Abstract
Bu çalışmada diz OA'li hastalarda burst tipi TENS tedavisinin ağrı, fonksiyonel performans ve yaşam kalitesi gibi klinik parametrelere olan etkisini ve plaseboya üstün olup olmadığını göstermeyi amaçladık.Yaşları 46-72 arasında değişen 50 hasta çalışmaya alındı. Hastalar iki gruba randomize edildi (Grup 1; burst tipi TENS+ev egzersiz programı, Grup 2; plasebo TENS+ev egzersiz programı). TENS tedavisi günde 15 dakika, haftada 5 gün 2 hafta süreyle uygulandı. Hastalar ev egzersiz programını 2 hafta süreyle günde 2 defa gerçekleştirdi. Tedavilerin etkinliği 2. hafta ve3. ayda değerlendirildi. Sonuçlar tedavi öncesi karşılaştırıldı. Ağrı değerlendirmesi için Vizüel Analog Skala (VAS), fonksiyonların değerlendirilmesi için WOMAC ve yaşam kalitesinin değerlendirilmesi için SF?36 kullanıldı.Her iki tedavi yöntemi diz osteoartritli hastalarda etkili bulundu. Bununla birlikte, fark skorları ve yüzde değişimleri açısından gruplar arasında yapılan karşılaştırmada, tedavi sonrası 2. haftada WOMAC A (p=0.035) değerinde grup 1 lehine, SF-36 sosyal fonksiyon (p=0.038) değerinde grup 2 lehine anlamlı farklılık saptandı. 3 ay değerlendirmede gruplar arası anlamlı fark saptanmadı.Sonuç olarak diz osteoartritli hastalarda burst tipi TENS uygulaması ile ağrı, fonksiyonel performans ve yaşama kalitesinde düzelmeler saptanmasına rağmen plaseboya üstünlüğü gösterilemedi.
This study aims to show the effect of burst type TENS treatment on clinical parameters like pain, functional performance and quality of life on patients with knee osteoarthritis and whether or not it is superior to placebo.The study was carried out on 50 patients aged between 46 and 72. The patients were randomized into two groups (Group 1: burst-type TENS + home exercise program; Group 2: placebo TENS + home exercise program). TENS treatment was applied for 15 minutes per day, 5 days per week over a 2-week period. The patients performed the home exercise program twice a day for 2 weeks. The effectiveness of the treatments was evaluated at the 2nd week and the 3rd month. The results were compared with the pre-treatment state. Visual Analogue Scale (VAS) was used for the evaluation of pain, WOMAC for the evaluation of functions, and SF-36 for the evaluation of quality of life.Both treatment methods were effective on patients with knee osteoarthritis. Nevertheless, the comparison of the two groups in terms of difference scores and percentage changes, at the 2nd week after the treatment, a significant difference was determined in WOMAC A (p=0.035)value in favour of group 1; and in SF-36 social function (p=0.038) value in favour of group 2. No significant difference was determined between the groups in the evaluation at the 3rd month.In conclusion, even though improvements were determined in pain, functional performance, and quality of life of patients with knee osteoarthritis with the burst type TENS and placebo TENS treatments, burst type TENS proved no superiority over placebo TENS.
