Abstract
Kolon cerrahisinde, en sık görülen ve hastanede kalış süresini önemli ölçüde arttıran yara infeksiyonu sorununun çözümü için uygulanan yöntemler ve önerilen antibiyotikler incelendi. Kolon ameliyatlarında yara infeksiyonundan en iyi korunma: elektif olgularda ; preoperatif mekanik temizlik, oral antibiyotik profilaksisi ve intraoperatif topikal gulamanın birlikte kullanılması ile. acil olgularda ise hemen anestezi öncesi başlanan sistemik antibiyotik profilaksisi ve intraoperatif topikal uygulama ile sağlanabilir. Oral yada sistemik profilakside kullanılacak antibiyotikler anaerob bakterilere özellikle Bakteroides fragilis'e etkili olmalıdır. Bunun için oral profilakside eritromisin yada tetrasiklin önerilir, birlikte aeroblara etkili olan neomisin yada kanamisin de kullanılmalıdır. Sistemik profilakside ise klindamisin ve aminoglikozid kombinasyonu (neomisin, kana misin) yada tek başına üçüncü jenerasyon sefalosporinler (sefobid, sefotaksim) aynı amaca uygundur.
The antibiotics and methods, that can be used for prophylaxis of wound infection, which is most frequently seen in colon surgery were reviewed in this article. In elective colon operations, the combination of oral preoperative and topical intraoperative antibiotics with mechanical cleansing provide the best protection. In emergency operations, in which preoperative administration of oral antibiotics are not possible, the combination of appropriate perioperative systemic antibiotics with intraoperative topical antibiotics should provide greatest protection against infections. The antibiotics which will be used in oral and systemic prophylaxis must be effective against the anaerobic microorganisma, particularly Bacteroides fragilis. The antibiotics that can be recommended for oral administration, are erythromycin or tetracyclin together with kanamycin or neomycin. Third generation cephalosporins (cefobid, cefotaxime) alone or clindamycin and aminoglycosıde (gentamycin, tobramycin) combination are used in systemic prophylaxis.
