Abstract
Evisserasyon ameliyatında küresel porlu polietilen (PP) implant aprimer olarak hareket pimi (HP) yerleştirilen hastalarda ameliyat sonuçları ve komplikasyonlarını değerlendirmek. Gereç ve Yöntem: Bu çalışma Mayıs 2003-Mart 2006 tarihleri arasında evisserasyon ameliyatı geçiren 27 hastayı (14 erkek, 13 bayan, yaş aralığı:10-66 yıl, ortalama yaş: 43 yıl) içerdi. Evisserasyon ön ve arka skleral gevşetici kesilerle birlikte yapıldı. Tüm hastalarda 18 veya 20 mm çaplık üresel PP implant kullanıldı. Hareket pimi 4 mm öne çıkacak biçimde implanta yerleştirildi. Sklera HP'nin etrafını çevirecek şekilde sütüre edildi,Tenon kapsülü ve konjonktiva HP'ni örtecek biçimde kapatıldı. Ameliyattan sonra HP kendiliğinden konjonktival yüzeye çıkmadığında, pimin önündeki konjonktiva açıldı. Postoperatif komplikasyonlar, cerrahi işlemler ve izlem süresi kaydedildi.Sonuçlar: Onbir hastada (% 41) HP postoperatif 1-8. haftada kendiliğinden konjonktiva yüzeyine çıktı. On iki hastada (% 44) konjonktiva açılarak HPortaya çıkartıldı. Dört hastada (% 15) HP konjonktiva altında bırakıldı. Erkenpostoperatif dönemde 10 hastada (% 37) toplam 15 komplikasyon gelişti.Bunlar, hafif pim desantralizasyonu (% 19), ağır pim desantralizasyonu (% 7),preseptal sellülit (% 4), konjonktiva prolapsusu (% 11) ve konjonktiva kistiydi(% 15). Dört hasta (% 15) komplikasyon onarımı için ikincil cerrahi gerektirdi. Yirmi bir hastaya (% 78) HP'yle birleşen bir göz protezi takıldı. Üç hastaya (%11) HP'yle birleşmeyen bir protez takıldı. Protez taktıktan sonra, 12 hastada(% 50) toplam 18 komplikasyon kaydedildi. Bunlar, soket akıntısı (% 50),implant açılımı (% 17), implant enfeksiyonu (% 4) ve hafif pimdesantralizasyonuydu (% 4). Bu komplikasyonlar nedeniyle 5 hasta (% 21)ii ikincil bir cerrahi tedavi geçirdi. İzlem süresi 3 ile 33 ay arasında değişti(ortalama, 14 ay).Tartışma: Bu çalışmada, evisserasyon ameliyatı sırasında küresel PP implanta HP konan hastalarda önceki çalışmalara kıyasla daha yüksek oranda komplikasyona rastlanmıştır. Evisserasyon sırasında HP konan hastalar olası komplikasyonlar açısından yakından izlenmelidir.
To evaluate the outcome and complications of surgery in patientswho underwent evisceration surgery with primary placement of motilitycoupling post (MCP) in the spheric porous polyethylene (PP) implant.Materials and Method: This study included 27 patients ( 14 males, 13females; age range:10-66 years, mean age:43 years ) who underwentevisceration surgery between May 2003-March 2006. Evisceration wasperformed with anterior and posterior scleral relaxing incisions. Spherical PPimplants of 18 mm or 20 mm were used in all patients. The MCP extruded 4mm from the implant. The sclera was sutured as encircling the MCP, then thepost was covered with the Tenon capsule and conjunctiva. The conjunctiva infront of the MCP was excised when it couldn?t extrude by itself from theconjunctival surface. Postoperative complications, surgical procedures andfollow up period were recorded.Results: The MCP was extruded by itself from the conjunctiva atpostoperative 1st-8th weeks in 11 patients (41 %). In 12 patients (44 %), theconjunctiva was incised and the MCP was exposed. In 4 patients, the MCPwas left buried under the conjunctiva. At early postoperative period, a total of15 complications developed in 10 patients (37 %). These were minimal postdecentration (19%), severe post decentration (7 %), preseptal cellulitis (4 %),conjunctival prolapsus (11 %) and conjunctival cysts (15 %). Secondarysurgery was required in 4 patients (15 %) for relief of the complications. In 21patients, (78 %) a post-coupled prosthesis was fitted. A prosthesis notcoupled with the MCP was fitted in 3 patients (11 %). After fitting an ocularprosthesis, 18 complications were recorded in 12 (50 %) patients. Thesewere socket discharge (50 %), implant extrusion (17 %), implant infection (4iv%) and minimal MCP decentration (4 %). Five patients (21 %) underwent asecondary surgery because of these complications. Follow up rangedbetween 3 and 33 months (mean, 14 months).Discussion: In this study, we encountered a higher rate of complications onthe patients who underwent primary MCP placement in the spheric PPimplants during the evisceration surgery than the previous studies. Patientswho underwent MCP placement at the time of evisceration should be closelyfollowed for the possible complications.
