 40 obese) were included in the study. The Impact of Weight on Quality of Life short version (IWQOL-lite) was used to determine validity. The PROS was readministered to 20 subjects after a 1-week interval for reliability analysis.Results: There was a high correlation between the PROS and IWQOL-lite mean scores (-0.855, p < 0.001). The test-retest reliability showed that the intraclass correlation coefficients (between 0.754 and 0.983) were significant for PROS. Factor analysis demonstrated a three-factor structure.Conclusions: PROS is a practical tool that is up-to-date, short, and easy to understand, implement, and calculate. The results of this study demonstrated that PROS is a valid and reliable questionnaire for measuring HRQoL in an overweight and obese Turkish population.

    Valorization of agricultural by-product: Optimization of alcohol-based extraction of polyphenols from chestnut peel using box-behnken design

    In the present work, solvent concentration, extraction time, and solid to solvent (S/S) ratio were evaluated in order to recover the majority of polyphenols from chestnut peel waste using ethanol and methanol. Extraction method for polyphenols from peel pre-treated by ohmic heating (OH) was optimized using response surface methodology (RSM). The effect of these independent variables on total phenolic content (TPC) and antioxidant capacity (AC) was studied using Box-Behnken Design (BBD). A second-order polynomial model provided a satisfactory fit to the experimental data with a high coefficient of determination (R-2) value. Results showed that S/S ratio and solvent concentration were generally significant variables during extraction in terms of TPC and AC. The optimum extraction conditions were obtained as 1/10 of S/S ratio and 60% of solvent concentration for both solvents. As the optimum extraction time, 82.41 min for ethanolic extraction and 116.97 min for methanolic extraction were selected. Under these optimal conditions, TPC values of the ethanolic and methanolic extracts were found to be 39.02 and 38.79 mg gallic acid equivalents per gram of dry matter (mg GAE/g DM), respectively, thus indicating highly close agreement to the predicted values. Consequently, the effectiveness of the solvents used was found to be very similar to each other. The OH pre-treatment appeared to be a promising technique for polyphenolic extraction from industrial wastes.
