Karpal tünel sendromlu hastalarda kortikosteroid fonoforezin etkinliği

Abstract
Amacımız 3 aydan uzun idiyopatik Karpal Tünel Sendromu (KTS) ile ilgili şikayetleri olan bayanlarda, girişimsel olmayan, etkili bir yöntem olduğunu düşündüğümüz kortikosteroid fonoforezin etkinliğini araştırmaktı.Çalışma Uludağ Üniversitesi Atatürk Rehabilitasyon Uygulama Araştırma Merkezinde Nisan 2008 - Şubat 2009 arasında idiyopatik KTS tanısı konmuş, yaşları 32?71 arasında değişen (52.1 ± 1.0), 60 bayan hastanın 102 eliyle yapıldı. KTS tanısı öykü, Tinel ve Phalen testleri ve sinir ileti çalışmaları ile konuldu. Çalışma randomize, kontrollü, tek kör olarak biçimlendirildi. 1. gruba ultrason (US) jeli ile 1 MHz, 1 w/cm2 sürekli akım terapötik US, 2. gruba %0.025'lik beklometazon dipropionat ile 1 MHz, 1 w/cm2 sürekli akım fonoforez, 3. gruba 0 MHz, 0 w/cm2 plasebo US her seansta 5 dakika, toplam 10 seans uygulandı. Gruplar başlangıçta demografik veriler, klinik parametreler ve sinir ileti parametreleriyle, 10 seans tedavi sonrası klinik ve tedaviden 8 hafta sonra klinik ile birlikte sinir ileti parametreleriyle değerlendirildi.Fonoforez grubunda tedavi öncesine göre her iki el Tinel ve sol el Phalen testinde, fonksiyonel durum ölçeğinde, sol el motor ileti amplitüd hariç tüm motor ileti parametrelerinde ve tüm duyusal ileti parametrelerinde anlamlı iyileşme bulundu. US grubunda ise sağ ve sol motor ileti hızı, sağ motor amplitüd, sağ ve sol duyusal ileti amplitüd ve latanslarında anlamlı iyileşme bulundu. Plasebo US grubunda anlamlı değişiklik bulunmadı. Fonoforez motor ve duyusal ileti parametreleri açısından US ve plaseboya üstün bulunduSonuç olarak, KTS tedavisinde US ve fonoforez etkili bulunurken, fonoforezin hem klinik parametrelerde hem de sinir ileti çalışmalarında iyileşme açısından US ve plaseboya üstün olduğu gözlendi.
Our aim was to study the efficacy of corticosteroid phonophoresis which is an uninvasive and effective method on women who have idiopatic Carpal Tunnel Syndrome (CTS) for at least three months.The study was conducted at Uludağ University Atatürk Rehabilitation and Resarch Center between April 2008 - February 2009 on 60 women' 102 hands, between ages 32-71 (52.1 - 1.0). CTS diagnosis was made by patient history, Tinel and Phalen tests and nerve conduction studies. Study was shaped as randomised, controlled and single blinded. The 1st group was treated with 1 MHz, 1 w/cm2 continuous therapotic ultrasound (US) with US gel, the 2nd group was treated with 1 MHz, 1 w/cm2 continuous phonophoresis with 0,025% beclomethasone dipropionate, the 3rd group was treated with 0 MHz, 0 w/cm2 placebo US, all for 5 minutes, totally 10 sessions applied. The groups were assessed with demographic, clinical parameters and nerve conduction studies at the beginning, at the end of 10 sessions and after 8 weeks.In Phonophoresis group, left hand Phalen, both hand Tinel test, functional status, all motor parameters except left hand motor conduction amplitude and all sensory conduction paremeters showed significant improvement. In US group right and left hands motor conduction velocities, right hand motor amplitude, right and left hands sensory conduction amplitudes and latency prameters significantly improved. Phonophoresis was superior to US and plasebo groups for the right and left hand motor and sensory conduction parameters. No changes were observed in placebo group.In conclusion we found that in CTS, phonophoresis and US are effective methods while phonohoresis was more effective at clinical and nerve conduction study parameters.
