Abstract
Sirkadyan tipin diz izokinetik kuvvet ölçüm güvenirliliğine etkisini incelemek amacı ile yapılan bu çalışmaya 18-35 yaşlarında 44 erkek gönüllü katıldı. Çalışmaya katılan gönüllüler insan sirkadyan ritminde sabahçı ve akşamcı tipleri belirleyen anket formunda elde ettikleri toplam puanlara göre 3 farklı "sirkadyan tip"gruba ayrıldı. İzokinetik performans testleri sabahım ve akşamım tekrarlayan ölçümler şeklinde günün farklı zaman dilimlerinde (sabah 08:30-10:00 ve öğleden sonra 16:30 18:00) uygulandı. İzokinetik konsentrik eksentrik ölçümlerde 60° ve 1207san'lik açısal hızda 4, 1807san'lik açısal hızda ise 20 maksimal tekrar yaptırıldı. Tekrarlayan 4 dönem (sabah-akşamı.2) ölçüm değerlerin karşılaştırmasında tek yönlü varyans analizi (ANOVA), güvenirlilik değerlendirmesinde ise "Pearson korelasyon matriks yöntemi" kullanıldı. Yapılan 4 dönem karşılaştırmasına göre (sabahı.2-akşamı.2) vücut sıcaklığı ve kalp vurum sayısı ölçüm değerlerinde istatistiksel olarak anlamlı farklılık gözlemlenmedi. Sirkadyan tipler (ara tip, sabahçı ve akşamcı tip) için konsentrik ve eksentrik kas kuvvet özellikleri açısından tekrarlayan sabaha ve akşamı.2 dört dönem karşılaştırmasında istatistiksel anlamlı (p>0.05) bir farklılık bulunamadı (p>0.05). Aratiplerde konsentrik ekstansör kas kuvvet özellikleri test zamanına bağlı incelendiğinde sabah (r=.83) değeri akşam (r=.76) korelasyon değerine göre yüksek düzeyde idi. Akşam dayanıklılık korelasyon değeri (r=.81) sabah (r=.69) değerinden daha yüksek idi. Konsentrik fleksörlerde ise sabah (r=.70) değeri akşam(r=.82) korelasyon değerine göre düşük düzeyde idi. Dayanıklılık için sabah (r=.20) ve akşam (r=43) düşük düzeyde korelasyon değerleri tespit edildi. Eksentrik ekstansör kas kuvvet özellikleri değerlendirildiğinde ise sabah (r=.69) ve akşam (r=.62) korelasyonları arasında anlamlı farklılık yoktu. Dayanıklılık için sabah (r=.14) ve akşam (r=.43) düşük düzeyde korelasyonlar tespit edildi. Eksentrik fleksörlerde ise sabah (r=.82) değeri akşam (r=.51) korelasyon değerine göre daha yüksek düzeyde idi. Dayanıklılık için sabah (r=. 53) ve akşam (r=.62) orta düzeyde korelasyon tespit edildi. Sabahçı tiplerde konsentrik ekstansör kas kuvvet özellikleri test dönemine bağlı incelendiğinde sabah (r=.70) ve akşam (r=.75) korelasyonları arasında anlamlı farklılık yoktu. Fleksörlerde ise akşam (r^.83) korelasyon değeri sabah (r=.56) korelasyon değerine göre daha yüksek düzeyde idi. Konsentrik ekstansörlerde dayanıklılık akşam (r=.92) korelasyon değeri sabah (r=73) değerine göre yüksek düzeyde tespit edilirken, konsentrik IIIfleksörlerde sabah (r=.47) ve akşam (r=45) düşük düzeyde korelasyon değerleri tespit edildi. Eksentrik ekstansör ve fleksör kuvvet özellikleri zamana bağlı incelendiğinde sırasıyla sabah korelasyon düzeyleri (r=.74 ve.74) akşam korelasyon değerlerine (r=.59 ve.66) göre daha yüksek düzeyde idi. Dayanıklılık için eksentrik ekstansör ve fleksörlerde sırasıyla sabah (r=.48 ve.20) ve akşam (r=.20 ve.29) düşük düzeyde korelasyon tespit edildi. Akşamcı tiplerde konsentrik ekstansör kas kuvvet özellikleri test zamanına bağlı olarak değerlendirildiğinde akşam korelasyon değeri (r=.85) sabah (r=.72) korelasyon değerine göre daha yüksek, fleksörlerde ise sabah (r=.91) korelasyonu akşam (r=.75) korelasyon değerine göre daha yüksek düzeyde idi. Dayanıklılık konsentrik ekstansörlerde akşam korelasyon değeri (r=.75) sabah (r=.46) korelasyon değerine göre yüksek düzeyde iken, konsentrik fleksörlerde dayanıklılık için sabah (r=.44) ve akşam (r=.31) düşük düzeyde korelasyon değerleri tespit edildi. Eksentrik ekstansör genel kuvvet özellikleri akşam korelasyon değeri (r^.72) sabaha (r=.58) göre yüksek düzeyde tespit edilirken, eksentrik fleksörlerde sabah (r=.89) değeri akşam (r=.74) korelasyon değerine göre yüksek düzeyde idi. Eksentrik ekstansörlerde akşam dayanıklılık korelasyon değeri (r=.86), sabah (r=.48) değerinden daha yüksek, eksentrik fleksörlerde ise sabah (r=.55) ve akşam (r=.48) düşük düzeyde korelasyon düzeyleri tespit edildi. Bu bulgular ışığında; 1) istirahat oral vücut ısısı ve kalp vurum sayısı sabah akşam 4 dönem karşılaştırmasına göre diürnal özellik göstermediği, 2) izokinetik konsentrik ve eksentrik kas kuvvet ölçümleri ara tip, sabahçı ve akşamcı tipler için sabah-akşamı.2 tekrarlayan ölçüm karşılaştırmasında istatistiksel olarak farklılık olmadığı, 3) Sirkadyan tiplerde kas tipine bağlı kuvvet özellikleri değerlendirildiğinde sabahçı tiplerde sadece konsentrik ekstansör kuvvet özelliklerinin düşük açısal hızda (607san) sirkadyan özellik gösterdiği, akşamcı tiplerde ise konsentrik ekstansör kuvvet özellikleri akşam korelasyonun sabaha göre yüksek olduğu, konsentrik fleksörlerde sabah-akşam korelasyon düzeyleri arasında anlamlı farklılık olmadığı, 4) eksentrik ekstansör kas kuvvet özelliklerinin sabahçı ve akşamcı tipler için sirkadyan özellik gösterdiği, eksentrik fleksörlerde ise sabahçı tiplerde sirkadyan özellik göstermesine karşın akşamcı tiplerde sirkadyan özellik göstermediği yorumu yapılabilir. Bu konuda daha net ifadeler kullanabilmek için daha geniş denek grupları ile ileri çalışmalara gereksinim vardır.
The purpose of this study to investigate the effect of circadian types on the reliability of knee isokinetic muscle strength measurements. Forty-four male subjects aged 18-35 volunteered for this study. The morningness-eveningness questionnaire was applied-to all participants. They were separated into three groups depending on their total score. The isokinetic tests were performed in four tests sessions morning 1.2 and evenings, on times, (08:30-10:00 and 16:30-18:00). The subjects performed 4 maximal repitations for 60°/sec and 1207sec angular velocities, and 20 maximal repitations for 180°/sec angular velocities. ANOVA used for repeated measurements (morning 1.2 and evening 1.2) and results of tests reliability evaluated using "Pearson correlation matrix" test between the circadian types. The heart rate and body temperature were not significantly different among four measurements by ANOVA (p>0.05). Additionally, isokinetic concentric eccentric muscle strength characteristics were not different significantly among four measurements in circadian types by ANOVA (p>0.05). In intermediate types, the correlations coefficients for concentric extensor muscles strength were higher than both morning (r^.83) and evening (r=.76) values. Additionally, evening endurance ratio correlations (r=.81) were higher than the morning (r=.69) value. The correlations for concentric flexor muscles were lower in the morning (r=.70) than the evening (r=.82) and endurance ratio for the flexor muscles were both poor in the morning (r=.40) and in the evening (r=.43) time. The correlations for eccentric extensor muscle strength were not significantly different between morning (r=\69) and evening (r=.62), and endurance ratio values were poor both in the morning (r=.40) and in the evening (r=.43) time. The eccentric flexor muscle correlation value were higher in the morning (r=.82) than the evening (r=.51), and endurance ratio were poor to moderate in the morning (r=.53) and in the evening (r=.62), respectively. In morning types, the correlations for concentric extensor muscle strength were not significantly different between morning (r=.70) and evening (r=.75).The correlations for concentric flexor were higher in the evening (r=.83) than the morning (r=.56), and endurance ratio for the concentric extensor muscle strenght were higher in the evening (r=.92) than the morning (r=.73); whereas concentric flexor correlations value were poor in the morning(r=.47) and in the evening (r=.45). The correlations of eccentric extansor and fleksor muscle strenght characteristics were higer in the morning (r=.74 and.74, respectively) than the evening (r=.59 and.66, respectively). The results of this study indicated that: 1) The heart rate and body temperature were not significantly different among four measurements, 2) Isokinetic concentric eccentric muscle strength characteristics were no significantly different among four measurements in circadian types, 3) when the muscle types force characteristics evaluated in "morning" types, only concentric extensor force demonstrated circadian rhythm at low angular velocity (607sec), whereas the correaltion values for concentric extensor force in "evening" types was found higher in evening time than the morning; and there was no significant differences between morning and evening correlation values of concentric flexor force 4) In a general interpretation of the results, it is salient to note that the eccentric extansor muscle force characteristics for both morning and evening types demonstrated a circadian rhythm, whereas eccentric flexor muscles demonstrated circadian rhythm only for morning type not for evening one. In order to explain the conclusions better and clearly, further studies with wider subject groups are needed.
