Abstract
Uludağ Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalında Man 1983-Aralık 1989 (6 yıl 10 ay) tarihleri arasında cerrahi girişim uygulanan histopatolojik olarak tanısı konmuş 112 mide kanserli (adenokarsinom) hasta retrospektif olarak incelenmiştir Olguların 52'si (%46.4) rezektabl geri kalan 60'ının (% 53.6) non rezektabl olduğu saptanmıştır. Rezektabl olguların % 62'sine küratif rezeksiyon, kalan % 38'ine palyatif rezeksiyon uygulanmıştır. Non Rezektabl olgulara çeşitli palyatif girişimler uygulanmıştır. Yaşam süresi küratif rezeksiyon yapılanlarda 4 ay - 4.5 yıl, palyatif rezeksiyon yapılanlarda 3-14 ay arasındadır. Palyatif girişim uygulanan non rezektabl olgular postoperatif ilk 5 ayda ölmüşlerdir. Sonuç olarak; rezektabl mide kanserinde, palyatif amaçlı bile olsa rezeksiyonun olabildiğince geniş tutulmasına çaba gösterilmesinin yaşam süresine olumlu katkıda bulunacağı yargısına varılmıştır.
112 cases who underwent operations for gastric cancer, histopathologically diagnosed as adenocarcinoma between March 1983-December 1989 (6 years and 10 months) at the Department of Surgery, University of Uludağ, School of Medicine were analysed retrospectively. 52 cases (46.4 %) were reseelab/e and the remaining 60 cases (53.6 %) were unresectable. Curative resection was performed in 62 % of resectable cases whereas the remaining 38 % received palliative resection. Variety of palliative procedures were performed on unresectable cases. Survival after curative resections is between 4 months - 4.5 years, whereas after palliative resections, the survival is between 3-14 months. Unresectable cases who received palliative operations died in the first 5 months postoperatively. It is concluded that encouragement for extensive resection even for palliative purposes would improve survival rates for resectable gastric cancers.
